Surgery of the endolymphatic sac is controversial. Some consider it a placebo and others consider it the surgical treatment of choice in Ménière's disease. We studied the medical records of 87 patients who underwent surgery between 1978 and 1996. Simple decompression was practiced in 89% and a House shunt in 27.6%. The 1-year results were improvement or recovery from vertigo in 65.4%, no improvement in 25%, and reoperation in 9.5%. Tinnitus improved in only 11.9% and hearing loss improved in 9.5% and worsened in 19%. We reviewed the cases of 50 patients with a 5-year follow-up(60%). Vertigo im

Osteopetrosis is a rare congenital disorder of bone resorption, which causes generalized hyperostosis. One of its many complications involves the skull base and progressive encroachment of the optic nerves within their canals, resulting in optic atrophy and blindness. Management of patients with deteriorating vision may include surgical decompression of the optic canals; their selection requires careful assessment by radiology, computed tomography, visual evoked potentials, electroretinography and possibly haemodynamic studies of the ophthalmic arteries. Postoperative changes can create diffic

UNLABELLED: Premature closure of cranial sutures (primary craniosynostosis) in children leads to characteristic skull deformities and prevents the constricted brain from growing normally. Although the cause remains unknown, several etiological factors have been cited. Recently, hypovascularity has been reported as a possible cause of craniosynostosis.
METHODS: In a prospective study regional cerebral blood flow studies were carried out with 99mTc-HMPAO SPECT in seven children with craniosynostoses. Five preoperative and six postoperative studies were conducted and the results correlated with

Twenty-eight patients (two of them children) with septic arthritis of twenty-nine wrists were treated with early surgical drainage, parenteral antibiotics, and early motion after surgical decompression. The etiology was trauma in seventeen patients, and Staphylococcus aureus was the organism that was most commonly recovered on culture. In twenty-two patients (twenty-three wrists) who were followed for six months to nine years there were no recurrences. The results were evaluated in terms of range of motion, grip strength, and subjective complaints of discomfort and disability. Of the ten wrist

Intervertebral disc (IVD) degeneration is an age-related problem triggering chronic spinal issues, such as low back pain and IVD herniation. Standard surgical treatment for such spinal issues is the removal of the degenerated or herniated IVD and fusion of adjacent vertebrae to stabilise the joint and locally decompress the spinal cord and/or nerve roots to relieve pain. However, a key challenge of current surgical strategies is the increasing risk of adjacent segment degeneration due to the disruption of native biomechanics of the functional spinal unit, dominated by the loss of the IVD. This study by Aboukais et al. (2024) evaluates postoperative outcomes in patients with unruptured giant middle cerebral artery (MCA) aneurysms associated with intracranial hypertension and midline brain shift. Analyzing data from 2012 to 2022, the authors compare surgical approaches, emphasizing the potential benefits of systematic decompressive hemicraniotomy in improving patient outcomes. While the study's findings are valuable, the small sample size and absence of a control group limit its generalizability. The retrospective nature of the study introduces potential biases, and long-term cog

We report a case of superficial temporal arteriovenous fistula development following frontotemporoparietal hemicraniectomy. This patient presented with intracerebral hemorrhage (ICH) secondary to underlying arteriovenous malformation (AVM) rupture. Following decompressive hemicraniectomy and follow-up successful resection of the underlying AVM, the patient returned to the hospital with a seizure. Repeat angiography was performed, demonstrating no intracranial vascular lesion recurrence. However, an incidental superficial temporal arteriovenous fistula was identified, which was not visualized o

Extrahepatic biliary obstruction occurs infrequently in cats. Pathophysiology of biliary obstruction is characterized by cholestasis, obstruction, inflammation, and exudation. Clinical signs and laboratory examination findings are nonspecific. Ultrasonographic examination of the biliary tract aids significantly in diagnosis. The aims of feline biliary surgery are to determine the underlying cause and extent of the obstruction, restore bile flow to the gastrointestinal tract, and prevent bile leakage and subsequent peritonitis. Surgical techniques to restore bile flow and decompress the biliary

Ogilvie's syndrome is a rare condition with progressive dilatation of the proximal colon without mechanical obstruction. Untreated it can lead to coecal perforation, peritonitis and death. It develops in patients with medical or surgical complications, but can be idiopathic. Caesarean section is the most common preceding surgical procedure. An imbalance between the parasympathetic and the sympathetic innervation of the intestine is thought to be the cause. Trauma to the retroperitoneum, infections, bleeding and electrolyte disturbances, hormonal changes and medicamina are predisposing factors.

