[Reaction of synovial membranes in knee joint with primary extra-articular systemic diseases (author's transl)].

Postmortal histologic examinations of the synovial membrane of both knee joints were conducted in 85 selected cases. The following conditions were given priority: diabetes mellitus, advanced renal insufficiency, liver cirrhosis with ascites, chronic insufficiency of the right heart, vericose syndrome of the lower extremities, tumours compressing organs of the pelvis minor as well as a few rare infectious and tumerous diseases. Comparison of the histomorphologic findings revealed consistancies such as pronounced ultravillous branching of the synovial membrane with villous hyalinosis in diabetes

Development of a Metastatic Spinal Tumor Frailty Index (MSTFI) Using a Nationwide Database and Its Association with Inpatient Morbidity, Mortality, and Length of Stay After Spine Surgery.

OBJECTIVE: The aim of this study was to develop a perioperative metastatic spinal tumor frailty index (MSTFI) that could predict morbidity, mortality, and length of stay. METHODS: A large inpatient hospitalization database was searched from 2002 to 2011 to identify 4583 patients with spinal metastasis from breast (21.1%), lung (34.1%), thyroid (3.8%), renal (19.9%), and prostate (21.1%) cancer who underwent surgery. A multiple logistic regression model identified 9 independent parameters that were used to construct the MSTFI: anemia, chronic lung disease, coagulopathy, electrolyte abnormaliti

Association Between Automated Coronary Artery Calcium From Routine Chest Computed Tomography Scans and Cardiovascular Risk in Patients With Colorectal or Gastric Cancer.

BACKGROUND: As cardiovascular disease (CVD) is the leading cause of noncancer mortality in colorectal or gastric cancer patients, it is essential to identify patients at increased CVD risk. Coronary artery calcium (CAC) is an established predictor of atherosclerotic CVD; however, its application is limited in this population. This study evaluates the association between automated CAC scoring using chest computed tomography and atherosclerotic CVD risk in colorectal or gastric cancer patients. METHODS: A retrospective cohort study was conducted using electronic health records linked to claims

A Rare Case of Diffuse Large B Cell Lymphoma Presenting as a Cardiac Mass.

BACKGROUND Primary mediastinal diffuse large B cell lymphoma (DLBCL) presenting as a large intracardiac tumor is extremely rare and has not been significantly reported in the literature. Cardiac lymphoma consists of 2 subtypes: mediastinal DLBCL invading the heart and primary cardiac lymphoma. Both subtypes have a poor prognosis and are treated similarly. Mediastinal DLBCL is a life-threatening condition that, if diagnosed early, has a better survival rate. This is a rare case of a mediastinal DLBCL invading the right atrium as a large intracardiac mass, causing partial obstruction of the tric

18F-FDG PET/CT in a cardiac metastasis in a patient with history of malignant neuroectodermal tumour of the chest wall: Case report and review of the literature.

The case presented is a 25-year-old male with a malignant neuroectodermal tumour on the left chest wall (Askin tumour), treated with surgery after neoadyuvant chemotherapy and followed by consolidation chemotherapy. After 9 years of disease free survival, the patient developed an acute pulmonary embolism. The echocardiogram, thoracic CT, and cardiac MRI scans revealed a mass in the right atrium. Recurrence of an Askin tumour versus an atrium myxoma was suspected. 18F-FDG PET/CT showed an intense hypermetabolic right atrium mass with extension to the right ventricle highly suggestive of maligna

[Superior vena cava thrombosis or stricture secondary to implanted central venous access: Six cases of endovascular and direct surgical treatment in cancer patients].

Superior vena cava (SVC) stenosis or thrombosis is a well-known complication of central venous catheterization for endocavitary treatments, hemodialysis, or chemotherapy. In cancer patients, these SVC lesions are often symptomatic due to intimal damage and chemotherapy toxicity. We report our experience with six patients treated between 2007 and 2012 via an endovascular approach (n=5) or a direct surgical approach (n=1). All patients had SVC syndrome with facial edema, headache and upper limb edema. In three cases, the catheter was in place when the clinical symptoms occurred. Duplex Doppler a

Neurogenic stunned myocardium after acute hydrocephalus.

