Hepatoprotective bile acid 'ursodeoxycholic acid (UDCA)' Property and difference as bile acids.

Ursodeoxycholic acid (UDCA) is a bile acid, which is present in human bile at a low concentration of only 3% of total bile acids. It is a 7beta-hydroxy epimer of the primary bile acid chenodeoxycholic acid (CDCA). UDCA is isolated from the Chinese drug 'Yutan' a powder preparation derived from the dried bile of adult bears. For centuries, Yutan has been used in the treatment of hepatobiliary disorders. In Japan, it has also been in widespread use as a folk medicine from the mid-Edo period. In Japan, not only basic studies such as isolation, crystallization, definition of the chemical structure

Managing Choledocholithiasis in Rare Congenital Hepatobiliary Anomalies: A Report of Three Cases.

Background: Endoscopic retrograde cholangiopancreatography is an established therapeutic modality for pancreaticobiliary disease. Congenital anatomical anomalies, including situs inversus totalis and rare hepatobiliary malformations, substantially increase the technical complexity of endoscopic retrograde cholangiopancreatography and are seldom encountered in clinical practice. Published experience is limited predominantly to individual case reports and smaller case series. Methods: We present a report of three patients who underwent endoscopic retrograde cholangiopancreatography: two with sit

Bacterial contributions to cancer development: mechanisms, dysbiosis, and cross-cancer associations.

Cancer is a multifactorial disease influenced by complex interactions between genetic, environmental, and microbial factors. Growing evidence demonstrates that several bacterial species contribute to carcinogenesis through chronic inflammation, production of genotoxic metabolites, disruption of cell-cycle regulation, and modulation of host immune responses. Helicobacter pylori–induced gastric adenocarcinoma remains the most well-established example of bacteria-driven oncogenesis; however, accumulating studies indicate that additional virulence mechanisms and bacteria-dependent pathways are als

CD109 Expression in Tumor and Stromal Cells Serves as a Prognostic Biomarker for Tumor Progression and Outcome in Gallbladder Adenocarcinoma.

CD109 is a glycosylphosphatidylinositol-anchored glycoprotein that is upregulated in various human cancers and exhibits tumor-promoting effects. In this study, we investigated the role of CD109 in gallbladder adenocarcinoma (GBAC). CD109 expression in 77 resected GBAC samples was immunohistochemically evaluated. CD109 expression in tumor cells correlated with TNM stage, N factor, histological grade, lymphatic invasion, and perineural invasion and was associated with reduced disease-free survival (DFS) and overall survival (OS). Stromal CD109 expression was detected in several cases, similar to

Immunotherapy-Facilitated Conversion Surgery for Biliary Tract Cancer: A Case Series of Long-Term Survivors Following Durvalumab Combined With Gemcitabine and Cisplatin.

BACKGROUND: Conversion surgery after systemic therapy is gaining attention in biliary tract cancer (BTC), yet evidence under gemcitabine, cisplatin, and durvalumab (GCD) remains scarce. CASE PRESENTATION: We report three patients with initially unresectable BTC (two gallbladder cancers and one intrahepatic cholangiocarcinoma) who underwent curative surgery following GCD therapy. All patients received six to eight cycles of GCD, achieving significant tumor control. Radiologic responses included tumor regression, nodal clearance, and vascular detachment. Surgery consisted of extended hepatectom

Multidisciplinary Surgical Treatment of Hepatic Abscess in a Geriatric Dog with Congenital Extrahepatic Portosystemic Shunt.

Hepatic abscesses are uncommon in dogs and typically develop secondary to biliary tract disease or ascending bacterial infections. Although congenital extrahepatic portosystemic shunt (EHPSS) is known to impair hepatic perfusion and immune clearance, its potential role in predisposing geriatric dogs to hepatic abscess formation has not been previously reported. This case report describes the diagnostic approach, therapeutic decision-making, and clinical outcome of a geriatric dog in which a multidrug-resistant hepatic abscess occurred in association with congenital EHPSS, and to propose a path

A case report of splenic malignancy originating from the liver.

INTRODUCTION: Splenic tumors are infrequently encountered in clinical practice, with a metastatic tumor incidence estimated to be approximately 2%-4% of all splenic tumors. Metastatic tumors originating from the liver are even more uncommon. Given the scarcity of reported cases and limited literature on splenic tumors, particularly in recent years, clinicians face significant challenges in effectively managing this disease. CASE PRESENTATION: In this report, we present a case of a malignant splenic tumor originating from the liver. The patient, a 58-year-old male, was admitted to our hospital

In-Hospital Outcomes of Robotic-Assisted Cholecystectomy in Obese versus Normal-Weight Patients: A National Inpatient Sample Analysis.

