Designing new β-lactams: implications from their targets, resistance factors and synthesizing enzymes.

Penicillins and cephalosporins are β-lactam antibiotics widely used to treat bacterial infectious diseases. They mainly target the cell wall biosynthesis pathway to inhibit bacterial growth. The targets, known as penicillin-binding proteins, are enzymes involved in the polymerization of glycan chains, cross-linking them during bacterial cell wall formation. However, the dispensation of these antibiotics has been concomitant with increasing incidence of resistance to them. Reportedly, this is due to the evolvement of two resistance mechanisms in the bacterial pathogens. One is the production of

CCR1 as a target for multiple myeloma.

INTRODUCTION: By directing cell trafficking, differentiation and growth, chemokines modulate the immune response and are involved in the pathogenesis of autoimmune diseases and cancers, including multiple myeloma (MM). MM, the second most common hematological malignancy in the US, is characterized by disordered plasma cell growth within the bone marrow microenvironment. CCL3 and its receptors, CCR1 in particular, play a central role in the pathogenesis of MM and MM-induced osteolytic bone disease. AREAS COVERED: This review describes the functional role of CCR1 in MM and the preclinical resul

Development of a miniature, low-dose, frameless intrauterine levonorgestrel-releasing system for contraception and treatment: a review of initial clinical experience.

A low-dose levonorgestrel (LNG)-releasing intrauterine system (IUS) (FibroPlant) has been clinically developed since 1997 for endometrial suppression during hormone replacement therapy in peri- and postmenopausal women, for the treatment of menorrhagia in women with normal uteri or with uterine fibroids, for contraception, for the treatment of endometrial hyperplasia, and for alleviating primary and secondary dysmenorrhoea. Results of preliminary studies confirm the promising nature of this all-round drug delivery system. The low dose of LNG released accounts for the low hormonal side-effect r

Cell-laden hydrogel microspheres as functional 3D cellular units.

Engineering three-dimensional cellular systems with controlled size, matrix composition and cellular organization is central to disease modelling, drug evaluation and regenerative medicine. In this context, cell-laden hydrogel microspheres have emerged as a platform for constructing microscale living units. Their structure facilitates local cell-matrix interactions and cell-cell communication, while the hydrogel phase provides a tunable microenvironment for cell culture and function. Droplet microfluidics offers a powerful route to produce these microspheres with defined size, composition, arc

Strategies for Pancreatic Cancer-Responsive Nanodrug Platforms Targeting Tumor Hypoxic Environments.

Pancreatic ductal adenocarcinoma (PDAC) remains one of the most lethal malignancies, largely because of its profoundly hypoxic and desmoplastic tumor microenvironment (TME), which hinders drug delivery and promotes resistance to chemotherapy, radiotherapy, and immunotherapy. Hypoxia not only serves as a therapeutic barrier, but also presents a selective vulnerability that can be exploited by smart nanocarriers. This review highlights recent advances in hypoxia-responsive nanoparticle (NP) platforms designed to overcome these barriers through four key strategies: (1) hypoxia-triggered drug rele

Perspectives of biodegradable natural polysaccharides for site-specific drug delivery to the colon.

The ability to deliver drugs to the human colon in a specific manner has become feasible over the years. Targeting pharmaceutical drugs to the colon makes it possible to achieve local or systemic drug delivery to this site. To deliver the compounds in a non-degraded form to the lower part of the gastrointestinal tract, they must first of all pass through the stomach, the upper part of the intestine and must use the characteristics of the colon to specifically release the drugs in this part of the digestive tract. Among the different approaches available to achieve targeted drug release to the

Co-dispersion-mediated stabilization of a poorly soluble palmitoylation inhibitor within an alginate hydrogel for localized ferroptosis therapy.

Stabilizing high-loadings of hydrophobic small molecules in aqueous systems remains challenging due to crystallization and phase separation, which limit their therapeutic application. Here, we report a co-dispersion-mediated interfacial stabilization strategy that enables high-loading delivery of the poorly soluble palmitoylation inhibitor 2-bromopalmitic acid (2-BP) within an injectable alginate hydrogel system. Through high-shear co-dispersion with Fe₃O₄ nanoparticles, 2-BP undergoes amorphization and is stabilized in a uniformly dispersed state via interfacial interactions and spatial confi

Emerging Complexity and the Need for Advanced Drug Delivery in Targeting Candida Species.

