Your turn: At home turning angle estimation for Parkinson's disease severity assessment.

People with Parkinson's Disease (PD) often experience progressively worsening gait, including changes in how they turn around, as the disease progresses. Existing clinical rating tools are not capable of capturing hour-by-hour variations of PD symptoms, as they are confined to brief assessments within clinic settings, leaving gait performance outside these controlled environments unaccounted for. Measuring turning angles continuously and passively is a component step towards using gait characteristics as sensitive indicators of disease progression in PD. This paper presents a deep learning-bas…

Biomechanical Effects of Manipulating Preferred Cadence During Treadmill Walking in Patients With ACL Reconstruction.

BACKGROUND: Abnormal gait is common after anterior cruciate ligament reconstruction (ACLR) which may influence osteoarthritis risk in this population. Yet few gait retraining options currently exist in ACLR rehabilitation. Cueing cadence changes is a simple, low-cost method that can alter walking mechanics in healthy adults, but few studies have tested its effectiveness in an ACLR population. Here, we evaluated the acute effects of altering cadence on knee mechanics in patients 9 to 12 months post ACLR. HYPOTHESIS: Cueing larger steps will facilitate larger knee angles and moments, while cuei…

Is foot clearance influenced by different types of dual task in once-only faller community-dwelling older adults?

BACKGROUND: Tripping is one of the main causes of falls in older adults and has an important association with minimum toe clearance (MTC). MTC variability while performing alternating (ADT) or concurrent (CDT) dual-task activities may be a useful gait parameter to differentiate once-only fallers from non-faller older adults. RESEARCH QUESTION: Is the MTC variability influenced by ADT and CDT in once-only faller community-dwelling older adults? METHODS: Twenty-two community-dwelling older adults with a self-report of up to one fall in the last 12 months were allocated to the fallers group and…

Persistent Lower Limb Deformities Despite Amelioration of Rickets in X-Linked Hypophosphatemia (XLH) - A Prospective Observational Study.

BACKGROUND: Gait deviations, lower limb pain and joint stiffness represent key symptoms in patients with X-linked hypophosphatemia (XLH, OMIM 307800), a rare disorder of mineral homeostasis. While the pathomechanism for rickets is well understood, the direct role of PHEX (Phosphate-regulating neutral endopeptidase) deficiency in non-rachitic features including complex deformities, skull and dental affections remains unclear. FGF23-inhibiting antibody treatment can normalize serum phosphate levels and to improve rickets in XLH patients. However, linear growth remains impaired and effects on low…

Patella Alta in Ambulatory Children With Cerebral Palsy: Prevalence and Functional Significance.

BACKGROUND: Gait dysfunction associated with patella alta (PA) in subjects with cerebral palsy (CP) has been presumed but not objectively established clinically or through biomechanical modeling. It is hypothesized that PA is associated with increasing level of motor impairment, increasing age, obesity, and worse stance phase knee kinematics and kinetics in children with CP. METHODS: Retrospective case series of 297 subjects with CP studied in our Motion Analysis Center. Data analyzed included patient demographics (age, body mass index, CP classification), patella height (Koshino-Sugimoto Ind…

Recovery of vestibular ocular reflex function and balance control after a unilateral peripheral vestibular deficit.

This review describes the effect of unilateral peripheral vestibular deficit (UPVD) on balance control for stance and gait tests. Because a UPVD is normally defined based on vestibular ocular reflex (VOR) tests, we compared recovery observed in balance control with patterns of recovery in VOR function. Two general types of UPVD are considered; acute vestibular neuritis (AVN) and vestibular neurectomy. The latter was subdivided into vestibular loss after cerebellar pontine angle tumor surgery during which a vestibular neurectomy was performed, and vestibular loss following neurectomy to elimina…

A perspective on the use of the CatWalk device to assess dynamic spasticity in rodent models.

INTRODUCTION: Spasticity is a hallmark of upper motor neuron syndrome and arises from complex neurophysiological and biomechanical alterations, including disinhibition of the stretch reflex and secondary changes in muscle properties. While rodent models are widely used to study spasticity, objective assessment of dynamic spasticity during locomotion remains challenging. The CatWalk (CW) system is a well-established tool for automated gait analysis; however, its specific utility in quantifying spasticity-related gait abnormalities has not been systematically evaluated. METHODS: A focused liter…

Opposing effects of foot-loading strategies on patellofemoral and tibiofemoral joint compression forces during walking.

BACKGROUND: Patellofemoral (PF) and tibiofemoral (TF) joint loading during gait are strongly influenced by sagittal-plane mechanics and muscle force distribution. Individuals with knee disorders often modify foot-loading patterns, yet the effects of such strategies on PF and TF joint compression forces (PFCF and TFCF) during walking remain unclear. RESEARCH QUESTION: How do forefoot, preferred, and rearfoot weight-bearing walking strategies differentially affect PFCF, TFCF, and knee muscle forces during the stance phase of gait? METHODS: Twenty-eight healthy adults performed forefoot, prefer…

Age-related changes in mediolateral stability control during walking.

BACKGROUND: Younger adults (YA) may primarily regulate mediolateral stability during walking through proactive control of net ground reaction force orientation, with residual instability during challenging gait conditions mediated by reactive force generation later in the gait cycle. We sought to understand if older adults (OA) use a similar reactive strategy to regulate mediolateral stability during walking when proactive control is limited. RESEARCH QUESTION: Do gait velocity and step width constraints differentially alter mediolateral gait stability control among healthy younger (18-35) an…

Foot placement coordination is impaired in people with Parkinson's disease.

BACKGROUND: Gait instability is a common and disabling symptom of Parkinson's disease (PD), contributing to frequent falls and reduced quality of life. While clinical balance tests and spatiotemporal gait measures can predict fall risk, they do not fully explain the underlying control mechanisms. In healthy individuals, foot placement is actively adjusted based on an estimate of the Center of Mass (CoM) state to maintain gait stability, known as foot placement control. This estimation relies on the integration of multisensory information, which has been shown to be impaired in PD, potentially …
