Regulation of T Cell Senescence in Health and Diseases.

Aging is accompanied by a progressive decline in immune function, a process termed immunosenescence, which affects both innate and adaptive immune compartments. Among these, the adaptive immune system-and particularly T cells-undergoes the most profound functional and phenotypic alterations, critically impairing host defense against infections, cancer, and vaccination responses. The age-associated decline of adaptive immunity is shaped by the divergent senescence pathways of CD4+ helper and CD8+ cytotoxic T cells. While both lineages enter a state of cell-cycle arrest, their distinct immunolog

Squamous cell anal carcinoma: Current therapeutic approaches.

Anal squamous cell carcinoma (SCAC) is a relatively rare malignancy, but its incidence has been rising, largely due to infection with human papillomavirus (HPV), which is the main etiological driver. HPV vaccination and treatment of high-grade squamous intraepithelial lesions (HSIL) significantly lower the risk of progression to invasive cancer, while immunosuppressed individuals face a higher susceptibility to disease development. Most patients are diagnosed with localized SCAC, for which definitive concurrent chemoradiotherapy remains the standard of care, offering high rates of disease cont

Nanodiscs for drug delivery.

Lipid nanodiscs have emerged as a versatile and promising tool for drug delivery due to their biocompatibility, structural flexibility, and ability to mimic native cell membrane environments. These nanoscale assemblies, composed of lipid bilayers stabilized by scaffold amphipathic proteins, peptides, synthetic polymers, or saponins, offer a stable membrane mimetic system for encapsulating hydrophobic drugs and membrane proteins. Ongoing research continues to expand the diversity of nanodisc formulations, each with distinct advantages and limitations. Their tunable size, surface functionality,

Age-related thymic involution: Mechanistic insights and rejuvenating approaches to restore immune function.

The gradual decline in thymic function with age, known as age-related thymic involution, leads to reduced T cell production, thereby increasing the risk of infections and cancer susceptibility and leading to poor vaccine responses. Moreover, T cell defects were recently involved in the age-related loss of tissue integrity and function. Mechanistically, thymic involution is driven by several factors, including hormonal modifications and chronic inflammation, leading to functional changes in the hematopoietic and stromal compartments. These progressive changes alter the cross-talk between develo

Isolation and Proteomic Characterization of Outer Membrane Vesicles in Fusobacterium nucleatum.

Fusobacterium nucleatum is an anaerobic Gram-negative bacterium associated with periodontal disease and colorectal cancer. Outer membrane vesicles (OMVs) produced by F. nucleatum are nanoscale lipid vesicles enriched with immunogenic and virulence-associated proteins, making them promising candidates for vaccine development. This protocol describes a comprehensive workflow for culturing F. nucleatum, isolating and purifying OMVs via ultracentrifugation and density gradient ultracentrifugation, and characterizing their biophysical properties using transmission electron microscopy (TEM) and dyna

PD-1 is requisite for skin TRM cell formation and specification by TGFβ.

Tissue-resident memory T (TRM) cells provide infectious, cancer and vaccine-trained immunity across barrier sites. TRM cells are implicated in autoimmunity, successful response to immune checkpoint blockade in the tumor microenvironment and toxicities that occur after immune checkpoint blockade in peripheral tissues. Here, we identified that signaling through the immune checkpoint programmed death receptor 1 (PD-1) strongly impacts the early specification of CD8+ TRM cells in the skin. PD-1 is expressed broadly across mouse and human skin TRM cells, in the absence of persistent infection, and

The Production and Characterization of Synthetic Glycoconjugate Vaccines.

Chemical conjugation of polysaccharides to protein carriers has become essential in carbohydrate vaccine development to enhance the poor immunogenicity of glycans. Neutral and uncharged carbohydrates induce T-cell independent responses, failing to produce robust IgG antibodies or sustained T-cell memory and conjugating native or synthetic glycans with carrier proteins such as the Cross-Reactive-Material-197 (CRM197) diphtheria toxin mutant, tetanus toxoid, or protein D of H. influenzae eliciting T-cell-dependent immune responses and class switching that produces high-affinity antibodies. Unusual Presentation of Advanced Urothelial Cancer in a Young Patient.

