Assessment of phonophoresis and iontophoresis in the treatment of carpal tunnel syndrome: a randomized controlled trial.

To define the role of phonophoresis and iontophoresis of corticosteroids in conjunction with wrist splint use in the treatment of carpal tunnel syndrome (CTS) compared to wrist splint use alone, 52 CTS subjects were analyzed based on clinical and electrophysiological criteria. A prospective, randomized controlled trial was carried out to assess symptom severity, motor skills, and hand function according to the Boston Symptom Severity Scale (BSSS), grip strength, and nine-hole peg test (NHPT), respectively, on the initial visit and in the 3rd month after treatment. The patients underwent conser

Parallel facilitatory reflex pathways from the foot and hip to flexors and extensors in the injured human spinal cord.

Spinal integration of sensory signals associated with hip position, muscle loading, and cutaneous sensation of the foot contributes to movement regulation. The exact interactive effects of these sensory signals under controlled dynamic conditions are unknown. The purpose of the present study was to establish the effects of combined plantar cutaneous afferent excitation and hip movement on the Hoffmann (H) and flexion reflexes in people with a spinal cord injury (SCI). The flexion and H-reflexes were elicited through stimulation of the right sural (at non-nociceptive levels) and posterior tibia

[Cardiac arrhythmias in the elderly].

In the elderly, cardiac arrhythmias and conduction disturbances are characterized by their high frequency, diagnostic difficulties, low tolerance, and delicate treatment. Atrial fibrillation, the prevalence of which exceeds 10% after 80 years, is usually related to hypertensive or ischemic heart disease, and is the cause or the consequence of heart failure. It is first and foremost a cause of thromboembolic events, and especially cerebrovascular embolism. In elderly patients, sinus node dysfunction and AV block are often induced or aggravated by drugs. The iatrogenic risk associated with antia

Adductor T and H reflexes in humans.

In the belief that changes in the adductor reflex (AR) may be helpful in evaluating lumbar root and plexus lesions, expression of the AR was studied in 43 healthy human subjects. ARs elicited with an electronic reflex hammer were recorded from the inner side of the proximal thigh using needle and surface electrodes, and patellar reflexes (PRs) were recorded simultaneously. These reflexes were obtained by tapping the ipsilateral medial aspect of the knee, the contralateral patellar tendon, the ipsilateral and contralateral anterior superior iliac spines, and the Achilles tendon. The H reflex of

Prospective randomized comparison of closed cooled-tip versus 8-mm-tip catheters for radiofrequency ablation of typical atrial flutter.

INTRODUCTION: Cooled-tip and 8-mm-tip catheters have been found to be more effective than conventional 4-mm-tip catheters for radiofrequency (RF) ablation of common atrial flutter. The aim of this study was to compare the efficacy and safety of cooled-tip and 8-mm-tip catheters for flutter ablation in a randomized, prospective study. METHODS AND RESULTS: In 100 consecutive patients referred for ablation of common atrial flutter, cavotricuspid ablation was performed with a closed cooled-tip catheter (n = 50) or an 8-mm-tip ablation catheter (n = 50). RF current was applied for 60 to 120 second

Repetitive training of a synchronised movement induces short-term plastic changes in the human primary somatosensory cortex.

The aim of our study was to assess possible short-term plastic changes in the human primary somatosensory cortex (S1) induced by a repetitive synchronised movement of the right thumb and shoulder. We therefore performed a source localisation of somatosensory evoked potentials after median nerve stimulation in twelve healthy subjects before and after 1 h of motor training. We found a significant medial shift of the N20 dipole on the left hemisphere after training, whereas the dipole location on the right hemisphere remained unchanged. However, no significant correlation was seen between the dip

The role of Ca2+ stores in the muscarinic inhibition of the K+ current IK(SO) in neonatal rat cerebellar granule cells.

Cerebellar granule neurons (CGNs) possess a standing outward potassium current (IK(SO)) which shares many similarities with current through the two-pore domain potassium channel TASK-1 and which is inhibited following activation of muscarinic acetylcholine receptors. The action of muscarine on IK(SO) was unaffected by the M2 receptor antagonist methoctramine (100 nM) but was blocked by the M3 antagonist zamifenacin, which, at a concentration of 100 nM, shifted the muscarine concentration-response curve to the right by around 50-fold. Surprisingly, M3 receptor activation rarely produced a detec

Stereoselective interaction of thiopentone enantiomers with the GABA(A) receptor.

1. As pharmacokinetic differences between the thiopentone enantiomers seem insufficient to explain the approximately 2 fold greater potency for CNS effects of (-)-S- over (+)-R-thiopentone, this study was performed to determine any enantioselectivity of thiopentone at the GABA(A) receptor, the primary receptor for barbiturate hypnotic effects. 2. Two electrode voltage clamp recording was performed on Xenopus laevis oocytes expressing human GABA(A) receptor subtype alpha1beta2gamma2 to determine relative differences in potentiation of the GABA response by rac-, (+)-R- and (-)-S-thiopentone, and

cAMP increases apical IsK channel current and K+ secretion in vestibular dark cells.

