The effect of body mass index on complications from cardiac surgery in the oldest old.

OBJECTIVES: Overweight (body mass index (BMI)>25) and obesity (BMI>30) are associated with adverse outcomes in the general population. In older people, an increased risk of adverse events was observed in leaner and overweight older subjects. We evaluated the association between BMI and cardiac surgery complications in subjects aged 75 and older. DESIGN: Retrospective review of complication rates compiled as part of the New York State Department of Health Adult Cardiac Surgery Report. SETTING: Academic tertiary care medical center. PARTICIPANTS: One thousand four hundred forty-eight subjects

Factors associated with family caregivers' attitudes toward deprescribing in long-term care facilities: a cross-sectional study in South Korea.

BACKGROUND: Deprescribing has emerged as an important strategy for addressing polypharmacy among older adults in long-term care facilities (LTCFs). Family caregivers often play a key role in medication-related decision-making, yet factors associated with caregivers' attitudes toward deprescribing remain insufficiently understood. This study aimed to identify the factors associated with caregivers' attitudes toward deprescribing among residents of LTCFs in South Korea. METHODS: A cross-sectional online survey was conducted with 973 family caregivers of residents in LTCFs. Attitudes toward depr

Prognostic Value of the Acute Physiology and Chronic Health Evaluation II (APACHE II) Score in Secondary Peritonitis: A Narrative Review.

Secondary peritonitis is a major surgical emergency worldwide and is associated with high morbidity and mortality, particularly in resource-limited settings. It most commonly results from perforation of a hollow viscus, causing contamination of the peritoneal cavity and triggering a severe systemic inflammatory response that may progress to sepsis and multi-organ dysfunction. Early identification of high-risk patients and objective assessment of disease severity are essential for guiding clinical decision-making, optimizing resource utilization, and improving outcomes. However, the lack of a p

Predictors of Depression, Anxiety, and Stress Symptoms Among Older Adults in Rural North India.

BACKGROUND: Mental health problems among older adults are an important and underrecognized public health concern in India. Depression, anxiety, and stress in later life are influenced by demographic, social, behavioural, and clinical factors, and identifying their independent predictors is essential for designing targeted preventive strategies. OBJECTIVES: This study aims to estimate the burden of depression, anxiety, and stress among older adults in rural North India and to identify independent predictors of depression, anxiety, and stress symptoms. METHODS: A community-based cross-sectiona

Blood pressure variability and frailty: mechanisms, evidence, and clinical implications.

Blood pressure variability (BPV), fluctuations in blood pressure across beat-to-beat, 24-h, day-to-day, and visit-to-visit timescales, has emerged as a risk marker that is independent of mean blood pressure. Frailty, a multidimensional syndrome of diminished physiological reserve, shares core biology with BPV, including vascular aging, impaired baroreflex function, autonomic dysregulation, and chronic low-grade inflammation. This narrative review synthesizes mechanistic, epidemiologic, and clinical evidence linking BPV and frailty. Mechanistically, arterial stiffness and endothelial dysfunctio

Beyond Medication: A Cross-Sectional Study of How Sex Influences Overall Chronic Disease Adherence Behaviour Using the French Generic Adherence for Chronic Diseases Profile (GACID-P).

BACKGROUND: Adherence research has historically focused on medication-taking behaviors. While evidence suggests that males are generally more adherent to medication regimens than females, the influence of sex on overall adherence behaviors remains unclear. We explored how sex influences a multidimensional profile of adherence beyond medication using the Generic French Adherence in Chronic Diseases Profile (GACID-P). METHODS: We conducted a cross-sectional study using data from the validation of the GACID-P. Participants were adults (>18 years) with chronic conditions attending specialist care

Studying an educational intervention and its impact on health-related quality of life and fasting blood glucose levels among patients with type 2 diabetes mellitus in rural China.

BACKGROUND: Type 2 diabetes mellitus (T2DM) remains a major public health challenge in rural China, where healthcare access and health literacy are limited. Although primary healthcare (PHC) plays a key role in chronic disease management, evidence on the long-term effects of educational interventions in rural settings is scarce. This study, part of the initiative "Studying the Vertical Integration Strategy of Chronic Disease Service Based on Multiple Incentive Mechanisms in Rural China" (2015-2017), evaluated the impact of a structured educational intervention on fasting blood glucose (FBG) tr

High sensitivity C-reactive protein and the risk of cardio-kidney outcomes in adults with moderate to severe chronic kidney disease: a nationwide study.

BACKGROUND AND HYPOTHESIS: The burden and prognostic significance of inflammation in chronic kidney disease (CKD) remains poorly characterized, with prior studies yielding conflicting estimates of the association between elevated high-sensitivity C-reactive protein (hsCRP) and cardio-kidney outcomes. METHODS: Observational nationwide study of 23,008 adults with CKD stages 3-5 attending nephrology care and enrolled in the Swedish Renal Registry (2008-2023). Inflammation was assessed longitudinally via 94,500 repeated hsCRP registrations. Time-varying, cause-specific Cox models accounting for c

A Johari-Inspired Framework for Population Gap Analysis: Linking Underdiagnosis, Overdiagnosis, Diagnostic Competition, and Prevention.

Linear care cascades (the Rule of Halves and awareness-treatment-control frameworks) quantify attrition among system-recognized cases but cannot represent true disease that remains undiagnosed, system-labeled disease that is absent or non-actionable, or symptomatic individuals whose diagnostic encounters are consumed by competing labels. A structural solution to these simultaneous limitations does not exist within current population health methodology. We conducted a narrative review of prior healthcare uses of the Johari Window, empirical applications and limitations of linear gap frameworks,

Physiotherapy with integrated virtual reality for patients with severe chronic low back pain: cluster-randomized controlled trial (VARIETY).

INTRODUCTION: Chronic low back pain (CLBP) is a leading cause of disability worldwide. Physiotherapy is a common treatment, but its effect on physical functioning is generally modest, particularly for patients with severe complaints (i.e., high levels of disability and pain). Virtual Reality (VR) may complement physiotherapy, yet evidence for its effectiveness remains limited. The aim of this study was to assess the effectiveness and feasibility of a VR intervention integrated within physiotherapy for people with severe CLBP. METHODS: A cluster-RCT across Dutch physiotherapy practices was con