BACKGROUND/AIM: Urothelial carcinoma, common in older adults, is rare in younger populations and even less common in the prostatic urethra. Advanced disease is typically managed with platinum-based chemotherapy, immune checkpoint inhibitors, and targeted therapies. However, rare presentations in young patients with aggressive disease highlight the need for innovative and personalized treatment strategies. CASE REPORT: This case report presents a rare instance of metastatic urothelial carcinoma originating in the prostatic urethra of a 37-year-old male. Initial symptoms led to diagnosis throug

Management of ocular toxicity in patients with gynecologic cancer receiving novel antibody-drug conjugates: a narrative review.

Antibody-drug conjugates (ADCs) represent a promising therapeutic approach in gynecologic cancers, particularly ovarian and cervical malignancies. Agents such as mirvetuximab soravtansine, and tisotumab vedotin, targeting folate receptor alpha and tissue factor, respectively, reported clinical efficacy in patients with limited options. However, their use is associated with ocular toxicities, including keratopathy, blurred vision, and dry eye, which may impact adherence and quality of life. This review summarizes current evidence on the incidence, pathophysiology, and clinical presentation of A

A proteogenomic analysis of cervical cancer reveals therapeutic and biological insights.

Although the incidence of cervical cancer (CC) has been reduced in high-income countries due to human papillomavirus (HPV) vaccination and screening strategies, it remains a significant public health issue that poses a threat to women's health in low-income countries. Here, we perform a comprehensive proteogenomic profiling of CC tumors obtained from 139 Chinese women. Integrated proteogenomic analysis links genetic aberrations to downstream pathogenesis-related pathways and reveals the landscape of HPV-associated multi-omic changes. Multinational epidemiological analysis of oral human papillomavirus incidence in 3,137 men.

Oral human papillomavirus (HPV) is associated with oropharyngeal cancer (OPC). Although OPC incidence is increasing globally, knowledge of oral HPV infection rates is limited. Here we carried out an observational epidemiological analysis of oral HPV incidence in 3,137 men enrolled from the United States, Mexico and Brazil between 2005 and 2009. Individuals were followed for new HPV infection for a median of 57 months. Cumulative incidence and factors associated with acquisition were also assessed. The incidence rate of oral oncogenic HPV was 2.4 per 1,000 person-months, did not vary with age a

British Society of Gastroenterology Best Practice Guidance: outpatient management of cirrhosis - part 3: special circumstances.

The prevalence of cirrhosis has risen significantly over recent decades and is predicted to rise further. Widespread use of non-invasive testing means cirrhosis is increasingly diagnosed at an earlier stage. Despite this, there are significant variations in outcomes in patients with cirrhosis across the UK, and patients in areas with higher levels of deprivation are more likely to die from their liver disease. This three-part best practice guidance aims to address outpatient management of cirrhosis, in order to standardise care and to reduce the risk of progression, decompensation and mortalit

The many facets of immune-mediated thrombocytopenia: Principles of immunobiology and immunotherapy.

Immune thrombocytopenia (ITP) is a rare autoimmune condition, due to peripheral platelet destruction through antibody-dependent cellular phagocytosis, complement-dependent cytotoxicity, cytotoxic T lymphocyte-mediated cytotoxicity, and megakaryopoiesis alteration. This condition may be idiopathic or triggered by drugs, vaccines, infections, cancers, autoimmune disorders and systemic diseases. Recent advances in our understanding of ITP immunobiology support the idea that other forms of thrombocytopenia, for instance, occurring after immunotherapy or cellular therapies, may share a common patho

Is there a real risk of bacterial infection in patients receiving targeted and biological therapies?