BACKGROUND: Candida species are the important etiologic agents for candidiasis, the most prevalent cause of opportunistic fungal infections. Candida invasion results in mucosal to systemic infections through immune dysfunction and helps in further invasion and proliferation at several sites in the host. The host defence system utilizes a wide array of the cells, proteins and chemical signals that are distributed in blood and tissues which further constitute the innate and adaptive immune system. The lack of antifungal agents and their limited therapeutic effects have led to high mortality and

Transdermal therapeutic system of isradipine: effect of hydrophilic and hydrophobic matrix on in vitro and ex vivo characteristics.

Isradipine (ISDP) is an effective calcium channel blocker used in the treatment of hypertension. It undergoes extensive first pass metabolism and bioavailability through the oral route is only about 15 to 24%. Hence we attempted to develop a matrix type controlled transdermal drug delivery system for ISDP. Formulations A1, A2, A3 were composed of Eudragit RL100 and hydroxypropyl methyl cellulose (HPMC) in 1:3, 1:1, 3:1 ratios; A4, A5, A6 were composed of Eudragit RS100 and HPMC in 1:3, 1:1, 3:1 ratios. All six formulations carried 5 mg of ISDP/patch area, 5% v/w of D-limonene, 15 % v/w of prop

Enhanced oral delivery of insulin via the colon-targeted nanocomposite system of organoclay/glycol chitosan/Eudragit®S100.

This study aimed to develop a ternary nanocomposite system of organoclay, glycol-chitosan, and Eudragit®S100 as an effective colon targeted drug delivery carrier to enhance the oral absorption of insulin. A nanocomplex of insulin and aminoclay was prepared via spontaneous co-assembly, which was then coated with glycol-chitosan and Eudragit S®100 (EGAC-Ins). The double coated nanocomplex, EGAC-Ins demonstrated a high entrapment efficiency of greater than 90% and a pH-dependent drug release. The conformational stability of insulin entrapped in EGAC-Ins was effectively maintained in the presence

Transdermal delivery of sex steroids for hormone replacement therapy and contraception. A review of principles and practice.

The percutaneous route is an effective method for delivery of reproductive hormones. Several transdermal therapeutic systems (TTS) releasing estrogens, progestogens and androgens from patches attached to the skin are currently in clinical use. For women, transdermal systems have been developed for hormonal replacement therapy (HRT) and recently for contraception. HRT with patches releasing only estradiol (E2) should be supplemented with a progestogen to protect the endometrium. Patches simultaneously releasing both E2 and a progestogen are also available. Combined regimens are either continuou

Ternary Fucoidan-Cisplatin-Dendrimer Nanoparticles as a Platform for Drug Delivery in Glioblastoma.

Fucoidan, a sulfated marine polysaccharide, provides a versatile platform for constructing biocompatible nanostructures through controlled electrostatic assembly. Here, we report ternary nanoparticles formed by sequential complexation of aquated cisplatin with fucoidan, followed by stabilization with biodegradable cationic bis-MPA dendrimers. Fucoidan extracted from Fucus vesiculosus and Undaria pinnatifida, despite their differences in composition and structure, generated nanoparticles with comparable size, surface charge, and drug content. Both systems displayed pH-responsive cisplatin relea

Pulmonary Surfactant Nanoparticles for Lung-Targeted and Dose-Efficient Delivery.

Although pulmonary drug delivery enables lung-targeted therapy, its clinical efficacy is often limited by inflammation, heterogeneous deposition, and physiological barriers such as mucociliary clearance and phagocytic uptake. Here, we report a biomimetic platform based on pulmonary surfactant nanoparticles (PSNPs) that preserves native surfactant proteins and biophysical functionality, resulting in enhanced pulmonary delivery. Microfluidic synthesis yielded highly uniform PSNPs with improved encapsulation efficiency and colloidal stability, while markedly increasing surfactant protein retentio

Brain delivery of proteins with shuttle peptides.

Therapeutic proteins have emerged as potential candidates for the management of neurodegenerative disorders affecting the central nervous system (CNS). However, their effective delivery to the CNS remains a major challenge, primarily due to the presence of the blood-brain barrier (BBB). To overcome this challenge, several brain-targeted delivery strategies based on peptides have been developed. These peptides harness endogenous BBB transport pathways to enhance the delivery of therapeutic proteins to the CNS. In order to develop new potential protein-based therapeutic candidates, considerable

Glioblastoma: Overcoming fundamental biological and delivery barriers to therapy.

Glioblastoma remains the most aggressive primary malignant brain tumor in adults, with survival largely unchanged despite advances in molecular diagnostics and supportive care. Therapeutic failure reflects fundamental biological and anatomical barriers, including intratumoral heterogeneity, an immunosuppressive tumor microenvironment, and restricted drug delivery across the blood-brain barrier. In this Review, we summarize the current standard of care and critically examine emerging strategies aimed at overcoming these constraints, including locoregional delivery technologies, immunotherapy, b
