Early transcriptional states of spermatogonia and marker expressions in the prepubertal human testis following chemotherapy-induced depletion.

STUDY QUESTION: Which spermatogonial differentiation states are present in prepubertal testes under normal conditions and following chemotherapy-induced depletion of spermatogonia in paediatric patients with cancer? SUMMARY ANSWER: Single-cell transcriptomic analysis reveals that only undifferentiated spermatogonia are present in prepubertal boys, while differentiated states emerge during puberty, with reduced protein expression of advanced spermatogonial markers observed in younger patients, those treated with alkylating agents, or those with a diminished spermatogonial pool. WHAT IS KNOWN

Fractionated Intermediate-Dose Etoposide Plus G-CSF for Peripheral Blood Stem Cell Mobilization in Patients With Lymphoma and Multiple Myeloma Undergoing Autologous Stem Cell Transplantation: A Prospective Observational Study.

Efficient and safe mobilization of peripheral blood CD34+ stem cells is essential for successful autologous stem cell transplantation (ASCT) in patients with lymphoid malignancies. However, current mobilization strategies are often limited by suboptimal yield, significant toxicity, or high cost. We conducted a prospective observational study including 59 patients with lymphoma or multiple myeloma undergoing ASCT. Patients received fractionated intermediate-dose etoposide (175 mg/m2 twice on days 1 and 2) followed by granulocyte colony-stimulating factor (G-CSF, 10 μg/kg daily). Mobilization ef

Mesenchymal stromal cell therapy for post-COVID-19 syndrome: associated impact and mechanism.

BACKGROUND: Post-COVID-19 syndrome (PCS) affects around 10% of individuals who experience SARS-CoV-2 infection worldwide. This four-year follow-up study investigated the long-term impact of mesenchymal stromal cell (MSC) therapy on both the occurrence of PCS and its underlying mechanisms. METHODS: We enrolled 148 survivors of severe COVID-19 (92 who received MSC therapy and 56 who served as controls). The clinical evaluations included safety (new-onset comorbidities, oncogenic risk) and efficacy outcomes (computed tomography [CT] imaging, functional capacity, pulmonary function, 36-Item Short-

Regenerative therapies for upper limb functional recovery after stroke: study protocol of a randomized clinical trial.

BACKGROUND: Stroke-related long-term disability is primarily due to impaired motor function. Rehabilitation efforts have traditionally focused on central strategies while ignoring the affected muscles. Regenerative medicine approaches have emerged as a promising option for treating various conditions, including muscular disorders. The present study aims to compare the effects of intramuscular injections of mesenchymal stromal cells (MSCs) and Platelet-rich plasma (PRP) on motor recovery in poststroke survivors. METHODS: A single-blind, randomized, controlled trial will be followed. Fifteen st

Long-term outcomes of mesenchymal stem cell therapy in severe COVID-19 patients: 3-year follow-up of a randomized, double-blind, placebo-controlled trial.

BACKGROUND: The long-term effects and outcomes of human mesenchymal stem cell (MSC) therapy in patients with severe coronavirus disease 2019 (COVID-19) remain poorly understood. This study aimed to evaluate the extended safety and efficacy of MSC treatment in severe patients with COVID-19 who participated in our earlier randomized, double-blind, placebo-controlled clinical trial, with follow-up conducted over 3 years. METHODS: One hundred patients with severe COVID-19 were randomized to receive either an MSC infusion (n = 65, 4 × 107 cells/dose, on days 0, 3, and 6) or a placebo, with both gr

Donor-specific mesenchymal stem cell infusion in human and nonhuman primate kidney transplantation.

We report the results of 2 independent, concurrently performed studies evaluating the safety and efficacy of donor-derived mesenchymal stromal cell (MSC) infusions in inducing immune-tolerance in nonhuman primate (NHP) and human kidney transplant recipients treated with depletional induction and belatacept/sirolimus maintenance. Fifteen NHPs received rhesus ATG induction and were divided into 3 groups: control (n = 6), pretransplant thymic irradiation (n = 4), and thymic irradiation with monthly donor-MSC infusion (n = 5). Sirolimus was discontinued at day-180, and belatacept at day-365 posttr

Raising awareness may increase the likelihood of hematopoietic stem cell donation: a nationwide survey using artificial intelligence.