Adenosine 3',5'-cyclic monophosphate (cAMP) is known to stimulate exogenous IsK channel current in the Xenopus oocyte expression system. The present study was performed to determine whether elevation of cytosolic cAMP in a native mammalian epithelium known to secrete K+ through endogenously expressed IsK channels would stimulate K+ secretion through these channels. The equivalent short circuit current (Isc) across vestibular dark cell epithelium in gerbil was measured in a micro-Ussing chamber and the apical membrane current (IsK) and conductance (gIsK) of IsK channels was recorded with both t

Measurement and simulation of noninactivating Ca current in smooth muscle cells.

An attempt was made to obtain electrophysiological evidence for continuous influx of Ca ion through voltage-dependent Ca channel (VDCC) in smooth muscle during long depolarization, for example in high K solution. Noninactivated Ca current [ICa(ni)] remaining after the accomplishment of voltage-dependent inactivation by prolonged depolarization for approximately 1 min was detected by three means under whole cell voltage clamp in several types of smooth muscle cells. The measurement of ICa(ni) was performed by micropuff application of Cd2+ or Ca2+ in the presence or absence of 5 mM extracellular

Ventricular fibrillation.

Ventricular fibrillation is the most common mechanism of sudden unexpected cardiac death in persons with asymptomatic or symptomatic coronary artery disease. The electrophysiologic mechanisms reviewed in this article include: automaticity of pacemaker fibers, transformation of nonpacemaker into pacemaker fibers, "injury" currents and reentry. Some of the conditions facilitating ventricular fibrillation include bradycardia, long QT syndrome, electrocution, electrolyte imbalance, drugs, sympathetic stimulation and myocardial ischemia. Electrophysiologic studies during acute myocardial ischemia s

A frequency-dependent phase-transition bioelectronic device with in-situ signal-filtering and Janus-adhesive capabilities for low-artifact sensing and tendon repair.

Mechanical and electrical mismatches exist at bioelectronic interfaces and tissue interfaces under complex environments, which easily lead to signal artifacts and adhesive residues. As a result, it is difficult to achieve both high sensitivity and anti-interference performance in bioelectronic interfaces. Inspired by multiple biological systems, this work proposes a multi-scale biomimetic construction strategy (JAH). An ionic-electronic hybrid conductive hydrogel with frequency-dependent phase transition is designed, and a Janus structure is constructed. In this way, two core functions, in-sit

Optimizing the thermoelectric performance of the earth-abundant layered phosphide BaCuP through defect engineering.

Thermoelectric materials offer a direct route for converting waste heat into electricity, but the discovery of efficient, sustainable, and non-toxic compounds remains a major challenge. Here, we investigate the thermoelectric potential of the layered 1-1-1 phosphide BaCuP using a combination of hybrid density functional theory, defect calculations, Boltzmann carrier transport theory, and machine-learning assisted lattice thermal conductivity simulations. The electronic structure reveals a small indirect band gap of 0.19 eV and strongly anisotropic valence-band dispersion, arising from Cu-P der

LMNA Deficiency Reveals a Role for SK3 Channel Dysfunction in Atrioventricular Block.

BACKGROUND: Atrioventricular block (AVB) is a conduction system disorder that, when severe, results in pacemaker implantation. LMNA mutations affecting the nuclear envelope proteins lamin A/C have been linked to AVB in patients, yet the underlying mechanisms remain largely unclear. The authors identified a nonsense LMNA-R225X mutation in a kindred that was associated with progressive AVB and atrial arrhythmias, followed by left ventricular dysfunction. OBJECTIVES: The aims of this study were to elucidate the mechanisms underlying LMNA-R225X-mediated AVB and to identify novel therapeutic targe

The use of biostimulants as a key to sustainable hydroponic lettuce farming under saline water stress.

BACKROUND: The utilization of high-quality water in agriculture is increasingly constrained by climate change, affecting availability, quality, and distribution due to altered precipitation patterns, increased evaporation, extreme weather events, and rising salinity levels. Salinity significantly challenges salt-sensitive vegetables like lettuce, particularly in a greenhouse. Hydroponics water quality ensures nutrient solution stability, enhances nutrient uptake, prevents contamination, regulates pH and electrical conductivity, and maintains system components. This study aimed to mitigate salt

Mapping the rat gastric slow-wave conduction pathway: bridging in vitro and in vivo methods, revealing a loosely coupled region in the distal stomach.

Rhythmic electrical events, termed slow waves, govern the timing and amplitude of phasic contractions of the gastric musculature. Extracellular multielectrode measurement of gastric slow waves can be a biomarker for phenotypes of motility dysfunction. However, a gastric slow-wave conduction pathway for the rat, a common animal model, is unestablished. In this study, the validity of extracellular recording was demonstrated in vitro with simultaneous intracellular and extracellular recordings and by pharmacological inhibition of slow waves. The conduction pathway was determined by in vivo extrac
