Prevalence, risk factors and adverse outcomes of intrapartum related asphyxia amongst newborns from mothers with low-risk deliveries in peri-urban Gambian health facilities: a retrospective cohort study.

INTRODUCTION: Despite significant progress in perinatal care, intrapartum-related asphyxia remains a leading cause of neonatal morbidity and mortality, especially in the poorest regions of the world such as sub-Saharan Africa. The objective of this study is to determine the prevalence, risk factors, and adverse outcomes of intrapartum-related asphyxia among low-risk health facility-based deliveries in The Gambia. METHODS: This is a secondary analysis of a randomised clinical trial conducted in western Gambia between October 2017 and May 2021. Women with low-risk pregnancies in labour and thei

Uncovered, underfunded, and undervalued: How policy contexts shape knowledge production in multi-site substance use trials.

BACKGROUND: Pragmatic trials evaluate the effectiveness or feasibility of a treatment in clinical practice. However, in substance use research, the "real world" is characterized by patterns of structural stigma embedded in everchanging drug policies and public perceptions that vary by jurisdiction. While such contextual influences typically go unmeasured in clinical research, these localized features may shape participants' trial experiences and outcomes, with relevance for the conduct of multisite trials and their results. METHODS: We conducted a nested qualitative study with people who use

Severe Adverse Events in Neoadjuvant Therapy of Triple-Negative Breast Cancer: A Systematic Review and Meta-Analysis of Randomized Phase 3 Trials.

Neoadjuvant therapy (NAT) is standard treatment for triple-negative breast cancer (TNBC) but toxicity is a major concern. This study aimed to assess the incidence and risk of grade ≥ 3 adverse events (AEs) associated with NAT in nonmetastatic TNBC. We performed a systematic review and meta-analysis registered in PROSPERO (CRD42024562785). Four databases were searched for randomized controlled trials (RCTs) from 2010 to 2025 reporting grade ≥ 3 AEs in TNBC. Pooled prevalence and risk ratios (RRs) were estimated using random-effects models. Six RCTs including 3316 patients were analyzed. Neutrop

Tocilizumab for Painful Chronic Pancreatitis (TOPAC trial): Protocol for a phase 2 randomized, placebo-controlled, double-blind, investigator-initiated trial.

BACKGROUND: Chronic pancreatitis (CP) is a fibro-inflammatory disease that damages the pancreas, leading to severe abdominal pain and metabolic complications. Activated macrophages and pancreatic stellate cells drive CP progression, and their activity is regulated by complex immune signals, including interleukin-6 (IL-6). Preclinical studies suggest that blocking IL-6 signalling may have pain-relieving effects in CP. Based on these findings, we hypothesise that tocilizumab, an anti-IL-6 receptor antibody, will reduce abdominal pain and improve physical functioning and quality of life in patien

Study protocol for the investigator-initiated Danish pragmatic randomised STENO INTEN-CT trial: does screening and intervention for subclinical coronary artery disease in type 2 diabetes reduce cardiovascular events?

INTRODUCTION: Cardiovascular disease (CVD) risk remains high but unevenly distributed in patients with type 2 diabetes mellitus (T2DM). Current risk stratification strategies are far from optimal, leading to both undertreatment and overtreatment of patients. The STENO INTEN-CT trial aims to evaluate a strategy of improved CVD risk management by using cardiac CT (coronary artery calcification (CAC)) for stratification and tailoring of multifactorial cardiovascular treatment based on CAC score. We hypothesise that (1) intensified medical treatment will lower CVD event rates in high-risk patients

The effectiveness of high-intensity interval training versus moderate intensity continuous training in prehabilitation among patients undergoing major abdominal surgery: A study protocol.

BACKGROUND: Prehabilitation programmes for major abdominal surgery enhance patients' condition preoperative and promote recovery by building surgical resilience. However, the precise protocol of prehabilitation pertaining to the prescription of exercise training remains undefined. This paper describes the protocol of the study that aims to evaluate the effectiveness of high-intensity interval training (HIIT) compared to moderate intensity continuous training (MICT) among patients undergoing major abdominal surgery. METHODS: This is pragmatic double-blind randomized controlled trial, with para

Protocol of a randomized trial of acceptance and commitment therapy for patient fatigue interference and caregiver burden in advanced gastrointestinal cancer.

