Paired tone facilitation in dorsal cochlear nucleus neurons: a short-term potentiation model testable in vivo.

It has been suggested that the dorsal cochlear nucleus (DCN) is involved in coding stimulus history or prior auditory activity [Manis (1989) J. Neurophys., 61, 149-161; Manis (1990) J. Neurosci., 10, 2338-2351]. The major output neurons of the DCN are the fusiform (pyramidal) cells which are thought to receive excitatory inputs from the descending branch of the acoustic nerve onto their basal dendrites and significant inhibitory glycinergic and GABAergic inputs to the soma and dendrites. The apical dendrites of these neurons lie within the molecular layer of the DCN and encounter parallel fibe

Dose prescription and dose-volume effects in radiosurgery.

The optimal use of radiosurgery as a treatment technique requires thorough planning, including careful fitting of the high-dose treatment volume to the target volume, and an understanding of the effects of high-dose single-fraction irradiation on both the target volume and the surrounding normal brain. The integrated logistic formula appears to be useful as an aid for predicting the risks of complications from radiosurgery, but greater understanding of the radiation tolerance for all the different areas of the brain (particularly the cranial nerves) is needed. The risk of developing MR imaging

Photodynamic therapy using pheophorbide a and Nd:YAG laser.

The authors describe a new photodynamic therapy (PDT) method for malignant brain tumors. Pheophorbide a (Ph-a), the photosensitizer, has low toxicity, causes no skin sensitization and is activated with an acoustic Q switched neodymium yttrium-argon-garnet (Nd:YAG) laser which achieves deep tissue penetration. The Ph-a distribution in Fisher 344 (F344) rats bearing rat T9 glioma at 24 hours after intravenous injection was very low in the normal brain tissue, but significantly higher in the T9 glioma giving a tumor to normal brain tissue concentration ratio of 7.5:1. The in vitro survival rate o

Molecular biology of brain tumors.

Through the technical advances in molecular biology during the past decade, important new insights into the fundamental chromosomal changes associated with brain tumors have been gained. The pace of such research is accelerating, and most of the published reports have appeared outside the neurosurgical literature. Furthermore, many neurosurgeons may not be sufficiently familiar with the terminology and techniques involved to remain abreast of the field. In this review, we discuss through specific examples of recent work on brain tumors the basic techniques of molecular biology, including the S

Transtragal, transtympanic electrode placement for intraoperative electrocochleographic monitoring.

Transtragal, transtympanic electrode placement was performed for the purpose of intraoperative electrocochleographic (ECoG) monitoring during seven suboccipital acoustic neuroma resections. The promontory, the tragus, and two external sutures stabilized the electrode during surgery. The only noted otologic sequallae of electrode placement were small circular lesions at tympanic membrane puncture sites. Satisfactory ECoG recordings were obtained in five of seven cases. Peak-to-peak N1 amplitudes were (on average) 13.4-fold larger than the corresponding amplitudes of peak I of the surface-record

[Diagnostic reliability of ipsilateral evoked impedance changes of the tympanic membrane (author's tranl)].

The ipsilateral registration of impedance change is necessary in case of unilateral cochlear or retrocochlear disease with middle ear impairment of the other ear and, therefore, impossibility of contralateral evocation of the reflex. If contralateral evocation fails, moreover, it will be useful in order to clarify, whether the evocation did not succeed or the registration at the contralateral ear; thus enabling a differentiation between lesions of the reflexe's afferent and efferent way. In ipsilateral impedance-change-measurements it should be distinguished between an increase of the acoustic

Forward and Reverse Middle Ear Transmission in Gerbil with a Normal or Spontaneously Healed Tympanic Membrane.

Tympanic membranes (TM) that have healed spontaneously after perforation present abnormalities in their structural and mechanical properties; i.e., they are thickened and abnormally dense. These changes result in a deterioration of middle ear (ME) sound transmission, which is clinically presented as a conductive hearing loss (CHL). To fully understand the ME sound transmission under TM pathological conditions, we created a gerbil model with a controlled 50% pars tensa perforation, which was left to heal spontaneously for up to 4 weeks (TM perforations had fully sealed after 2 weeks). After the

Effects of exposure to noise during perceptual training of non-native language sounds.

Listeners manage to acquire the sounds of their native language in spite of experiencing a range of acoustic conditions during acquisition, including the presence of noise. Is the same true for non-native sound acquisition? This study investigates whether the presence of masking noise during consonant training is a barrier to improvement, or, conversely, whether noise can be beneficial. Spanish learners identified English consonants with and without noise, before and after undergoing one of four extensive training regimes in which they were exposed to either consonants or vowels in the presenc

Risk factors for distortion product otoacoustic emissions in young men with normal hearing.

