Update on rheumatology: part 1.

There are many rheumatic diseases. Part 1 of this 2 part series on rheumatology presented a few of those most commonly seen in the community. Home health clinicians can be helpful in managing these diseases and preventing progression by watching for new symptoms or acute attacks of pain or disability, ensuring that patients take their medications appropriately, reminding patients to see their rheumatology providers and have their lab work done regularly, and reporting adverse effects to medications promptly. Additionally, as with most home health patients, an interdisciplinary approach that in

Body mass index and premature mortality in physically heavily working men--a ten-year follow-up of 20,000 construction workers.

OBJECTIVE: Although obesity is clearly associated with diabetes and cardiovascular disease, health consequences of being mildly or moderately overweight are less clear in physically heavily working men. METHODS: The association between body mass index (BMI) and mortality was assessed over a 10-year period in a cohort of 19,513 male construction workers, who underwent routine occupational health examination during 1986 to 1992. RESULTS: Among never smokers normal and overweight men experienced similar mortality. Among smokers, we observed a strong U-shaped association between BMI and all-caus

A review of dietary and environmental correlates of obesity with emphasis on developing countries.

The relationships between dietary and environmental factors and obesity are reviewed. Findings from selected population studies of diet and body weight are presented. In general, the results from population studies of diet and obesity have been inconsistent and marked with methodological weaknesses, especially the use of cross-sectional study design. Apart from the diet, several social and economic factors appear to be important correlates of obesity in the population. However, most studies have focused on the socioeconomic status as a broad, composite measure. The relationships between income

An interdisciplinary psychoeducation program for schizophrenic patients and their families in an acute care setting.

A psychoeducation program in an acute care inpatient psychiatric setting involves schizophrenic patients and their families in a range of educational interventions that are responsive to their particular strengths and vulnerabilities. Patients receive one-on-one instruction about their illness from a psychiatrist and the nursing staff and learn community living skills in classes and tutorials conducted by occupational therapists. Families discuss ways of coping with the patients' illness in a series of meetings with a social worker, and they attend workshops led by a team of clinicians and an

Hygienic-toxicological aspects of exposure to lead. A critical review.

This review on hygienic, toxicologic and ecologic aspects of environmental burden of lead deals with the following topics: occurrence, production and use of lead; metabolism of lead and experimental toxicology of lead; toxicity of lead to man; hygienic and ecologic aspects of lead pollution. Lead ore mining and processing, production of lead-acid batteries and combustion of gasoline containing tetraethyl-lead as an anti-knock ingredient constitute at present the most important sources of lead pollution. The last named source of lead pollution in urban atmosphere is most hazardous to human heal

Mental health and wellbeing interventions for uniformed service personnel: a mixed methods systematic review.

BACKGROUND: Uniformed service personnel are routinely exposed to occupational trauma in their roles which contribute to elevated rates of mental health conditions. A wide range of mental health and wellbeing interventions may help uniformed service personnel manage their psychological responses to challenging incidents at work. Previous reviews of those interventions have focused on single groups of professionals or intervention types, limiting cross-sector insights. This review uses a mixed methods approach to synthesise evidence associated with mental health and wellbeing interventions acros

Volatile organic compounds exposure and all health outcomes: An umbrella review and evidence map.

Volatile organic compounds (VOCs) are common environmental and occupational pollutants, yet evidence on their health impacts remains fragmented. This umbrella review aimed to integrate and evaluate the credibility of associations between exposures to major VOCs and adverse health outcomes. We conducted an umbrella review of meta-analyses, systematically searching PubMed/MEDLINE, Embase, CINAHL, and Google Scholar from inception to May 30, 2025. Eligible studies were meta-analyses of observational studies (cohort, case-control, and cross-sectional) that investigated exposure to VOCs and health

One-step construction of a thiolysis-driven chemodosimeter for robust colorimetric and turn-on fluorometric hydrogen sulfide detection integrated with a smartphone platform for environmental monitoring.

Herein, a novel dinitrophenyl-appended phthalonitrile chemodosimeter DNPPn, readily synthesized in a single step, was introduced as a colorimetric and fluorescent probe for the selective and sensitive detection of hydrogen sulfide (H2S), and validated in real sample matrices. The molecular structure of the DNPPn was comprehensively elucidated using single-crystal X - ray diffraction analysis (XRD), and well-established spectroscopic techniques (1H NMR, 13C NMR, ESI-MS, and FT-IR). DNPPn exhibited a broad concentration range, and dual-mode (colorimetric and turn-on fluorometric) sensing capabil

Under pressure: A systematic review of the mental health impact of COVID-19 pandemic on mental health workers.