Neurogenic stunned myocardium (NSM) is a syndrome of cardiac stunning after a neurological insult. It is commonly observed after aneurysmal subarachnoid hemorrhage but is increasingly being reported after other neurological events. The underlying mechanism of NSM is believed to be a hypothalamic-mediated sympathetic surge causing weakened cardiac contractility and even direct cardiac myocyte damage. The authors report 2 cases of NSM in pediatric patients after acute hydrocephalus. Both patients experienced severe cardiac dysfunction in the acute phase but ultimately had a good neurological out

Simultaneous aortic valve replacement and left lower lobectomy: technical considerations.

An 84-year-old patient underwent combined aortic valve replacement and left lower lobectomy (LLL) for bronchogenic cancer. At surgery, which was performed via median sternotomy, the fissure was found to be totally fused. The fissure, artery, and bronchus were treated off pump, while the left inferior pulmonary vein and the inferior ligament were sectioned on pump. Finally, the stenotic aortic valve was replaced with a bioprosthesis. The postoperative course was smooth and the patient discharged on the ninth day. The simultaneous treatment of lung cancer and cardiac disease has been widely desc

An evaluation of two techniques for beam intensity modulation in patients irradiated for stage III non-small cell lung cancer.

In locally advanced lung cancer, the use of high dose radiotherapy (RT) and/or concurrent chemo-RT is associated with significant pulmonary and esophageal toxicity. Despite a 3D conformal RT technique and the omission of elective mediastinal fields, three (of ten) patients with inoperable stage 3 NSCLC who were treated with induction chemotherapy (carboplatin-paclitaxel) followed by RT to 70 Gy, developed symptomatic radiation pneumonitis. In this planning study, the actual treatment plans of all ten patients were compared to plans derived using two beam intensity-modulated (BIM) techniques, f

Transcriptomic analysis reveals heart failure-related lncRNA LINC00670 as a regulator of colon cancer mesenchymal genes.

Recent evidence suggests that cardiovascular diseases and cancer are linked through circulating factors, mainly proteins with oncogenic properties found in heart failure (HF) patients. However, the role of long non-coding RNAs (lncRNAs) as potential factors in cancer progression associated with HF remains unknown. This study aims to determine the role of HF-related lncRNAs in cancer progression. Using public RNA-sequencing datasets from the left ventricle and whole blood of HF patients, as well as colon cancer tumors, we identified HF-related lncRNAs with predicted competing endogenous RNA (ce

Safety and Efficacy of Unresectable Malignant Cardiac Tumors Treated with Concurrent Chemoradiation Therapy Using a 1.5T MR-Linac (GASTO-1078).

PURPOSE: This trial aimed to investigate the safety and efficacy of treating malignant cardiac tumors with hypofractionated radiation therapy and concurrent chemotherapy (hypo-CCRT) using a 1.5T MR-Linac. METHODS AND MATERIAL: Patients with both primary and secondary malignant cardiac tumors underwent split-course hypo-CCRT on a 1.5T MR-Linac, receiving 30 Gy/6 fractions initially, followed by a boost course of 20 to 30 Gy/4 to 6 fractions, totaling 50 to 60 Gy. Concurrent chemotherapy (docetaxel and nedaplatin) was administered weekly. The primary endpoint was local recurrence-free survival

Evaluating cardiovascular risk factors in breast cancer survivors: The role of echocardiography and cardiopulmonary exercise testing in the Munich Cardio-Oncology-Exercise retrospective Registry.

BACKGROUND: Breast cancer survivors (BCS) have an increased risk of developing cardiovascular disease risk factors (CVDRF). However, the role of cardiopulmonary fitness, echocardiography and treatment regimen e.g. anthracycline, HER2-targeted therapy (AC/H) or endocrine therapy (ET) are uncertain. METHODS: This single-center, retrospective study included BCS without visceral metastases, who had been treated with either AC/H or ET and assessed by echocardiography, cardio-pulmonary exercise testing (CPET) and the H2FPEF score at baseline. We included BCS with a left ventricular ejection fractio