INTRODUCTION: Obesity increases perioperative risks in cholecystectomy due to its association with gallbladder disease. Robotic-assisted cholecystectomy (RC) may mitigate technical challenges in this population. This study compares in-hospital outcomes of RC between obese (BMI ≥30 kg/m2) and normal-weight (BMI 18-25 kg/m2) patients using National Inpatient Sample data. METHODS: A retrospective analysis of National Inpatient Sample data (2016-2021) identified 15,048 adults undergoing RC. Patients were stratified into obese (n = 4570) and normal-weight (n = 10,478) groups. Demographics, comorbi

An Unusual Case of Upper Digestive Bleeding-Metastatic Amelanotic Melanoma: Case Report and Literature Review.

Metastatic melanoma is one of the most common malignancies associated with the spread of the primary tumor. The primary site is usually the skin or the eyes. The most frequent site of metastases is the gastrointestinal tract, accounting for 60% of cases at autopsy. In 2% of patients, metastases occur without a detectable primary tumor. We present a rare case of upper digestive bleeding caused by multiple gastrointestinal tract metastases from an amelanotic melanoma. This case report describes a 65-year-old male who arrived at the emergency department after experiencing an episode of upper gast

Transcriptomic landscape of Gallbladder cancer reveals altered pathways related to cell cycle and Aurora kinase.

Gallbladder cancer (GBC) is a rare but aggressive biliary tract malignancy. This study explores the transcriptomic profile of GBC to identify differentially expressed genes (DEGs) and dysregulated pathways involved in its pathogenesis. RNA sequencing was performed on 13 GBC tumors and 6 matched controls. Functional enrichment analysis (FEA) as well as weighted gene co-expression network analysis (WGCNA) were used to identify dysregulated pathways, functionally relevant gene modules and hub genes. Key targets were validated in patient tissues and cell lines. A total of 1319 DEGs were identified

Feasibility of Surgical Resection for Advanced Gallbladder Carcinoma Based on Preoperative Clinical T Stagings: A Multicenter Retrospective Analysis (SNOW-22-05).

BACKGROUND/PURPOSE: Surgical resection remains the only potentially curative treatment for advanced gallbladder cancer (GBC). However, preoperative diagnosis and staging are often challenging, and the indication for extended resection remains controversial. This study evaluated the feasibility of upfront resection for advanced GBC based on clinical T (cT) staging. METHODS: Patients radiologically diagnosed with cT3 or cT4 GBC at six Japanese centers between 2010 and 2022 were retrospectively analyzed. Resection rate, benign lesions, perioperative outcomes, and overall survival (OS) were compa

A rare case of obstructive shock caused by a giant biloma with right heart compression in a patient undergoing hemodialysis.

A biloma is a localized bile collection caused by bile leakage that is usually resolved with percutaneous drainage. In most cases, clinical manifestations are limited to local symptoms such as abdominal pain and distension. Although bilomas can occasionally cause septic shock attributable to secondary infection or obstructive jaundice attributable to direct compression of the bile duct, cases involving heart compression by a biloma that leads to circulatory failure have been rarely reported. We report the case of a 79-year-old man undergoing maintenance hemodialysis who developed gallstone-ass

[Cholangiocarcinoma as a model disease: new pathways in personalized medicine].

Biliary tract cancer comprises a biologically heterogeneous group of malignant tumors, including intrahepatic, perihilar and distal cholangiocarcinoma, as well as gallbladder cancer. In cases of advanced disease, systemic treatment has changed significantly in just a few years. Immunochemotherapy with gemcitabine/cisplatin plus durvalumab or pembrolizumab has become the standard first-line treatment, while the choice of second-line therapy increasingly depends on molecular results. Clinically relevant alterations include FGFR2 fusions or rearrangements, IDH1 mutations, HER2 amplification or ov

Primary mucosa associated lymphoid tissue lymphoma of the gallbladder: A case report and review of literature.

BACKGROUND: The primary lymphomas of mucosa associated lymphoid tissue (MALT) of the gallbladder (GB) is an extremely rare of non-Hodgkin lymphoma. Many patients exhibit symptoms like gallstone disease, and in some cases, the lymphoma may be detected through imaging even without apparent symptoms. Only 19 cases of primary MALT lymphoma in the GB have been previously reported. Differential diagnosis from typical GB carcinoma based solely on imaging findings can be challenging, and definitive diagnosis often requires surgical intervention. CASE SUMMARY: We present a patient in an 82-year-old ma

Next-Generation models of Gallbladder Carcinoma: Linking biological insights to precision medicine.

Gallbladder carcinoma (GBC) is among the most aggressive and deadly malignancies of the biliary tract, with limited early diagnosis and treatment options largely due to a poor understanding of its complex tumor biology. Effective preclinical in vitro models are essential for advancing the understanding of its complex pathobiology and improving therapeutic strategies. Recent research has expanded experimental platforms for studying GBC biology beyond conventional two-dimensional (2D) cultures to include advanced three-dimensional (3D) systems, xenograft models, and emerging technologies such as