Fatigue's interference with activities, mood, and cognition is one of the most prevalent and distressing problems of patients with advanced gastrointestinal cancer. As fatigue interferes with patient functioning, family caregivers often report feeling burdened by increasing demands. Evidence-based interventions for patient fatigue interference and caregiver burden are lacking in advanced gastrointestinal cancer. In a pilot trial, telephone-based Acceptance and Commitment Therapy (ACT) showed potential for reducing patient fatigue interference and caregiver burden in this population. The curren

A clinical trial evaluating pharmacist-guided self-management of hypertension among veterans with CKD, rationale and study design.

RATIONALE & OBJECTIVE: Chronic kidney disease (CKD) associates with high morbidity and mortality due to CKD progression and cardiovascular disease (CVD). Blood pressure (BP) lowering reduces the risk of CVD and CKD progression. Despite the large number of BP medications available, a significant proportion of patients with CKD have BP above the goal. The current practice involves licensed providers performing medication titrations to achieve BP goals and is associated with limited patient engagement. Here, we evaluate the effectiveness of pharmacist-guided patient-driven titration of BP medicat

Two-fraction Ultrahypofractionated Radiotherapy with focal BOost for intermediate-risk, localized prostate cancer (TURBO): study protocol of a phase II, randomized controlled clinical trial.

BACKGROUND: Hypofractionation in prostate cancer radiotherapy (RT) has drastically reduced fractionation schemes from 35 to 5 fractions for patients with intermediate-risk prostate cancer, providing good oncological outcomes and well-tolerated treatment toxicity. With an ageing population and a subsequently higher volume of patients with localized prostate cancer, optimizing treatment options is essential to maintain a sustainable healthcare system. Magnetic resonance (MR)-guided RT, allows for the delivery of higher doses of radiation with more accuracy, while limiting genitourinary (GU) and

Risk of Dyslipidemia Associated With Oral Janus Kinase Inhibitors: A Systematic Review and Meta-Analysis of Randomized Placebo-Controlled Trials.

Janus kinase inhibitors (JAKi) have sparked a new era in the treatment of immune-mediated diseases. While some studies have reported an increased incidence of dyslipidemia in JAKi-treated patients, the full extent of this adverse event is not established. The study aimed to assess the association between treatment with oral JAKi and dyslipidemia in phases 2 and 3 placebo-controlled randomized clinical trials (RCTs). A systematic review and meta-analysis were conducted, encompassing phase 2 and 3 RCTs. The Embase, PubMed, and Web of Science databases were searched up to March 9, 2025. Only RCTs

Treatment-emergent major adverse cardiovascular and thromboembolic events were infrequent during clinical trials of pegloticase.

OBJECTIVES: Long-term maintenance of serum urate levels <6 mg/dl reduces gout flare frequency. However, urate-lowering therapy (ULT) initiation can induce gout flare. The incidence of thromboembolic (TE) and cardiovascular (CV) events has been shown to increase in the 30 and 120 days following gout flare, respectively; therefore, the question of ULT initiation increasing patient risk for CV/TE events has been raised. Here, we investigate CV/TE event incidence following pegloticase initiation in clinical trials. METHODS: This post hoc analysis of pooled data from four trials examined treatment

FLIPI24: A Modern Prognostic Model and Clinical Trial Enrichment Tool for Newly Diagnosed Follicular Lymphoma.

PURPOSE: Although most patients with follicular lymphoma (FL) can expect an indolent course, progressive lymphoma remains the primary cause of death during the first decade after diagnosis. Progression of disease within 24 months (POD24) of starting first-line (1L) immunochemotherapy defines a high-risk population with poor survival, but better risk stratification at diagnosis is needed. METHODS: The FLIPI24 model was developed and internally validated to predict 24-month event rates using individual data from 4,485 patients treated with 1L immunochemotherapy from 10 observational cohorts of

The TRaditional versus Early Aggressive Therapy for Multiple Sclerosis (TREAT-MS) trial: design and baseline characteristics of participants.