Measurements of mid sagittal diameter (MSD) and interpedicular diameter (IPD) in patients operated on for central lumbar spinal stenosis were compared to the control group. Both groups can be matched in terms of gender and age. We found that in the stenotic patients the MSD and the IPD were smaller than in the control group, all of the measurements except the IPD in male stenotic patients was statistically different. Sagittal and axial MR images of the stenotic patients were used to evaluate the status of the posterior epidural fat which was graded as normal, small, very small and absent. All

This case report documents an acute rupture of the lateral head of the gastrocnemius muscle at the myotendinous junction and a lateral dislocation of the tibialis anterior tendon with a clinical presentation consistent with a compartment syndrome, despite the intraoperative finding of a rupture of the posterior and lateral compartments. Extensive hematoma formation led to marked edema, paresthesias, muscle weakness, and severe pain in the involved leg. Surgical repair of the torn muscle and dislocated tendon and evacuation of the dissecting hematoma resulted in a well-functioning extremity. Th

In 1971-1985 surgical treatment was carried out of 29 patients in the Neurosurgery Department WAM in Lódź and 5 in the Rehabilitation Centre in Konstancin for spondylolisthesis complicated with signs of cauda equina damage. In the surgical treatment the technique used was that of a combination of neurosurgical approach (full decompression of the nervous elements in the vertebral canal by laminectomy with foraminectomy or facetectomy, removal of protruding discs and osteophytes and parts of the vertebral bodies narrowing the vertebral canal), and orthopaedic procedures (spondylodesis between ve

BACKGROUND AND INTRODUCTION: Apersistent primitive trigeminal artery (PPTA) is an embryonic vessel that connects the cavernous part of the internal carotid artery with the posterior circulation. Though many are incidental, it is associated with conditions such as trigeminal neuralgia (TN).
OBJECTIVE: In this video, we present a case of TN due to neurovascular conflict with associate PPTA. The close association of PPTA with trigeminal nerve results in increased incidence of TN. Surgical view and technique of surgery are described in the video.
SURGICAL TECHNIQUE: A 55-year-old male presented

Background Portal vein aneurysms are rare dilations in the portal venous system, for which the etiology and pathophysiological consequences are poorly understood. Method We reviewed the existing literature as well as present a unique anecdotal case of a patient presenting with a very large portal vein aneurysm that was successfully managed conservatively and non-operatively without anticoagulation, with close follow-up and routine surveillance. Result The rising prevalence of abdominal imaging in clinical practice has increased rates of portal vein aneurysm detection. While asymptomatic aneury

INTRODUCTION: Silent sinus syndrome is an unusual cause of progressive enophthalmos and hypoglobus due to atelectasia of the maxillary sinus associated with osteolysis of the orbital floor. This syndrome is classically idiopathic, but the term is also used to describe traumatic or iatrogenic (surgical orbital decompression) cases.
CASE REPORT: We report the case of a 33-year-old man who presented with a left orbital trauma without functional disorder. Computed tomography (CT) scan revealed a nondisplaced fracture of the left orbital floor. No surgical indication was made. Three months later,

Orbital surgery is often conducted in areas with limited exposure where vital structures are tightly crowded together. A bloodless field is paramount in orbital surgery for the proper identification of normal and pathologic tissue and even minimal bleeding can obscure the surgical field, making surgery more difficult and increasing the risk of complications. Surgery for highly vascular orbital lesions is an additional situation where maintaining an adequate surgical field is often challenging but paramount. The role of the anaesthetist in controlling surgical blood loss has been increasingly r

The authors describe a case of spinal cord compression due to an osteochondroma arising from the T-6 vertebral body in a patient with hereditary multiple exostoses. This 16-year-old boy presented with spastic paraparesis. Surgical decompression was followed by resolution of the neurological impairments. Osteochondroma is the most common bone tumor. The distribution of osteochondromas greatly favors the extremities and these lesions rarely arise in the vertebral column. Osteochondromas occur in 2 distinct clinical settings--as solitary or multiple tumors, the latter being often associated with

BACKGROUND: Trigeminal neuralgia is usually caused by microvascular conflict with the fifth cranial nerve in the pontocerebellar angle. Rarely is it secondary to other extra-axial or intra-axial lesions. Few cases of trigeminal neuralgia due to cavernous angiomas have been reported in the literature. This is the first report of surgical decompression of the intra-axial trigeminal nerve tract from a deep pontine cavernous angioma.
METHODS: A 45-year-old man came to our attention for frequent and intense left facial pain episodes compatible with trigeminal neuralgia in the V1 and V2 branches, p

Microvascular decompression (MVD) is an effective form of surgery for the treatment of trigeminal neuralgia (TN), and is superior to other procedures utilized in the treatment of this condition. TN is commonly seen in the elderly, but there is debate regarding the use of MVD in elderly patients. This report presents a retrospective review of the clinical features and surgical outcomes in elderly patients with TN. We divided TN patients into two groups: an elderly group more than 70 years old and a younger group under 70 years old. There were no serious complications in either group, and the le

INTRODUCTION: Although the aging of the population is causing a dramatic rise in the incidence of lumbar spinal stenosis, the indications and options for surgical treatment are not clearly defined.
METHODS: In an attempt to aid clinical decision making, a selective literature review was conducted, taking into account the guidelines of the Association of the Scientific Medical Societies in Germany (AWMF).
RESULTS: In degenerative lumbar spinal stenosis hypertrophy of the facet joints and yellow ligaments brings about constriction of the spinal canal, leading to back pain and activity-dependen
