Forty-three-Year Follow-up of Salvage Knee Reconstruction Using the Patella to Replace Lost Tibial Articular Surface: A Case Report.

CASE: A 19-year-old soldier presented with sudden onset of left knee pain and giving-way while running. Evaluation showed a pathologic fracture through a radiolucent lesion of the tibial metaphysis. Excision of a giant cell tumor of bone entailed removal of much of the lateral tibial articular surface. The joint surface was replaced by transposition of the patella, which was transfixed in position with an ASIF (Association for the Study of Internal Fixation) condylar blade plate. This technique resulted in satisfactory knee function that has persisted for 43 years. CONCLUSION: When excision o

Cherubism: An Unusual Study With Long-Term Follow-Up.

Cherubism is an uncommon hereditary benign fibro-osseous disorder characterized by bilateral enlargement of the mandible and the maxilla tha … f the biopsy specimen showed proliferating fibrous connective tissue interspersed by multinucleated giant cells. It was diagnosed as a case nonfamilial cherubism. An extensive long-term follow-up till 12 years was maintained. In 2014, after 12-years follow-up for this lesion, panoramic imaging examination showed progress of this lesion in mandible and maxilla …

Kidney involvement in medium- and large-vessel vasculitis.

Medium- and large-vessel vasculitides (MVV and LVV, respectively) comprise a heterogeneous group of disorders whose common denominator is the inflammatory involvement of vessels of medium and large size. This disease spectrum includes giant-cell arteritis and Takayasu's arteritis, which typically affect the aorta and its main branches, and Kawasaki's disease and polyarteritis nodosa, which involve medium-sized arteries. Chronic periaortitis, characterized by a perivascular fibro-inflammatory reaction affecting the abdominal aorta and the periaortic tissue, frequently has a systemic distributio

Tuberculosis arthritis of the metatarsal phalangeal: a rare location.

Tuberculosis TB is common in countries constituting endemic areas like Morocco, spinal sites represents half of osteo-articular locations, while peripheral locations in the limbs are rare. The authors relate in this observation the case of a particular location of tuberculosis arthritis. It is osteoarthritis of the metatarsophalangeal joint of the 2(nd) ray of the foot. Clinical signs were characterized by a moderately painful swelling of the dorsum of the foot with slow evolution. The definitive diagnosis was histologically obtained. Clinical cure was achieved after 09 months of medical treat

Case report: sublingual epidermoid cyst in an elderly patient.

The occurrence of epidermoid cysts in the floor of the mouth is uncommon, particularly in elderly patients. We present the case of a 77-year-old female, who presented with minimal symptoms, despite a large floor-of-mouth swelling, which obscured her tongue from vision. The mass was removed via an intraoral approach under local anaesthesia (LA) and intravenous (IV) sedation, keeping complications and recovery time to a minimum. The mass was revealed to be an epidermoid cyst and the patient made a swift recovery. The features and classification of these cysts are discussed.

Tongue infarction as first symptom of temporal arteritis.

Giant cell arteritis (GCA) is the most common systemic vasculitis affecting people over 50 years. This disease is a diagnostic challenge with a range of clinical symptoms and findings due to different affected vessels. Because of this, the initial diagnosis can be tricky, and some of the patients present at first time with a real unusual initial manifestation. One of these can be tongue necrosis, which is according to the literature in accordance with scalp necrosis, the rarest initial manifestation of GCA We describe a patient who presented with tongue necrosis as initial symptom of GCA. The

Polymyalgia rheumatica/arteritis temporalis and acquired factor VIII inhibitor.

Acquired haemophilia is a rare, but often life-threatening bleeding disorder caused by autoantibodies, mostly directed against factor VIII (FVIII inhibitor). The incidence of acquired haemophilia, which most commonly affects patients over 50 years of age, is estimated between 0.2 and 1 million per year. Although its aetiology remains obscure, 40-50% of cases are associated with other conditions, such as pregnancy, malignancies, drugs and autoimmune disorders as rheumatoid arthritis, systemic lupus erythematosus and Sjögren's disease. We report the case of a 69-year-old patient with spontaneous

Peripheral ameloblastoma: a case report.

Peripheral ameloblastoma is an uncommon pathological variant of the more usual intraosseous central ameloblastoma. It most typically presents as a localized soft tissue mass occurring in the tooth-bearing areas of the jaws and is often provisionally diagnosed as an epulis. In this paper, a 43-year-old male presented with a three-year history of a painful, slowly enlarging gingival swelling. There was no evidence of bony involvement on radiographs but it was revealed histologically to be a peripheral ameloblastoma. Treatment was by wide local excision, including a bony margin, and primary closu

Temporal arteritis: report of a case.

Temporal arteritis is a rheumatic disease that affects large and medium-sized arteries. It is a severe arteritis involving both the intima and media of the vessel and is a cause of headache that is frequently diagnosed erroneously as "atypical migraine." The patients have a burning or throbbing type of pain. Ultimately, there is localized inflammation or cellulitis over the swollen, tortuous artery. Jaw claudication, eye pain, photophobia, diplopia, and even blindness may accompany the temporal symptoms. As many as 20% to 60% of inadequately treated or untreated patients will lose their vision

[Polymyalgia rheumatica--fibromyalgia-syndrome: symptoms, syndromes or diseases?].

Polymyalgia rheumatica and Fibromyalgia are probably not distinct clinical entities. Rather polymyalgia rheumatica is the common denomiator of a large spectrum of different diseases. The historical separation between Polymyalgia rheumatica and Giant-Cell Arteritis is no more clinically applicable in most cases. A better clinical approach is to view them as extrems of a continuum. Similarly, fibromyalgia is one manifestation of chronic pain syndromes of undetermined etiology. In addition, fibromyalgia can often not be delineated clearely from functional disorders, including depression.

Polymyalgia rheumatica.

Polymyalgia rheumatica (PMR) is an inflammatory condition of unknown etiology characterized by aching and stiffness in the shoulder and in the pelvic girdles and neck. In the past, PMR was considered a manifestation of giant cell arteritis (GCA) or a variant of elderly-onset rheumatoid arthritis (EORA). The current diagnostic criteria for PMR were empirically formulated by clinical experts who had studied the disease extensively. Arthroscopic, radioisotopic and magnetic resonance imaging studies all have indicated the presence of a synovitis in proximal joints and periarticular structures. The

[Central nervous system vasculitis and of the peripheral nerves in the elderly].

Vasculitis of the nervous system are rare in the elderly. When present, they may constitute an urgent diagnosis and a therapeutic emergency. Clinical expression is rich and without specificity. Atypical signs (unusual course of dementia, systemic signs) or atypical laboratory results (inflammatory syndrome) may suggest the diagnosis of vasculitis. However, as multiple comorbidity is the rule in elderly subjects, searching for intercurrent factors (e.g. atrial fibrilation due to infectious disease causing embolic stroke) may be more contributive than searching for proof of a rare disease (vascu

Emergency department management of the acute headache.

Headache is a common complaint of patients seeking care at an emergency department (ED). A survey of more than 16,755 walk-in patients at an ED showed that 323 (1.9%) had a chief complaint of migraine (1). Almost one sixth of these patients had used the ED more than once. In fact, migraineurs used the ED and other health care providers 2 to 5 times more than nonmigraineurs (2). Fortunately, headaches associated with significant morbidity and mortality occur infrequently (3). The ED physician must be able to address the patient's need for pain management and establish the correct diagnosis for