BACKGROUND: Whether early use of higher-efficacy disease-modifying therapy (DMT) reduces later risk of disability in multiple sclerosis (MS) has not been studied as a prospective treatment strategy. Herein, we describe the TRaditional versus Early Aggressive Therapy for MS (TREAT-MS) trial design and provide the baseline characteristics of the enrolled population. METHODS: TREAT-MS is a pragmatic, randomized, rater-blinded trial of DMT strategies in treatment-naive people aged 18-60 years with relapsing-remitting MS (RRMS). Participants were randomized (1:1, stratified by site and long-term d

Health-related quality of life in patients with metastatic hormone-sensitive prostate cancer treated with androgen receptor signaling inhibitors: the role of combination treatment therapy.

BACKGROUND: While the addition of androgen receptor signaling inhibitors (ARSIs) to androgen deprivation therapy (ADT) results in better of overall survival in patients with metastatic hormone-sensitive prostate cancer (mHSPC), information regarding health related quality of life (HR-QoL) is sparse. We aimed at summarizing current evidence on the impact of ARSIs on HR-QoL. METHODS: We performed a systematic review of the published literature on PubMed/EMBASE, Web of Science, SCOPUS, and the Cochrane libraries between January 2011 and April 2022. We included only phase III randomized controlle

Design and outcome measures of LAVENDER, a phase 3 study of trofinetide for Rett syndrome.

INTRODUCTION: Rett syndrome (RTT) is a debilitating neurodevelopmental disorder with no approved treatments. Trofinetide is a synthetic analog of glycine-proline-glutamate, the N-terminal tripeptide of insulin-like growth factor 1. In a phase 2, placebo-controlled trial in 82 females with RTT aged 5-15 years, a significant (p ≤ 0.042) improvement over placebo was observed with the highest trofinetide dose (200 mg/kg twice daily [BID]) on three measures: Rett Syndrome Behaviour Questionnaire (RSBQ), Clinical Global Impression-Improvement (CGI-I), and RTT-Clinician Domain Specific Concerns-Visua

Randomised double-blind phase 3 clinical study testing impact of atorvastatin on prostate cancer progression after initiation of androgen deprivation therapy: study protocol.

INTRODUCTION: Blood cholesterol is likely a risk factor for prostate cancer prognosis and use of statins is associated with lowered risk of prostate cancer recurrence and progression. Furthermore, use of statins has been associated with prolonged time before development of castration resistance (CR) during androgen deprivation therapy (ADT) for prostate cancer. However, the efficacy of statins on delaying castration-resistance has not been tested in a randomised placebo-controlled setting. This study aims to test statins' efficacy compared to placebo in delaying development of CR during ADT tre

Erythropoietic effects of vadadustat in patients with anemia associated with chronic kidney disease.

Patients with chronic kidney disease (CKD) develop anemia largely because of inappropriately low erythropoietin (EPO) production and insufficient iron available to erythroid precursors. In four phase 3, randomized, open-label, clinical trials in dialysis-dependent and non-dialysis-dependent patients with CKD and anemia, the hypoxia-inducible factor prolyl hydroxylase inhibitor, vadadustat, was noninferior to the erythropoiesis-stimulating agent, darbepoetin alfa, in increasing and maintaining target hemoglobin concentrations. In these trials, vadadustat increased the concentrations of serum EP

Efficacy and safety of guselkumab in biologic-naïve patients with active axial psoriatic arthritis: study protocol for STAR, a phase 4, randomized, double-blinded, placebo-controlled trial.

BACKGROUND: Axial involvement constitutes a specific domain of psoriatic arthritis (PsA). Interleukin (IL)-23 inhibitors have demonstrated improvement in axial PsA (axPsA) symptoms, but have not shown efficacy in treating ankylosing spondylitis (AS), suggesting differences in axPsA processes and treatments. In a post hoc, pooled analysis of patients with investigator- and imaging-confirmed sacroiliitis in two phase 3, randomized, placebo-controlled studies (DISCOVER-1 and DISCOVER-2), patients treated with guselkumab, an IL-23p19 inhibitor, had greater axial symptom improvements compared with
