Microsurgical replantation for child total scalp avulsion.

Total scalp avulsion is an intractable problem in clinical practice. It often occurs in female adults, but rarely in children. In this article, we described the replantation of 2 scalp segments in a 4-year-old girl. The right segment involved the hairy scalp merely, whereas the left one involved the hairy scalp, forehead, eyelid, ear, and part of the face. Warm ischemia time was about 28 hours, and operation time was 13 hours. Although an explorative operation was performed, the left segment survived partly. A relatively satisfactory aesthetic result was obtained by transplanting split skin fr

[Improvement of registration accuracy for navigated-control drill in mastoidectomy (autopilot)].

BACKGROUND: The goal of this study is the improvement of the surgical accuracy of a navigate-controlled drill for mastoidectomy in a lab test. METHODS: For lab tests an artificial model of the temporal bone with color-coded injury identification of the facial nerve (solution of 0.5 mm) was used. Two different registration methods were examined: (group 1) navigation bow with 4 integrated markers at the upper jaw; (group 2) landmark registration with 4 titanium micro screws. An optical navigation system was used. The targets were illustrated by 3 titanium screws within the range of the planum m

Easy facial analysis using the facial golden mask.

For over 2000 years, many artists and scientists have tried to understand or quantify the form of the perfect, ideal, or most beautiful face both in art and in vivo (life). A mathematical relationship has been consistently and repeatedly reported to be present in beautiful things. This particular relationship is the golden ratio. It is a mathematical ratio of 1.618:1 that seems to appear recurrently in beautiful things in nature as well as in other things that are seen as beautiful. Dr. Marquardt made the facial golden mask that contains and includes all of the one-dimensional and two-dimensio

[Reconstruction of the oral commissure in microstomia patients with deep dermal facial burns: a modified technique].

BACKGROUND: Deep thermal dermal wounds of the oral region with subsequent microstomia are a rare and complex indication for surgical reconstruction of the oral commissure. Apart from functional and mimic aspects, aesthetic expectations are high. Based on converse technique, we performed a reconstruction of the oral commissure in 17 patients. 8 of these needed surgical re-operation due to recurrence and insufficient results. This high rate of complications led to a modification of the surgical procedure. PATIENTS: We present a modified technique which gave good results in our clinic. Based on

Forces necessary for the disruption of the cisternal segments of cranial nerves II through XII.

Manipulation of the cisternal segment of cranial nerves is often performed by the neurosurgeon. To date, attempts at quantifying the forces necessary to disrupt these nerves in situ, to our knowledge, has not been performed. The present study seeks to further elucidate the forces necessary to disrupt the cranial nerves while within the subarachnoid space. The cisternal segments of cranial nerves II through XII were exposed in six unfixed cadavers, all less than 6 hr postmortem. Forces to failure were then measured. Mean forces necessary to disrupt nerves for left sides in increasing order were

Systematic facial nerve monitoring in middle ear and mastoid surgeries: "surgical dehiscence" and "electrical dehiscence".

OBJECTIVES: To evaluate and systemize intraoperative facial nerve monitoring (IOFNM) in middle ear and mastoid surgeries. STUDY DESIGN AND SETTING: A prospective study. METHODS: IOFNM was performed in 100 patients undergoing middle ear and mastoid surgeries. We checked "surgical dehiscence" under microscopes, and also estimated the minimal threshold of electric current needed to change the electromyography of facial muscles using Nerve Integrity Monitor (NIM)-2 (Xomed, Minneapolis, MN, USA). RESULTS: Forty-three percent of cases showed "surgical dehiscence" and responded to electric stimula

Changes in expression of prosaposin in the rat facial nerve nucleus after facial nerve transection.

Prosaposin is the precursor of saposins A, B, C and D, which are activators of sphingolipid hydrolases. In addition, unprocessed prosaposin functions as a neurotrophic factor in the central and peripheral nervous systems by acting to prevent neuronal apoptosis, to elongate neurites and to facilitate myelination. In this study, the expression pattern of prosaposin in the facial nerve nucleus after facial nerve transection was examined by immunohistochemistry and in situ hybridization. Prosaposin immunoreactivity in the neurons on the operated side facial nerve nucleus showed a biphasic pattern:

Unusual case of resistant actinomycosis following facial trauma.