Over the past decades, the advent of targeted and biological therapies has revolutionized the management of cancer and autoimmune, hematological and inflammatory conditions. Although a large amount of information is now available on the risk of opportunistic infections associated with some of these agents, the evidence regarding the susceptibility to bacterial infections is more limited. Biological agents have been shown to entail a variable risk of bacterial infections in pivotal randomized clinical trials and post-marketing studies. Recommendations on risk minimization strategies and therape

Hallmarks of T cell aging.

The aged adaptive immune system is characterized by progressive dysfunction as well as increased autoimmunity. This decline is responsible for elevated susceptibility to infection and cancer, as well as decreased vaccination efficacy. Recent evidence indicates that CD4+ T cell-intrinsic alteratins contribute to chronic inflammation and are sufficient to accelerate an organism-wide aging phenotype, supporting the idea that T cell aging plays a major role in body-wide deterioration. In this Review, we propose ten molecular hallmarks to represent common denominators of T cell aging. These hallmar

Development of an innovative delivery system for bacillus Calmette-Guérin bladder administration.

PURPOSE: To describe the development of an innovative process to deliver bacillus Calmette-Guérin (BCG) to an offsite urology clinic for bladder instillation. SUMMARY: The use of BCG, a live virus vaccine for treatment of patients with localized cancer of the urinary bladder, has created many logistical problems for hospitals and infusion center pharmacies. Due to its short stability, the drug cannot be made ahead of time and coordination with a patient's arrival at an infusion site is challenging. This becomes exceptionally challenging when a urology clinic has limited compounding capacity a

Development of CMV-CD19 bi-specific CAR T cells with post-infusion in vivo boost using an anti-CMV vaccine.

Adoptive transfer of in vitro expanded, chimeric antigen receptor (CAR)-redirected CD19-specific T cells can induce dramatic disease regression in patients with leukemia and lymphomas. However, the full potential of this emerging modality is hampered in some cancer settings by a significant rate of therapeutic failure arising from the attenuated engraftment and persistence of CAR-redirected T cells, and tumor relapse following adoptive transfer. Here, we discuss an advanced strategy that facilitates post-infusion in vivo boosting of CAR T cells via CMV vaccination, to mediate durable remission

Moving forward-the 2019 ASCCP Risk-Based Management Consensus Guidelines for Abnormal Cervical Cancer Screening Tests and Cancer Precursors and beyond: implications and suggestions for laboratories.

The 2019 ASCCP Risk Based Management Consensus Guidelines for prevention of cervical cancer promote clinical management recommendations aligned with our increased understanding of HPV biology and cervical carcinogenesis. They employ HPV-based testing as the basis for risk estimation, allow for personalized risk-based management by incorporating knowledge of current results with prior results, and streamline incorporation of new test methods as they are validated. They continue to support the principles of "equal management for equal risk" and "balancing harms and benefits" adopted in the 2012

Reversal of Immunity After Clearance of Chronic HCV Infection-All Reset?

Chronic viral infections cause deterioration of our immune system. However, since persistent infections rarely can be eliminated, the reinvigoration capacity of an exhausted immune system has remained largely elusive. Chronic hepatitis C virus (HCV) infection can since some years be effectively cured with novel direct acting antiviral agents. Thus, it is now possible to study reversal of immunity in patients that are cured from a long-lasting chronic infection. We here highlight recent developments in the analysis of various immune cell populations during and after clearance of HCV infection.

Bacillus Calmette-Guérin Cases Reported to the National Tuberculosis Surveillance System, United States, 2004-2015.

Mycobacterium bovis bacillus Calmette-Guérin (BCG) is used as a vaccine to protect against disseminated tuberculosis (TB) and as a treatment for bladder cancer. We describe characteristics of US TB patients reported to the National Tuberculosis Surveillance System (NTSS) whose disease was attributed to BCG. We identified 118 BCG cases and 91,065 TB cases reported to NTSS during 2004-2015. Most patients with BCG were US-born (86%), older (median age 75 years), and non-Hispanic white (81%). Only 17% of BCG cases had pulmonary involvement, in contrast with 84% of TB cases. Epidemiologic features
