BACKGROUND: We previously showed that Lactobacillus casei cell wall extract-induced Kawasaki disease (KD) vasculitis significantly accelerates atherosclerosis in hypercholesterolemic mice on high-fat diet. Here, we investigated the contribution of IL-1 (interleukin-1) signaling on vascular smooth muscle cells in this model.
METHODS: Tamoxifen-inducible vascular smooth muscle cell-specific IL-1 receptor (Il1r1) knockout (SMCΔ/Δ) mice and Il1r1SMCWT/WT [wildtype/wildtype] littermate controls, all on ApoE-/- background, were injected with either PBS or Lactobacillus casei cell wall extract. Two …

BACKGROUND: Anti-endothelial cell antibodies (AECA) are implicated in vascular injury associated with autoimmune and inflammatory diseases. However, their distribution and clinical significance across diverse conditions remain inadequately characterized.
METHODS: We analyzed 1,334 serum samples from patients with vasculitis, systemic autoimmune diseases, kidney diseases, arthritis, and other immune-mediated disorders. AECA titers were measured using indirect immunofluorescence assays and categorized as low (1:100), moderate (1:320) or high (1:1000). Laboratory parameters, including hematologi…

BACKGROUND: Frosted branch angiitis is a rare, severe condition. It can be either a primary or a secondary condition and is characterized by rapid deterioration of vision and fulminant retinal vasculitis that manifests as diffuse sheathing of retinal vessels, macular edema, papillitis, vitritis and anterior uveitis. We aimed to describe a case of frosted branch angiitis and cerebral venous sinus thrombosis as an initial neuro-Behçet's disease onset. Diagnosis of Behçet's disease was based on the current 2014 International Criteria for Behçet's Disease and the International consensus recommenda…

BACKGROUND: Cryoglobulinaemic vasculitis (CV) is a lymphoproliferative disorder related to hepatitis C virus (HCV) infection; anti-viral therapy is the first therapeutic option. CV can be incapacitating, compromising the patients' quality of life (QoL). In a controlled study, interferon-based therapy was associated with a lower virological response in vasculitic patients than in patients without vasculitis. Limited, uncontrolled data on direct-acting anti-virals are available.
AIM: To evaluate safety, clinical efficacy, virological response and the impact of interferon-free treatment on QoL i…

We describe the clinical presentation and outcome in a series of eight patients with systemic necrotizing vasculitis and severe alpha1-antitrypsin (AAT) deficiency followed up at three Swedish hospitals during 1968-92. We also review six other cases reported in the literature during the same period. Diagnosis of severe AAT deficiency was based on the presence of the PiZZ phenotype, or low plasma total trypsin inhibitory capacity, or a low plasma AAT concentration (10-40% of the normal mean value) and presence of the PiSZ or PiFZ phenotype. The diagnosis of systemic vasculitis was biopsy-verifi…

Acute mesenteric ischaemia is a life-threatening cause of abdominal pain caused by sudden intestinal hypoperfusion. While most cases are related to cardio-embolic events or thrombosis of a pre-existing atherosclerosed vessel in patients with significant cardiovascular risk factors, rarely, it may be the initial manifestation unmasking an underlying systemic vasculitis. We describe a case of polyarteritis nodosa causing mesenteric ischaemia, based on the presence of an inflammatory state and characteristic radiological findings of medium-vessel vasculitis. After treatment with high-dose intrave…

We herein report the first case of dyspnea with hemidiaphragm elevation in a 68-year-old woman with active giant cell arteritis (GCA), including successful treatment. Contrast-enhanced computed tomography showed a reduced density of the left ophthalmic artery and the left superficial temporal artery with increased soft tissue compared to the other side, indicating that the GCA had flared up and suggesting that the hemidiaphragm elevation might be caused by vasculitis-associated ischemia of the right phrenic nerve. Hemidiaphragm paralysis due to vasculitis-associated ischemia in patients with G…

Illicit and recreational drugs, such as cocaine, heroin, amphetamines, and marijuana, can result in drug-related vasculitis or vasculopathy. Similarly, the use of certain antithyroid, oncologic, and immunosuppressive medications for therapeutic purposes can lead to vasculopathy. This in turn may result in significant complications in the central nervous system, including intracranial hemorrhage and stroke. Cocaine abuse can also lead to midline destructive lesions of the sinonasal complex. MR imaging, Vessel Wall imaging, and CT/CTA are valuable imaging tools for the evaluation of patients wit…

Deficiency of adenosine deaminase 2 (DADA2) is a monogenic vasculitis syndrome caused by biallelic mutations in the adenosine deaminase 2 gene. The diagnosis of DADA2 is confirmed by decreased enzymatic activity of ADA2 and genetic testing. Symptoms range from cutaneous vasculitis and polyarteritis nodosa-like lesions to stroke. The vasculopathy of DADA2 can affect many organ systems, including the gastrointestinal and renal systems. Hematologic manifestations occur early with hypogammaglobulinemia, lymphopenia, pure red cell aplasia, or pancytopenia. Treatment can be challenging. Tumor necros…

Varicella-zoster virus (VZV) vasculopathy is a granulomatous vasculitis that has a wide variety of clinical presentations. It is most common in patients with HIV not on anti-retroviral therapy (ART) with low cluster of differentiation (CD)4 cell counts. This disease affects the central nervous system and can cause small intracranial bleeds. Our patient presented with stroke-like symptoms in the setting of recent VZV reactivation in the ophthalmic distribution with HIV on ART. Her MRI scan showed a small punctate bleed and the CSF workup was consistent with VZV vasculitis. She was treated with …
