[Bilateral biceps replacement-plasty by transposition of the latissimus dorsi muscle in arthrogryposis multiplex congenita. A case report].

The case of a four-year old girl with arthrogryposis multiplex congenita is reported. Primary clinical examination revealed bilateral passive elbow flexion only 0-20-70 without active elbow flexion. A two-stage operative procedure was chosen for both sides. At first an elbow arthrolysis was performed with subsequent intensive physiotherapy. In a second session we performed a bipolar transposition of the latissimus dorsi as a myocutaneous flap. The patient was first operated on the left and afterwards on the right side. By this procedure, the patient achieved a significant improvement of both p

Unreduced dislocation of the elbow: case report and review of the literature.

A case report of an unreduced dislocation of the elbow requiring late open reduction is presented, and the literature is reviewed. Early reduction of elbow dislocation, by closed or open means, is of paramount importance if good functional results are to be obtained. Closed reduction of an elbow dislocation is unlikely to be successful if attempted later than 21 days after the injury. The results are acceptable if open reduction is undertaken within 3 months of the injury, but after 6 months the results are disappointing and arthroplasty is an alternative.

Impaired activation pattern in antagonistic elbow muscles of patients with spastic hemiparesis: contribution to movement disorder.

Surface electromyographic "EMG" activity in biceps brachii (agonist muscle) and triceps brachii (antagonist muscle) as well as the discharge behaviour of motor units "MUs" (needle recording) in biceps brachii muscle were recorded during slow (0.33 Hz) and fast (0.66 Hz) voluntary elbow flexion movements (auditory matching task) in fifty patients with spastic hemiplegia. In the spastic limbs, a long lasting, small amplitude tonic co-contraction of antagonist muscles was seen during slow flexions (SF) in 29 cases. This effect was strongest during the fast elbow flexion movements (FF). In 33 pati

Olecranon fractures. A clinical and radiographic comparison of tension band wiring and plate fixation.

Forty-one adult patients with displaced olecranon fractures were treated with open reduction internal fixation in a prospective, randomized study comparing tension band wiring (TBW) and plate fixation (PF). Plate fixation required longer operative time, but did not lead to an increased complication rate. Range of elbow motion at six months did not differ significantly between the two groups. Symptomatic metal prominence was frequently observed after TBW (42%), although true Kirschner wire (K-wire) migration was seen in only one patient. Postoperative loss of reduction, leading to a significant

Effects of ultrasound and trolamine salicylate phonophoresis on delayed-onset muscle soreness.

The purpose of this study was to determine the effects of ultrasound and phonophoresis using an anti-inflammatory-analgesic cream (trolamine salicylate) on delayed-onset muscle soreness (DOMS). Repeated eccentric contractions were used to induce DOMS in the elbow flexors of 40 college-aged women. Subjects were then assigned randomly to one of four groups: (1) group 1 (n = 10) received sham ultrasound using placebo cream, (2) group 2 (n = 10) received sham ultrasound using trolamine salicylate cream, (3) group 3 (n = 10) received ultrasound using placebo cream, and (4) group 4 (n = 10) received

Repair of avulsion of insertion of biceps brachii tendon.

An end-result study was made of 16 cases of avulsion of the insertion of the biceps brachii tendon. The follow-up period spanned one to 25 years post-treatment (average, eight years). Fourteen of the 16 cases included in the study were acute cases with primary repair and reinsertion of the biceps tendon into the area of the radial tuberosity. One case, with the loss of function of the biceps muscle secondary to temporary paralysis of the musculocutaneous nerve, was explored. The tendon was found intact and did not require repair. The sixteenth case was included for comparison of a patient who

Efficacy of recombinant human alpha-L-iduronidase (laronidase) on restricted range of motion of upper extremities in mucopolysaccharidosis type I patients.

The aims of the study were to assess the effectiveness of enzyme replacement therapy (ERT) with laronidase on the range of motion (ROM) of upper extremities and influence on activities of daily living (ADLs) of patients with mucopolysaccharidosis type I (MPS I). The ROM of 17 patients with MPS I was followed from the first year of life until the introduction of ERT and after 52-208 weeks of treatment. In all patients (group 1, n = 10), passive ROM was assessed. In patients with Hurler/Scheie or Scheie phenotype (group 2, n = 7) both passive and active ROM, as well as daily life activities, wer

Joint hypermobility syndrome in childhood. A not so benign multisystem disorder?