A case of actinomycosis is reported which appeared as a complication of a ski-pole injury of the face. A 16-year-old girl was treated for 10 years for resistant actinomycosis. She took long-term antimicrobial therapies, underwent repeated excisions, hyperbaric oxygen therapy, repeated skin grafting, immunological treatment, and free flap transfer. The conservative antibiotic treatment was complicated by mycosis and pseudomembranous colitis. After the defect healed up, satisfactory contour of the face was restored with additional free flap transfer. The patient has also been satisfied with the

Total lower lip functional reconstruction with a prefabricated gracilis muscle free flap.

Total lower lip reconstruction was performed in an 18-month-old boy following a dog bite. In order to obtain an optimal functional result and to avoid any additional facial scarring, a prefabricated gracilis muscle free flap was used in a two-stage procedure. Firstly, the muscle was delayed on its main pedicle, a 'tendinous' strip was inserted along its free border and a silicone sheet was slid under its predicted intraoral side. At the time of reconstruction, the neomucosal lining obtained in this way reconstituted the labial vestibule. Furthermore, the muscle, reinnervated by the mandibular

[Post-traumatic myo-arthropathy of the mandibular joint. Report of a case].

The author reports a case of post-traumatic myoarthropathy which was resolved through the search for a new centric ratio which was subsequently transformed in ICP with a varied vertical dimension. This was based on the assumption that post-traumatic intracapsular modifications impose this therapeutic objective given that they do not fall within the range of muscular adaptability.

Porencephaly and transverse limb defects following severe maternal trauma in early pregnancy.

A 13 year old boy with mental retardation, hemiatrophy and left hemiplegia, right hemifacial microsomia and transverse digital amputations is reported. He is moderately mentally retarded, experiences grand mal seizures and has evidence of a porencephalic cyst in the right frontotemporal region on CT scan. At 52 days post conception his mother was involved in a road traffic accident and suffered extensive bruising of the abdomen and concussion. It is suggested that the patient's congenital anomalies are a consequence of hypoxia and thrombo-embolic events sustained during early pregnancy.

Surgical correction of late sequelae from facial bone fractures.

This study has described the UCLA experience over a 5 year period in the treatment of secondary traumatic deformities utilizing the craniofacial techniques originally proposed by Tessier. Because of scarring, absent parts, and the malposition of segments, correction of long-standing traumatic deformities remains difficult and treatment results remain less than ideal. Primary correction utilizing the principles we have described herein still provides the best functional and aesthetic results. Facial fractures can no longer be considered as an independent and unrelated entity when dealing with t

Experience in the application of the transconjunctival route for surgical exposure in the orbital region.

The transconjunctival approach for surgical exploration of the orbital region is a simple, direct and uncomplicated method. It is in accordance with the desired goals of the surgeon, which is the avoidance of a conspicuous incision. Usually, the resulting scar is barely noticeable. This special approach can be used for a variety of clinical problems. A small number of complications may arise through the use of the transconjunctival approach, but with experience, appropriate maneuvers can be learned to avoid these minor problems. The transconjunctival approach to the orbital region should be us

Maternal and neonatal outcomes following emergency cesarean section or operative vaginal delivery for fetal distress in the second stage of labor.

BACKGROUND: Operative vaginal delivery and emergency cesarean section are critical interventions used in the second stage of labor when fetal distress threatens maternal or neonatal well-being. Prompt recognition of fetal compromise and timely decision-making are essential to ensure safe delivery outcomes while minimizing complications for both mother and baby. The aim of this study is to compare maternal and neonatal outcomes between operative vaginal delivery and emergency cesarean section in cases of fetal distress occurring during the second stage of labor. METHODS: This is a retrospectiv

Physiotherapy Approach to an Internal Capsule Infarct With Upper Motor Neuron Facial Nerve Palsy: A Case Report.

The internal capsule (IC) is a vital brain structure housing descending and ascending fiber tracts, with traditional assumptions about the corticobulbar and corticospinal tracts descending through the genu and anterior third of the posterior limb of internal capsule (PLIC), respectively. However, observations of IC infarctions reveal that symptoms often deviate from the expected fiber pattern, prompting a deeper exploration of these complexities. The posterior limb of the IC receives its blood supply from the lenticulostriate branches of the middle cerebral artery and the anterior choroidal ar