The COVID-19 pandemic emerged suddenly, profoundly impacting the lives of us all, including mental health workers (MHW). This unprecedented crisis introduced significant challenges for MHW, exposing them to increased risks of psychological distress. This systematic review aims to evaluate the psychopathological effects of the COVID-19 pandemic on MHW. Additionally, it seeks to explore factors-social, regional, occupational, and others-influencing these effects. A systematic search was conducted across PubMed, PsycINFO, and Embase databases following PRISMA guidelines. A total of 2621 citations

Patient experiences and perspectives of health service access for carpal tunnel syndrome in Aotearoa New Zealand: a normalisation process theory-informed qualitative study.

BACKGROUND: Early access to care for carpal tunnel syndrome (CTS) can avoid higher rates of surgery and permanent harm yet is often delayed, particularly for populations more likely to underutilise care. OBJECTIVE: We sought to explore patient experiences and perspectives of health service access for CTS to inform an equity-focussed co-design of a health service for improving early care access. METHODS: In this Normalisation Process Theory (NPT)-informed qualitative study we conducted semistructured in-depth interviews with 19 adults with experience of CTS. Recruitment prioritised New Zealan

Anthrax attack at the United States Capitol. Front line thoughts.

One great fear was realized on October 15, 2001 when United States citizens witnessed firsthand the unprecedented release of anthrax into a community. Although the office of the Attending Physician to Congress had been preparing for such an unthinkable act, lessons were learned as the events unfolded. The following is a summary of the findings: Preparation, planning, and frequent review of bioterrorism response procedures are essential. Effective communication remains the key to successful team performance. Briefings conducted daily and on an as needed basis shape the progress and performance

Health and immunology study following exposure to toxigenic fungi (Stachybotrys chartarum) in a water-damaged office environment.

There is growing concern about adverse health effects of fungal bio-aerosols on occupants of water-damaged buildings. Accidental, occupational exposure in a nonagricultural setting has not been investigated using modern immunological laboratory tests. The objective of this study was to evaluate the health status of office workers after exposure to fungal bio-aerosols, especially Stachybotrys chartarum (atra) (S. chartarum) and its toxigenic metabolites (satratoxins), and to study laboratory parameters or biomarkers related to allergic or toxic human health effects. Exposure characterization an

Why most workers with occupational repetitive trauma do not file for workers' compensation.

Despite the availability of no fault insurance for wage replacement and medical care costs, the majority of workers diagnosed with an occupational disease do not apply for workers' compensation. The objective of the study was to determine the reasons why workers diagnosed with work-related musculoskeletal disease did not apply for workers' compensation benefits. A cross-sectional study of 1598 individuals diagnosed with neck, upper extremity, and low back work-related musculoskeletal disease from April to June 1996 was performed. All individuals were interviewed over the telephone using a stan

Tobacco-related health harms among Indigenous communities in India: Commercial determinants of health perspective.

Tobacco use has remained high among Indigenous communities in India over time, compared to other social groups. In this commentary, I use the lens of "commercial determinants of health" to understand commercial drivers of tobacco-related health harms among Indigenous communities, at three levels: the products, industry activities, and structural factors. At the level of products, we need to recognise diverse products and patterns of tobacco use, while differentiating the traditional use of tobacco from commercial tobacco use. Certain industry activities including "sachetisation" of tobacco pro

Defining severity of personality disorder using electronic health records: short report.

Severity of personality disorder is an important determinant of future health. However, this key prognostic variable is not captured in routine clinical practice. Using a large clinical data-set, we explored the predictive validity of items from the Health of Nation Outcome Scales (HoNOS) as potential indicators of personality disorder severity. For 6912 patients with a personality disorder diagnosis, we examined associations between HoNOS items relating to core personality disorder symptoms (self-harm, difficulty in interpersonal relationships, performance of occupational and social roles, an

Health-Promoting Physical Activity of Children Who Use Assistive Mobility Devices: A Scoping Review.

Children with physical disabilities who use assistive mobility devices (AMDs) are at risk for obesity and other secondary health conditions. Habitual physical activity is one lifestyle factor that may prevent obesity and contribute to overall health, and an active lifestyle in childhood improves prospects for lifelong healthy behaviors. Child, family, and environmental facilitators and barriers influence health-promoting physical activity (HPPA) for children without disabilities, but comparable models and levels of understanding for children who use AMDs are lacking. In this scoping review, we
