Cardiac magnetic resonance T1 mapping for amyloid cardiomyopathy in patients with heart failure.

AIMS: This study investigates the diagnostic accuracy of cardiac magnetic resonance (CMR) T1 mapping for the evaluation of cardiac amyloidosis (CA) in elderly patients with heart failure (HF). METHODS AND RESULTS: We prospectively enrolled consecutive patients aged ≥60 years with symptomatic HF, left ventricular ejection fraction (LVEF) ≥ 40%, increased left ventricular (LV) wall thickness (≥12 mm), and elevated cardiac biomarkers. All patients underwent standardized CMR, bone scintigraphy, and screening for monoclonal proteins. Primary endpoint was the diagnosis of CA with T1 mapping. The re

AAVrh.10hFXN Gene Therapy for the Cardiomyopathy of Friedreich Ataxia: A Nonrandomized Clinical Trial.

IMPORTANCE: Friedreich ataxia (FA), a fatal, autosomal recessive disorder caused by pathogenic variants in the frataxin (FXN) gene, is characterized by progressive neurologic and cardiac disease, with cardiac disease being the major cause of death. Preclinical studies have demonstrated that systemic administration of AAVrh.10hFXN, an rh.10 serotype cardiotropic adenoassociated virus (AAV) vector expressing the normal human FXN coding sequence, reverses the cardiomyopathy in FXN-deficient mice. OBJECTIVE: To assess the safety and preliminary efficacy of administration of AAVrh.10hFXN to adults

Pathways for Patients with Transthyretin Amyloid Cardiomyopathy from a District General Hospital Perspective.

BACKGROUND: The care of patients with transthyretin amyloid cardiomyopathy (ATTR-CM) is often fragmented and routine datasets rarely capture real-world clinical trajectories and reasons for diagnosis. We introduce a novel approach, called forensic data acquisition and pathway analysis, to examine the real-world experiences of patients with ATTR-CM in our district general hospital. METHODS: We retrospectively evaluated inpatient and outpatient healthcare records for our hospital between 2019 to 2025 as a part of a quality improvement project. RESULTS: We identified 26 cases of confirmed or li

Peripartum Cardiomyopathy Genetic Screening Protocol and Registry: The PERCVD Trial in the Gulf South.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a pregnancy-associated form of systolic heart failure with high prevalence and mortality worldwide. It occurs in 1:1800 to 1:3500 births in the United States and disproportionately affects women in the Gulf South. Louisiana has the highest maternal mortality rate in the United States, yet no prospective registry exists to investigate the genetic basis of perinatal cardiovascular disorders, creating an urgent need for genetic assessment. OBJECTIVES: The objective of the study was to establish the first PPCM prospective whole-genome sequencing and

Ferroptosis Signatures in Diabetic Cardiomyopathy: Multi-Omics Discovery and Validation of ACOT1 and TXNIP.

Background: Diabetic cardiomyopathy (DCM) is a serious cardiovascular complication specific to diabetes mellitus, with rising global prevalence. Ferroptosis, an iron-dependent form of regulated cell death driven by lethal lipid peroxidation, has been implicated in the pathogenesis of DCM. However, the key regulatory genes remain poorly characterized. This study aimed to identify and validate ferroptosis-related signature genes in DCM. Methods: Three murine transcriptomic datasets (GSE123975, GSE155377, and GSE210611) were retrieved from GEO and merged after batch correction. Differentially exp

High-density lipoprotein cholesterol levels and prognosis in non-ischemic dilated cardiomyopathy.

BACKGROUND: Although low high-density lipoprotein cholesterol (HDL-C) is associated with poor outcomes in heart failure, its specific prognostic role in non-ischemic dilated cardiomyopathy (DCM) remains undefined. This study aimed to investigate the association between HDL-C levels and long-term clinical outcomes in patients with DCM. METHODS: We retrospectively enrolled 297 patients with DCM who underwent cardiac magnetic resonance between January 2017 and October 2024. The primary endpoint was a composite of all-cause mortality and heart transplantation; the secondary endpoint additionally

A zebrafish stable model of Galectin-3 to elucidate its role in arrhythmogenic cardiomyopathy.

