Characteristics of mononuclear phagocytes mediating antilisterial resistance in splenectomized mice.

The characteristics of mononuclear phagocytes mediating resistance to infection with Listeria monocytogenes during the early phase (up to 48 h) of the response were investigated in mice of the A strain that had undergone splenectomy. Although irradiation in the sham-operated host had no effect on its antilisterial response when administered immediately before infection, it markedly reduced the ability of the splenectomized host to resist listerial challenge. This effect of radiation was demonstrable in the high-dose range (600 r) and could not be reversed immediately by repopulation with 20 x

Survival Outcome Was Better with Liver Resection than with Percutaneous Radiofrequency Ablation in Patients with Very Early-Stage Hepatocellular Carcinoma: A Single-Center Retrospective Study.

BACKGROUND: Antiviral therapies for hepatitis B or C virus may be associated with reduced risk of tumor recurrence and mortality of patients with hepatocellular carcinoma (HCC) undergoing curative treatments, compared to untreated patients. Previous studies comparing survival outcomes between percutaneous radiofrequency ablation (RFA) and liver resection (LR) for treating patients with very early-stage HCC did not evaluate the effects of antiviral treatments on survival outcome. Our study aimed to clarify this issue. METHODS: From 2011 to 2021, 265 patients undergoing LR and 269 patients unde

Surgery for Gastroenteropancreatic Neuroendocrine Tumors with Synchronous Liver Metastasis.

BACKGROUND: Surgical management strategies for patients with synchronous liver metastases from gastroenteropancreatic neuroendocrine tumors (GEP-NETs) are controversial. This study sought to investigate survival outcomes associated with various resection strategies. METHODS: Patients with grade I or II primary NETs originating from the small-bowel (SBNETs) or pancreas (PNETs) with synchronous liver metastasis alone were identified from the Surveillance, Epidemiology, and End Results (SEER) database (2010-2019). Data were analyzed using chi-square testing, the Kaplan-Meier method with the log-

Background Liver Fibrosis as a Microenvironmental Risk Factor for Intrahepatic Recurrence in Colorectal Liver Metastases.

BACKGROUND: Colorectal cancer liver metastases (CRLMs) are a frequent pattern of recurrence, yet the prognostic impact of background liver fibrosis remains unclear. This study aimed to quantitatively assess hepatic fibrosis histologically and determine its association with intrahepatic recurrence after resection. METHODS: Patients who underwent hepatic resection for CRLM between 1999 and 2022 were retrospectively included in this study. Background liver fibrosis was evaluated histologically and quantified via whole-slide imaging. Fibrosis was analyzed as a continuous variable using Cox propor

Textbook outcomes for robotic liver resection: assessing relevant clinical variables.

BACKGROUND: Textbook outcome (TO) is a multidimensional metric for assessing surgical success, integrating several postoperative variables into a single composite endpoint. While TO definitions have been validated for open and laparoscopic liver resections (LR), no consensus exist for the robotic approach. This retrospective study assesses TO and extended-TO (TOLS+) in patients undergoing robotic LR, and explores perioperative factors associated with TO-achievement. METHODS: We analyzed 622 consecutive robotic LR for benign and malignant disease between 2013 and 2024. TO and TOLS+ were define

Clinical impact of indocyanine green fluorescence imaging on perioperative outcomes in laparoscopic anatomical liver segmentectomy.

BACKGROUND: Laparoscopic anatomical segmentectomy is technically demanding due to the lack of visible surface landmarks identifying segmental borders. While indocyanine green (ICG) fluorescence imaging offers real-time navigation, its specific impact on anatomical orientation remains understudied. We evaluated the clinical impact of ICG fluorescence guidance on perioperative efficiency and outcomes in laparoscopic anatomical segmentectomy. METHODS: We analyzed 78 consecutive patients (2017-2025) undergoing laparoscopic anatomical segmentectomy, stratified into ICG-guided (n = 51, negative sta

Robotic vs. open liver resection after neoadjuvant therapy for hepatocellular carcinoma: a propensity score-matched analysis of short- and long-term outcomes.

BACKGROUND: Neoadjuvant therapy is recommended for hepatocellular carcinoma at advanced stages. It is unknown whether robotic liver resection (RLR) is superior to open liver resection (OLR) after neoadjuvant therapy. METHODS: We analyzed consecutive RLR and OLR patients from December 2020 to December 2023. Patient variables, short- and long-term outcomes were compared. A respective 1:3 propensity score-matched (PSM) analysis was performed between RLR and OLR groups. RESULTS: The analysis included 32 RLR and 386 OLR cases. After PSM, 29 RLR cases matched to 78 OLR cases. RLR had similar opera

Predictors, timing, and outcomes of Clostridium difficile infection following liver and pancreatic resection.

