Lower Socioeconomic Status Predicts Greater Obstacles to Care: Using Outpatient Cholecystectomy as a Model Cohort.

BACKGROUND: Patients with low socioeconomic status (SES) are disadvantaged in terms of access to health care. A novel metric for SES is the Distressed Communities Index (DCI). This study evaluates the effect of DCI on hospital choice and distance traveled for surgery. METHODS: A Florida database was queried for patients with symptomatic cholelithiasis or chronic cholecystitis who underwent an outpatient cholecystectomy between 2016 and 2019. Patients' DCI was compared with hospital ratings, comorbidities, Charlson Comorbidity Index, and distance traveled for surgery. Stepwise logistic regress

The prevalence and factors associated with mental disorders in a community setting in central Uganda.

BACKGROUND: Mental disorders are known to predict poverty, morbidity and mortality. In resource limited settings, low levels of mental health literacy (MHL) and high mental illness stigma (MIS) have been sighted as possible factors that may impede access to mental health care. However, little has been done to examine the association between mental disorders and these factors (MHL and MIS) in sub-Saharan Africa. METHODS: We assessed for the prevalence of major depressive disorders (MDD), substance use disorders (SUD), post-traumatic stress disorder (PTSD), generalized anxiety disorder (GAD), d

Urbanization and solid waste production: prospects and challenges.

The increasing urban population and the restructuring of urban economies are the main aspects that determine urban development in the twenty-first century. Rapid urbanization is among the most significant anthropogenic factors that impact ecosystems and sustainability. Urbanization is a "double-edged sword." Even though it contributes to economic prosperity and social development, at the same time, it poses severe challenges to the natural environment and social system. The scientific community emphasizes the need to investigate the relationship between cities and the environment to comprehend

Evaluation of human health and ecological risk of heavy metals in water, sediment and shellfishes in typical artisanal oil mining areas of Nigeria.

Heavy metal contamination in water and sediment is a serious concern in nations that depend heavily on natural resources such as Nigeria. In most coastal communities around oil mining areas in Nigeria, drinking water quality, staple food, and livelihoods are primarily dependent on ecological systems and marine resources (e.g., fish). Thus, humans and other receptors are exposed to heavy metal risks through ingestion and dermal contact. This research evaluated the potential ecological risks of heavy metals including Cadmium (Cd), Chromium (Cr), Nickel (Ni), and Lead (Pb) in water, sediments, an

Free-Rider Games for Cooperative Spectrum Sensing and Access in CIoT Networks.

With the rapid development of technologies such as wireless communications and the Internet of Things (IoT), the proliferation of IoT devices will intensify the competition for spectrum resources. The introduction of cognitive radio technology in IoT can minimize the shortage of spectrum resources. However, the open environment of cognitive IoT may involve free-riding problems. Due to the selfishness of the participants, there are usually a large number of free-riders in the system who opportunistically gain more rewards by stealing the spectrum sensing results from other participants and acce

Supporting clinical competency in managing peripherally inserted central catheters during the COVID-19 pandemic: an education evaluation.

INTRODUCTION: Hospitals had to create new practices and training due to the severe acute respiratory syndrome Coronavirus 2 (SARS-CoV-2) pandemic. An increase in patient acuity and the need for peripherally inserted central catheters (PICC) across the hospital required an urban community hospital to educate and support in-patient nurses to manage PICCs in acute and complex care units. Traditionally, these skills were performed by specialized registered nurses (RNs) from the Vascular Access Team (VAT). This paper highlights the education plan, implementation, and evaluation of a hospital-wide t

Exploring dermatologists' perspectives on vaccines in dermatology: a qualitative study.

Vaccination rates among adults in the United States, including dermatology patients, remain suboptimal. Previous research has concluded that outpatient specialty offices often have administrative and patient-related barriers to administering vaccines in their clinics, however, this has never been examined specifically in dermatology. This study aims to examine dermatologists' perspectives on vaccine education in dermatology clinics, identify facilitators and barriers to vaccine administration in dermatology clinics, and explore strategies to improve vaccination rates in dermatology patients. It Takes a Village: The Importance of Social Support after Hematopoietic Stem Cell Transplantation, a Qualitative Study.

Social support is essential to the recovery of patients who have undergone hematopoietic stem cell transplantation (HSCT). We undertook a qualitative study to explore the specific sources and benefits of social support as experienced by HSCT recipients, as well as their unmet social support needs. We conducted semistructured interviews with 25 HSCT recipients recruited from the Dana Farber Cancer Institute's HSCT database. The interviews explored the sources of support that patients receive, the type of assistance social support networks provide to patients, and unmet needs of social support.

Clinical utility of the alternative model of personality disorders: A 10th year anniversary review.

The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, (DSM-5) alternative model of personality disorders (AMPD) has now been available for researchers and clinicians for a decade. The present article aims to provide a narrative review of literature on the AMPD framework with particular emphasis on its clinical utility, inclusive of clinicians' and patients' acceptability, ease of use, diagnostic accuracy, continuity with clinically familiar constructs, and proposed utility for treatment planning, case-formulation, and clinical management. The review specifically covers the

The Opioid Epidemic: a Crisis Disproportionately Impacting Black Americans and Urban Communities.

The heroin epidemic has existed for decades, but a sharp rise in opioid overdose deaths (OODs) jolted the nation in the mid-twenty-teens and continues as a major health crisis to this day. Although the new wave of OODs was initially approached as a rural problem impacting a White/Caucasian demographic, surveillance records suggest severe impacts on African Americans and urban-dwelling individuals, which have been largely underreported. The focus of this report is on specific trends in OOD rates in Black and White residents in states with a significant Black urban population and declared as hot