Galectin-3 (Gal-3) dysregulation has emerged as a critical mediator of inflammatory processes in arrhythmogenic cardiomyopathy (AC), playing pivotal roles in modulating Wnt/β-catenin signaling and regulating macrophage polarization. AC is a rare genetic disorder, primarily driven by desmosomal gene variants, characterized by fibro-fatty replacement of the ventricular myocardium, progressive ventricular dysfunction, and heightened arrhythmic risk in the young and athletes. To investigate the role of this multifaceted lectin in AC pathogenesis, we developed and characterized a stable lgals3a (-a

Left atrial reservoir strain for predicting progression to end-stage in hypertrophic cardiomyopathy.

AIMS: End-stage hypertrophic cardiomyopathy (HCM), defined as a left ventricular (LV) ejection fraction (LVEF) < 50%, is associated with poor prognosis; however, predictors of progression remain unclear. We aimed to identify prognostic factors for progression to end-stage HCM. METHODS AND RESULTS: We analysed 925 patients with HCM between 2007 and 2023 who underwent ≥1 year of follow-up echocardiography. The primary outcome was progression to end-stage HCM, defined as an LVEF <50% without reversible causes. A cardiovascular magnetic resonance (CMR) subcohort included 491 patients with baselin

Septal reduction therapies for obstructive hypertrophic cardiomyopathy: Current strategies and evolving innovations.

In symptomatic patients with obstructive hypertrophic cardiomyopathy, the management of left ventricular outflow tract obstruction remains the central therapeutic challenge. Despite recent advances in pharmacologic therapy, septal reduction therapy continues to represent the gold standard for relieving obstruction. Among available techniques, transaortic septal myectomy and percutaneous alcohol septal ablation are the most widely practiced and well-established procedures. In recent years, novel techniques employing alternative approaches or energy sources have emerged, including transapical be

Overcoming barriers to early diagnosis and treatment of p.Val142Ile amyloid transthyretin (ATTR) cardiomyopathy.

The most common hereditary transthyretin (ATTRv) amyloidosis variant in the United States, Val122Ile (p.Val142Ile), is predominantly detected in populations of African ancestry. In this narrative review, we highlight the current challenges and discuss priority focus areas to improve the timely diagnosis and treatment of p.Val142Ile ATTRv amyloidosis across populations. Studies suggest geographic, economic, and socio-economic discordance in timely diagnosis, treatment access, and outcomes in regions with a high proportion of at‑risk individuals. Many ATTR amyloidosis symptoms overlap with other

Apathetic Thyroid Storm Presenting as New-Onset Dilated Cardiomyopathy and Cardiogenic Shock.

Thyroid storm is an uncommon but life-threatening endocrine emergency associated with high mortality, and its diagnosis remains largely clinical due to the lack of universally reliable criteria to distinguish it from severe thyrotoxicosis; atypical or "apathetic" forms are particularly rare and can delay recognition. A 49-year-old man with no prior medical history presented with a two-week history of progressive anasarca, palpitations, and nonspecific gastrointestinal symptoms in the absence of fever or agitation and was found to have atrial fibrillation with rapid ventricular response and dec

Dysregulation of glycogen metabolism in cardiomyopathy caused by glycogenin-1 (GYG1) missense variants.

CONTEXT: Glycogen storage diseases are inherited disorders of glycogen metabolism and are commonly associated with hypoglycaemia, hepatic dysfunction, or skeletal and cardiac myopathy, depending on the affected enzyme. Glycogen storage disease type 15 (GSD15) is caused by pathogenic variants in GYG1, which encodes glycogenin-1, the auto-glucosylating primer required for glycogen synthesis. GSD15 is characterized by cardiomyopathy and storage of abnormal glycogen and polyglucosan in cardiomyocytes. OBJECTIVE: To understand the pathobiology of GSD15, we investigated the storage material in hear

Redefining Sudden Cardiac Death Risk Stratification in Dilated Cardiomyopathy: An Integrative Approach.

Dilated cardiomyopathy (DCM) is one of the most common cardiomyopathy phenotypes. Moreover, sudden cardiac death (SCD) remains a leading cause of mortality in patients with DCM, predominantly driven by malignant ventricular arrhythmias (VAs). Current guideline-recommended risk stratification relies heavily on left ventricular ejection fraction (LVEF) to guide implantable cardioverter-defibrillator (ICD) therapy; however, attention has recently shifted toward a multiparametric strategy that integrates clinical characteristics with electrocardiographic markers, biomarkers, cardiac magnetic reson
