Nocturnal hemoglobin desaturation in chronically transfused adults with sickle cell disease: a retrospective study.

High-risk SCD may be managed with chronic red blood cell exchange transfusions. We examined the prevalence of sleep-associated hypoxemia (≥5 minutes at SpO2 ≤88%) or nocturnal Hb desaturation (NHD) in chronically transfused adults with SCD. Of 41 identified participants, 15 (36.6%) had tested positive for NHD at some point prior to enrollment. The median duration of desaturation (SpO2 ≤88%) in those that tested positive was 88.3 minutes (IQR 27.9-226.0 minutes). Participants with and without NHD were compared using non-parametric statistical tests. Compared to participants without NHD, those w

A prophylactic TXA administration effectively reduces the risk of intraoperative bleeding during open management of pelvic and acetabular fractures.

This study aimed to evaluate the efficacy of perioperative intravenous TXA in reducing blood loss in pelvic and acetabular fracture patients managed surgically. The study included 306 consecutive patients, divided as: group I, 157 patients who did not receive perioperative infusion of TXA and group II, 149 patients who received perioperative TXA. The perioperative blood test results and complication rates were compared between the two groups. The average perioperative hematocrit was higher during the preoperative period than during the first, second and third postoperative day in both groups.

An Outbreak of Japanese Encephalitis Virus in Australia; What Is the Risk to Blood Safety?

A widespread outbreak of Japanese encephalitis virus (JEV) was detected in mainland Australia in 2022 in a previous non-endemic area. Given JEV is known to be transfusion-transmissible, a rapid blood-safety risk assessment was performed using a simple deterministic model to estimate the risk to blood safety over a 3-month outbreak period during which 234,212 donors attended. The cumulative estimated incidence in donors was 82 infections with an estimated 4.26 viraemic components issued, 1.58 resulting in transfusion-transmission and an estimated risk of encephalitis of 1 in 4.3 million per com

Epstein-Barr Virus Epidemiology, Serology, and Genetic Variability of LMP-1 Oncogene Among Healthy Population: An Update.

The Epstein-Barr virus (EBV) is a DNA lymphotropic herpesvirus and the causative agent of infectious mononucleosis. EBV is highly prevalent … atent infection in human B-cells. However, it can be transmitted through organ transplantations and blood transfusions. In addition, symptoms of EBV infection are not distinguishable from other viral infections, and therefore, it remains questionable whether there is a need to screen for EBV prior to blood transfusion. Although the process of leukoreduction decreas …

Endoscopic craniectomy for early correction of craniosynostosis.

Twelve patients between 0.4 and 7.8 months of age were treated by an endoscopic approach to strip craniectomy. Nine patients had sagittal suture involvement. Two patients had a single unilateral lambdoid suture synostosis, and one patient had a combination of a right coronal synostosis and a metopic synostosis. Postoperatively, all patients were placed in cranial remodeling helmets and the results showed that the estimated blood loss ranged from 5 cc to 150 cc, with blood transfusion required in only one patient. All patients were discharged from the hospital by day 2, and all patients had an …

Diagnosis and epidemiology of transfusion-associated human cytomegalovirus infection: recent developments.

OBJECTIVE: The prevention and diagnosis of transfusion-associated HCMV infection has to address different questions: Is the blood donor potentially infections? Is the transfused recipient undergoing an active primary or secondary infection? DATA SOURCES: International literature (Current Contents Life Sciences) and reports of our study-group. SELECTION CRITERIA: Original articles in English and German. RESULTS: Monocytes have been identified as the major site of latency of human cytomegalovirus (HCMV) in peripheral blood. High seropositivity rates are observed in polytransfused individuals,

The development of the acquired immunodeficiency syndrome after bone-marrow transplantation.

Since bone-marrow transplant recipients receive considerable quantities of packed-cell, platelet and sometimes leukocyte transfusions, as well as the donor marrow infusion, it would be predictable that acquired immunodeficiency syndrome (AIDS) by blood-product transfusion would occur in this patient population. We report here two patients who received HLA-identical sibling bone-marrow transplants for acute non-lymphoblastic leukaemia during their first remission. Both developed category-A AIDS at days 342 and 546 after transplantation, respectively. Neither patient belonged to any known high-r

Blood Group Discrepancies at a Regional Blood Center.

Background: Blood group testing is an important part of supplying safe blood components in blood transfusion centers. Blood group discrepancy develops when reactions in forward grouping do not correspond with reverse grouping or if the preceding and recent results do not match. This study aimed to evaluate ABO blood group discrepancies among blood donors of Yazd, Iran. Materials and Methods: In this cross-sectional study, data of blood donors were obtained from the integrated database of Yazd Blood Transfusion Center during a period of eight years (2010 - 2017). Tube testing was used for deter…

Trauma-induced capillary leak syndrome after penetrating chest injury: Manifestation of massive ascites and pulmonary secretions aggravated by transfusion.