OBJECTIVES: Joint hypermobility (JH) or "ligamentous laxity" is felt to be an underlying risk factor for many types of musculoskeletal presentation in paediatrics, and joint hypermobility syndrome (JHS) describes such disorders where symptoms become chronic, often more generalized and associated with functional impairment. Clinical features are felt to have much in common with more severe disorders, including Ehlers-Danlos syndrome (EDS), osteogenesis imperfecta and Marfan syndrome, although this has not been formally studied in children. We defined the clinical characteristics of all patients

Two-stage surgical reconstruction for cubitus valgus secondary to neglected lateral condylar fracture non-union in older children.

BACKGROUND: This study aimed to evaluate the outcomes of a two-stage surgical protocol-comprising open reduction and internal fixation (ORIF) with bone grafting followed by corrective osteotomy-for managing cubitus valgus deformity secondary to neglected lateral condylar fracture non-union in older children. METHODS: We retrospectively reviewed nine patients (5 males, 4 females; mean age, 11.1 years; range, 10-14 years) who underwent this two-stage procedure between January 2015 and January 2023. The mean interval from initial injury to first surgery was 7.7 years (range, 6-10 years). All pat

Outcomes of suture bridge repair reinforced using a biceps rerouting technique.

BACKGROUND: Many techniques are available for arthroscopic rotator cuff repair, and autografts or allografts can be used in cases of poor tendon quality or large size tears. The long head of the biceps tendon is a good material for autografts. Rerouting of the biceps long head tendon during arthroscopic rotator cuff repair is a good option when repair is otherwise difficult. We analyzed the clinical results and retear rates over 2 years of follow-up after rotator cuff suture bridge repair reinforced using the biceps rerouting technique in patients with large rotator cuff tears. METHODS: We re

The use of vancomycin powder in total elbow arthroplasty; can we decrease infection risk?

BACKGROUND: Despite advances in implant technology and surgical techniques in total elbow arthroplasty (TEA), there remains a relatively high risk of postoperative infection compared to shoulder arthroplasty. The purpose of this study was primarily to investigate the use of intraoperative vancomycin powder (IVP) during TEA in regard to postoperative periprosthetic joint infections (PJIs). METHODS: A retrospective longitudinal review of patients undergoing TEA over a 22-year study period was performed. A total of 299 patients over the age of 18-years undergoing primary TEA with a minimum 6 mon

Reverse shoulder arthroplasty in upper extremity ambulators: a matched-cohort analysis.

BACKGROUND: Upper extremity ambulators (UEAs) who require prolonged use of assistive devices for mobility have a high incidence of shoulder pathology secondary to increased stress across the shoulder joint with upper extremity weight-bearing. Reverse shoulder arthroplasty (RSA) for rotator cuff arthropathy has historically been associated with increased complications in UEA, but more recent studies have shown more promising outcomes. The objective of this study is to evaluate clinical outcomes and complication rates between these 2 groups to define the relative risk of RSA in the UEA populatio

Epidemiology of isolated olecranon fractures: a detailed survey on a large sample of patients in a suburban area.

BACKGROUND: Literature lacks data concerning several epidemiologic aspects of isolated olecranon fractures (IOFs). The few studies that have analyzed this type of fracture show a low sample size and contradicting results. METHODS: This retrospective study included 165 consecutive patients (82 men and 83 women) who sustained an IOF in the past 10 years. Participants who were aged <16 years or had a previous elbow fracture or had a fracture that involved other bones of the elbow joint were excluded. Data regarding age, sex, season, date, and fracture side were collected. As per the mechanism of…

Anterior glenohumeral capsular ligament reconstruction with hamstring autograft for internal impingement with anterior instability of the shoulder in baseball players: preliminary surgical outcomes.

BACKGROUND: Shoulder pain and dysfunction are common in baseball players, and although "internal impingement" is recognized as one of the most common pathologies of shoulder dysfunction, the optimal surgical treatment for internal impingement with anterior instability of the shoulder remains controversial. This study evaluated baseball players' preliminary outcomes following anterior glenohumeral capsular ligament reconstruction for internal impingement with anterior instability of the shoulder. METHODS: Twelve baseball players (all male; mean age, 20.5 ± 2.2 years) with internal impingement

Outcomes of combined anterior latissimus dorsi and teres major tendon transfer for irreparable anterosuperior rotator cuff tears.

BACKGROUND: Irreparable anterosuperior rotator cuff tears (IASRCTs) can result in loss of active elevation, loss of internal rotation, and pseudoparalysis. Joint-preserving treatment options for IASRCTs in young and high-demand elderly patients are limited. The purpose of this study was to evaluate the clinical and radiologic outcomes of combined anterior latissimus dorsi and teres major (aLDTM) tendon transfer in patients with IASRCTs without reconstruction of the superior rotator cuff and capsular portion. METHODS: This retrospective study was conducted between 2015 and 2018. We included pa