BACKGROUND: In Italy, the demand for allogeneic transplantation exceeds the number of compatible donors in the Italian Bone Marrow Donor Registry (IBMDR). This study aimed to explore the knowledge, beliefs, opinions, values, and feelings of the Italian population regarding stem cell donation. METHODS: An online survey was shared via social media. Respondents were retrospectively identified as registered on the IBMDR (donor group) or never registered (non-donor group). Statistical analyses confirmed the relationship between knowledge level and willingness to donate. Six machine learning classi

Unlocking the promise of mesenchymal stem cells and extracorporeal photopheresis to address rejection and graft failure in intestinal transplant recipients.

INTRODUCTION: In patients with irreversible intestinal failure, intestinal transplant has become a standard treatment option. Graft failure secondary to acute or chronic cellular rejection continues to be a significant challenge following transplant. Even with optimal immune suppression, some patients continue to struggle with refractory rejection. Both extracorporeal photopheresis (ECP) and extracellular vesicles derived from mesenchymal stem cells (EVs) have been used to treat refractory rejection following intestinal transplantation, although their use remains limited and consistent treatme

Optimized peripheral blood progenitor cell mobilization for autologous hematopoietic cell transplantation in children with high-risk and refractory malignancies.

BACKGROUND: Autologous hematopoietic stem cell transplantation (aHSCT) using hematopoietic progenitor cells (HPCs) has become an important therapeutic modality for patients with high-risk malignancies. Current literature on standardized method for HPC apheresis in children is sparse and failure rate reported as high as 30%. PATIENTS/METHODS: A retrospective study of 125 pediatric patients with high-risk malignancies undergoing aHSCT in Western Australia between 1997 and 2016 was conducted. RESULTS: Mobilization was achieved by means of chemotherapy and granulocyte colony-stimulating factor (

Prospective randomized comparative observation of single- vs split-dose lenograstim to mobilize peripheral blood progenitor cells following chemotherapy in patients with multiple myeloma or non-Hodgkin's lymphoma.

In patients with hematologic malignancies, granulocyte colony-stimulating factor (G-CSF) following chemotherapy is widely used to mobilize peripheral blood progenitor cells (PBPCs), but there have been no trials comparing schedules of G-CSF following chemotherapy. We conducted a prospective randomized comparative observation of the mobilization with a single dose (10 microg kg once a day) or split dose (5 microg kg twice a day) of lenograstim following chemotherapy in 25 multiple myeloma (MM) and 15 non-Hodgkin's lymphoma (NHL) patients. Chemotherapy was cyclophosphamide 4 g/m2 for MM and ESHA

Surgical management of complex perianal fistula revisited in a systematic review: a critical view of available scientific evidence.

BACKGROUND: Treating complex perianal fistulas in Crohn's disease patients remains a challenge. Classical surgical treatments for Crohn's disease fistulas have been extrapolated from cryptoglandular fistulas treatment, which have different etiology, and this might interfere with its effectiveness, in addition, they increase fecal incontinence risk. Recently, new surgical techniques with support from biological approaches, like stem cells, have been developed to preserve the function of the sphincter. We have performed a systematic literature review to compare the results of these different tec

Effectiveness of 2-Dose Vaccination with mRNA COVID-19 Vaccines Against COVID-19-Associated Hospitalizations Among Immunocompromised Adults - Nine States, January-September 2021.

Immunocompromised persons, defined as those with suppressed humoral or cellular immunity resulting from health conditions or medications, account for approximately 3% of the U.S. adult population (1). Immunocompromised adults are at increased risk for severe COVID-19 outcomes (2) and might not acquire the same level of protection from COVID-19 mRNA vaccines as do immunocompetent adults (3,4). To evaluate vaccine effectiveness (VE) among immunocompromised adults, data from the VISION Network* on hospitalizations among persons aged ≥18 years with COVID-19-like illness from 187 hospitals in nine

Adult disease burden in patients with mucopolysaccharidosis type I H (Hurler syndrome): A comprehensive literature review with patient case analysis.

INTRODUCTION: Mucopolysaccharidosis type I H (MPS IH; Hurler syndrome) is a rare autosomal recessive disease that causes deficiency of alpha-L-iduronidase - an enzyme responsible for catabolizing glycosaminoglycans (GAGs). While historically considered a pediatric disease with an overall median life expectancy of 8.7 years, advances in MPS IH treatment options have extended patient lifespans into adulthood; however, long-term progression of MPS IH currently remains minimally described. This study aims to provide a comprehensive literature review about the adult disease burden of MPS IH as vali