Trauma with prolonged shock can cause systemic capillary leak syndrome regardless of the site of injury and a transfusion can aggravate it. The systemic capillary leak induces both an abdominal compartment syndrome and pulmonary edema, and a transfusion can aggra-vate these sequelae within hours. In our case, 21-year-old man with a penetrating injury in his left thorax experienced delay in rescue and definitive surgery. To manage life-threatening shock, massive blood transfusion and crystalloids had been infused. Cardiopulmonary cerebral resuscitations were performed 2 times during the surgery…

Liver transplantation of a patient with extreme thrombocytopenia - A case report.

BACKGROUND: Patients with chronic liver disease (CLD) planned for liver transplantation (LT) often show severe thrombocytopenia, but there is a lack of evidence in deciding the threshold for prophylactic platelet transfusion. CASE: A 47-year-old female with acute liver failure was referred for LT. Despite daily transfusion of platelets, platelet counts remained under 10,000/µl. During LT, 2 units of single donor platelets (SDP) were transfused. Although platelet counts remained extremely low (3,000-4,000/µl) no diffuse oozing was observed and the blood loss was 860 ml. Postoperatively, there

Base deficit is superior to lactate in trauma.

BACKGROUND: Base Deficit (BD) and lactate have been used as indicators of shock and resuscitation. This study was done to determine the association of BD and lactate and to determine if one is superior. METHODS: A retrospective review from 3/2014-12/2016 was performed. Data included demographics, systolic BP, ISS, BD, lactate, blood transfusion, and outcomes. BD and lactate were modeled continuously and categorically and compared. RESULTS: 1191 patients were included. BD and lactate correlated strongly (r = -0.76 p < 0.001). Higher lactate and more negative BD were associated with transfusio…

Current risk for transfusion transmitted infections.

PURPOSE OF REVIEW: Blood safety is a topic of continuing concern, and much effort is expended on measures to decrease the risk for transmission of infectious agents via transfusion. At the same time, emerging infections may threaten this safety. A periodic review of risk is therefore appropriate. RECENT FINDINGS: The risk for major transfusion transmissible infections continues to decline as a result of continually strengthening interventions and because of more general improvements in public health. More attention is being paid to emerging infections, and recently donor testing has been impl

Let us use physiologic transfusion triggers: Favorable outcome in an 86-year-old Jehovah's witness with a haemoglobin nadir of 44g L-1.

The number of Jehovah's Witnesses worldwide is estimated at approximately 8.6 million. As Jehovah's Witnesses refuse administration of blood and blood-derived products on religious grounds, transfusion of red blood cells is not an option. It is especially problematic when anaemia develops acutely, as in non-elective surgery, as there is no time to optimise volume of red blood cells preoperatively. We present a case of an 86-year-old woman who underwent non-elective laparoscopic cholecystectomy due to gangrenous cholecystitis. Post-operatively the patient was hospitalised in an intensive care u

The breed prevalence of dog erythrocyte antigen 1.1 in the Onderstepoort area of South Africa and its significance in selection of canine blood donors.

The blood group antigen Dog Erythrocyte Antigen (DEA) 1.1 is clinically the most important canine blood group as DEA 1.1 antibodies are capable of causing acute haemolytic, potentially life-threatening transfusion reactions. Dogs do not have naturally occurring antibodies to DEA 1.1 but are rapidly sensitised by the first incompatible transfusion. The prevalence of DEA 1.1 in the general dog population is estimated at 42-46%. Canine blood donors registered with the Onderstepoort Animal Blood Bank (n = 93) as well as potential donors (n = 140) were typed for DEA 1.1 using a monoclonal antibody

Hospital policy of tranexamic acid to reduce transfusion in major non-cardiac surgery (TRACTION): protocol for a phase IV randomised controlled trial.

INTRODUCTION: Tranexamic acid (TXA) is an inexpensive and widely available medication that reduces blood loss and red blood cell (RBC) transfusion in cardiac and orthopaedic surgeries. While the use of TXA in these surgeries is routine, its efficacy and safety in other surgeries, including oncologic surgeries, with comparable rates of transfusion are uncertain. Our primary objective is to evaluate whether a hospital-level policy implementation of routine TXA use in patients undergoing major non-cardiac surgery reduces RBC transfusion without increasing thrombotic risk. METHODS AND ANALYSIS: A

Pulmonary embolism associated with transfusion after severe post-partum haemorrhage: is less more?

BACKGROUND: Transfusion of red blood cells is associated with superficial vein thrombosis (SVT) and venous thromboembolism (deep vein thrombosis and/or pulmonary embolism, VTE). The present study investigated the prevalence of SVT and VTE in women transfused in the peri-partum period. MATERIALS AND METHODS: We carried out an observational study in a tertiary level obstetrics department in the Apulia Region of Southern Italy to investigate VTE in women transfused during or after labour. The study included all women who delivered between January 1st and November 30th, 2018. A thrombotic event w