The purpose of this study was to evaluate the possible effects of risk factors on distortion product otoacoustic emissions (DPOAEs) in young adult men with normal hearing. Four hundred thirty-six United States Marine recruit men (mean age = 19.2 years +/- 1.8 years; age range = 17-29 years) participated in this study. Questionnaires were given to each recruit to obtain demographic data and history of noise exposure, solvent exposure, smoking history, and hearing-related histories. Otoscopy, tympanometry, pure-tone air-conduction audiometry (2.0-8.0 kHz) and DPOAEs (2.3-8.0 kHz) were measured.

Audiologic findings in botulism poisoning.

Botulism poisoning results in a presynaptic failure of neuromuscular transmission, and may be expected to impair Eustachian tube and stapedius reflex function. The audiologic findings in five normal-hearing patients with type A botulism poisoning are presented. Results were varied. Two patients demonstrated type B or C tympanograms during the early stages, while three with normal middle ear function demonstrated marked bilateral or unilateral stapedius reflux threshold elevation. One patient had a reversible reflex decay, and the superimposition of a second stimulus upon the first resulted in

Non-invasive assessment of ventricular shunt function using tympanic membrane displacement measurement technique.

A technique which was originally developed for measuring cochlear fluid pressure, has been under trial for the past two years as a means of assessing ventricular shunt function. The principle of the technique has been reported in previous papers (1, 2, 3) and has been shown to provide a reliable measure of intracranial pressure (ICP) in terms of induced displacement of the tympanic membrane. This study concentrates on the assessment of shunt blockage in spina bifida patients who subsequently underwent shunt revision. The tympanic displacement technique is shown to be of value to this patient g

Sensitivity and specificity of audiological tests in patients with vertigo.

In patients with vertigo a comprehensive test battery has been used for localizing pathological processes responsible for the symptoms. Audiological tests have been an integral part of the otoneurological investigation. However, in many studies the diagnostic sharpness of psychoacoustic tests in the localization of specific disease in the cranial nerve system has been called in question. Other tests, such as stapedius reflex and auditory brainstem response, have been of increasing importance in the diagnostic set-up. The purpose of this paper is to present a survey of the literature regarding

Electrodiagnostic evaluation of deafness in two English setter littermates.

Two English Setter littermates (male and female) were evaluated for hearing difficulties at 4, 16, and 24 months of age. Auditory function was evaluated by behavioral response to pure-tone sounds, tympanometry, brain stem auditory evoked response, and acoustic reflex testing. Hereditary sensorineural deafness was diagnosed in both dogs. The extent of the hearing deficit, as characterized by these tests, was slightly different between the dogs, as well as between the ears in 1 dog.

Kissing Cochlea: An Extreme Inner Ear Malformation, Novel Surgical Approach via Cystic Vestibule.

In cases of extreme inner ear anomalies such as cochleas located adjacent to each other in the midline due to a fusion defect and a single, vertical internal acoustic canal, and internal carotid artery and facial nerve dehiscence in the middle ear that make conventional cochlear implant surgery impossible, cochlear implantation can be successfully performed with the help of endoscopes via the cystic vestibule approach.

Effects of Non-traumatic Noise and Conductive Hearing Loss on Auditory System Function.

The effects of traumatic noise-exposure and deafening on auditory system function have received a great deal of attention. However, lower levels of noise as well as temporary conductive hearing loss also have consequences on auditory physiology and hearing. Here we review how abnormal acoustic experience at early ages affects the ascending and descending auditory pathways, as well as hearing behavior.

[Use of instrumental vocal analysis in phoniatrics].

The growing need for quantitative evaluation of health care intervention along with the pitfalls of perceptual analysis is leading clinicans to utilise different instrumental vocal analysis methods including acoustic and aerodynamic methods. The fact that perceptual analysis is most often presented as the gold standard for vocal evaluation remains paradoxical. The authors present the use of aerodynamic measurements in particular oral airflow and subglottic pressure.

Amygdala dysfunction in narcolepsy-cataplexy.

The blink reflex of acoustic startle reflex (ASR) is modulated by emotions and a loss of physiological aversive ASR potentiation is reported in humans following amygdala lesions. Patients with narcolepsy-cataplexy (NC) were found to have normal ASR, but they failed to exhibit startle potentiation during unpleasant stimuli. This absence of aversive ASR potentiation gives support to the hypothesis of an amygdala dysfunction in human NC.