BACKGROUND: Clostridium difficile infection (CDI) is a common healthcare-associated infection. We assessed the incidence, timing, risk factors, and outcomes of CDI following liver and pancreatic resection. METHODS: Adults undergoing elective hepatic or pancreatic resection were identified in ACS-NSQIP (2015-2021). The primary exposure was 30-day postoperative CDI. Outcomes included 30-day mortality, non-home discharge, unplanned readmission, and length of stay (LOS). Multivariable regression analyses were conducted. RESULTS: Among 76,083 patients (liver: 30,145 [39.6%], pancreas: 45,938 [60.

Does Surgical Approach Influence Benchmark Attainment in Intrahepatic Cholangiocarcinoma?

BACKGROUND: Intrahepatic cholangiocarcinoma (ICC) is a rare malignancy with rising incidence and poor prognosis. Surgical resection remains the only curative option, with outcomes driven by R0 margins and lymphadenectomy (LND). While open liver resection (OLR) is standard, laparoscopic (LLR) and robotic liver resection (RLR) are increasingly used, with concerns about their oncologic adequacy. OBJECTIVE: To assess the association between composite surgical benchmark attainment (R0 + LND) and short- and long-term outcomes after ICC resection, stratified per surgical approach. METHODS: A retros

The association between antiviral therapy and 10-year overall survival of patients undergoing liver resection for hepatitis C virus- or hepatitis B virus-related hepatocellular carcinoma.

The efficacy of antiviral therapy against hepatitis C virus (HCV) and hepatitis B virus (HBV), following liver resection (LR) for hepatocellular carcinoma (HCC), has been studied extensively. However, its long-term overall survival (OS) benefit remains to be conclusively demonstrated. This retrospective cohort study included 1121 patients (777 with HBV-related HCC and 344 with HCV-related HCC) who underwent LR. The primary outcome was 10-year OS. The indication for antiviral therapy was serum HBV DNA > 2000 IU/ml in patients with HBV-related HCC and detectable serum HCV RNA in patients with HC

Perioperative infections as a prognostic risk factor in hepatocellular carcinoma and cholangiocellular carcinoma: a comparative analysis.

BACKGROUND: Hepatocellular Carcinoma (HCC) and cholangiocellular adenocarcinoma (CCA) are the most common primary liver tumors representing a major global health burden. In early disease stages, tumor resection may provide long-term survival in selected patients. However, morbidity and mortality rates are still relatively high after extended liver surgery with perioperative bacterial infections representing major complications. In this study, we evaluate the impact of perioperative infection on the postoperative overall survival (OS) of patients undergoing resection of HCC or CCA. MATERIAL AN

Reduced Floxuridine Dose Limits Hepatobiliary Toxicity Without Negatively Impacting Survival After Resection of Colorectal Cancer Liver Metastases.

BACKGROUND: Hepatic arterial infusion (HAI) of floxuridine is an effective adjuvant therapy for colorectal liver metastases (CRLM) following complete resection, but its use is limited by hepatobiliary toxicity. Dosing characteristics, factors associated with dose reductions, and the impact of starting dose on hepatobiliary toxicity and survival in the adjuvant setting are poorly described. METHODS: From 2015 to 2024, patients who received adjuvant HAI floxuridine after complete CRLM resection at City of Hope were included. Hepatobiliary toxicity and oncologic outcomes of standard (0.12 mg/kg/

LIVERATION trial: a multicentre European randomised study on radiofrequency margin coagulation and its impact on oncological outcomes after liver surgery - study protocol.

INTRODUCTION: Surgical margins are crucial in determining postoperative local recurrence (LR) in patients with colorectal liver metastasis (CRLM) and hepatocellular carcinoma (HCC). Achieving a margin greater than 1 cm can be challenging due to constraints related to remnant liver reserve, proximity to major vascular structures and tumour depth. We previously published findings from a retrospective study suggesting that additional margin coagulation (AMC) using radiofrequency may reduce LR, and this multicentre randomised clinical trial aims to further assess this hypothesis. Neoadjuvant therapy for cholangiocarcinoma: A comprehensive literature review.

Biliary tract cancers (BTCs) comprise a heterogenous group of aggressive and rare malignancies arising in the bile duct outside or within the liver. BTCs include cholangiocarcinoma (CCA), gallbladder cancer (GBC) and ampulla of Vater cancer (AVC); according to the "historical" anatomical classification, CCAs are further subdivided into extrahepatic cholangiocarcinomas (eCCAs) - including distal (dCCA) and perihilar (pCCA) - and intrahepatic cholangiocarcinomas (iCCA). Notably enough, these subtypes reflect distinct features in terms of biology, epidemiology, prognosis and therapeutic strategie

An Uncommon Focal Liver Lesion: Intrahepatic Splenosis.

Multiple focal liver lesions were incidentally detected in a patient screened by ultrasound for a recent diagnosis of lower limb deep vein thrombosis, for which anticoagulation had been initiated. Past medical history reported a post-traumatic splenectomy 15 years before. Magnetic resonance imaging (MRI) and contrast-enhanced ultrasound (CEUS) showed a subcapsular lesion in liver segment 5 consistent with focal nodular hyperplasia (FNH) and multiple other nodules, with a different pattern from the former, judged as probable hepatic adenomas by MRI but probable hemangiomas by CEUS (hyperenhance
