Generative AI-Assisted Progressive-Disclosure Case-Based Learning for Clinical Reasoning in Occupational Medicine: Quasi-Experimental Study.

BACKGROUND: Case-based learning (CBL) promotes transfer of knowledge to practice, yet occupational health CBL must also develop exposure assessment and epidemiologic thinking. Static, single-session cases that disclose all information upfront can truncate iterative reasoning and encourage premature diagnostic closure. Generative AI (GenAI) can support the efficient development of high-fidelity, progressively disclosed cases, but hallucination risks require strict quality control. OBJECTIVE: This study aimed to develop and evaluate a multicomponent GenAI-assisted, 4-act progressive-disclosure

Risk factors for early revision following primary total shoulder arthroplasty in a single health system.

BACKGROUND: Shoulder arthroplasty has emerged as a reliable treatment for end-stage glenohumeral arthritis. However, as the volume of total shoulder arthroplasty (TSA) increases nationwide, revision procedures are expected to rise in parallel. These surgeries are already known to be technically challenging and associated with higher complication rates, longer recovery, and increased costs. Despite this growing demand, limited large-scale data exist to identify which patient factors predict early failure and adverse outcomes. This study aimed to evaluate predictors of revision and postoperative

The effect of a nurse-led health education intervention on treatment delay in patients with diabetic high-risk foot: A randomised controlled trial.

BACKGROUND: Diabetic high-risk foot-the pre-ulcerative phase of diabetic foot disease-offers a critical window for prevention. Insufficient self-care and during this stage markedly increases ulcer risk, and once ulcers develop, treatment postponement precipitates infection, amputation, and death. Targeted education that enhances self-care and motivates prompt consultation is therefore essential, yet structured nurse-led healthcare-seeking programs specifically designed for diabetic high-risk foot remain scarce. OBJECTIVE: To develop and evaluate the effects of a nurse-led health education pro

Oncofertility and Reproductive Counseling in Patients with Breast Cancer: A Retrospective Study.

INTRODUCTION: Improving the prognosis of breast cancer patients is of utmost importance in terms of increasing survival rates. Modern medicine has therefore prioritized better quality of life for patients, even after the disease, through a better management of the potential long-term side effects induced by anticancer treatments. Fertility preservation and family planning are therefore crucial issues to be addressed in all cancer patients of reproductive age. Along those lines, a new branch of medicine with distinct multidisciplinary characteristics has developed over the years: oncofertility.

Neuropsychiatric Ramifications of Severe COVID-19 and Other Severe Acute Respiratory Infections.

IMPORTANCE: Individuals surviving severe COVID-19 may be at increased risk of neuropsychiatric sequelae. Robust assessment of these risks may help improve clinical understanding of the post-COVID syndrome, aid clinical care during the ongoing pandemic, and inform postpandemic planning. OBJECTIVE: To quantify the risks of new-onset neuropsychiatric conditions and new neuropsychiatric medication prescriptions after discharge from a COVID-19-related hospitalization, and to compare these with risks after discharge from hospitalization for other severe acute respiratory infections (SARI) during th

An Analysis of Essential Pediatric Surgical Cases Encountered During a Decade of Large-Scale Military Humanitarian Aid Missions.

INTRODUCTION: Disease Control Priorities, 3rd Edition (DCP3) is an evidence-based, published resource that outlines essential procedures recommended for developing health care systems. These systems must consider various populations and the incidence of certain surgical conditions that require treatment. In relation to pediatric patients, the prevalence of certain surgical conditions encountered remains unclear in several low- and middle-income countries. Over the past 15 years, the USNS Mercy, one of the U.S. Navy's large hospital ships, has led the Pacific Partnership missions, which provide

Determining an Appropriate Sample Size for Qualitative Interviews to Achieve True and Near Code Saturation: Secondary Analysis of Data.

BACKGROUND: In-depth interviews are a common method of qualitative data collection, providing rich data on individuals' perceptions and behaviors that would be challenging to collect with quantitative methods. Researchers typically need to decide on sample size a priori. Although studies have assessed when saturation has been achieved, there is no agreement on the minimum number of interviews needed to achieve saturation. To date, most research on saturation has been based on in-person data collection. During the COVID-19 pandemic, web-based data collection became increasingly common, as tradi

Feasibility and Acceptability of a Motivational Interviewing-Based Telehealth Intervention for Bacterial Sexually Transmitted Infection Screening: Protocol for a Sequential Explanatory Mixed Methods Study.

BACKGROUND: Gay, bisexual, and other men who have sex with men living with HIV (GBMSM-LWH) in the United States bear a heavy burden of bacterial sexually transmitted infections (STIs). Timely diagnosis and treatment are key to prevention. Only a few studies have combined home specimen self-collection for bacterial STI screening with live audio and video (AV) conferencing. None have focused on GBMSM-LWH or incorporated motivational interviewing (MI), a client-centered, strengths-based counseling approach that seeks to support individuals to create positive behavioral change. OBJECTIVE: Our stu

Methods development for epidemiologic investigations of the health effects of prolonged ozone exposure. Part II. An approach to retrospective estimation of lifetime ozone exposure using a questionnaire and ambient monitoring data (California sites).

An extensive body of data supports a relation between acute exposures to ambient ozone and the occurrence of various acute respiratory symptoms and changes in measures of lung function. In contrast, relatively few data are available on the human health effects that result from long-term exposure to ambient ozone, Current efforts to study long-term ozone-related health effects are limited by the methods available for ascertaining lifetime exposures to ozone. The present feasibility study was undertaken as part of the Health Effects Institute's Environmental Epidemiology Planning Project (Health

Digital interventions for the management of chronic obstructive pulmonary disease.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is associated with dyspnoea, cough or sputum production (or both) and affects quality of life and functional status. More efficient approaches to alternative management that may include patients themselves managing their condition need further exploration in order to reduce the impact on both patients and healthcare services. Digital interventions may potentially impact on health behaviours and encourage patient engagement. OBJECTIVES: To assess benefits and harms of digital interventions for managing COPD and apply Behaviour Change Tec
