[The effect of calcium on the release of gastric inhibitory polypeptide (GIP) - with reference to the release of GIP in patients with hyperparathyroidism (author's transl)].

Plasma gastric inhibitory polypeptide (GIP), insulin, glucagon concentrations and blood glucose levels in response to the ingestion of 100 g glucose were measured in 5 patients with hyperparathyroidism in order to elucidate the effect of hypercalcemia on the release of these hormones. In addition, the effect of acute hypercalcemia on the release of these hormones in response to glucose ingestion was investigated in normal subjects. Fasting plasma GIP concentration in patients with hyperparathyroidism was significantly greater than the value in seventeen normal subjects. Significantly higher re…

The effects in the rat of varying intakes of dietary calcium, phosphorus, and hydrogen ion on hyperparathyroidism due to chronic renal failure.

Renal failure of 4 wk duration in rats led to parathyroid enlargement, increased bone resorption, and decreased tubular reabsorption of phosphate by the remnant kidney. The degree of hyperparathyroidism was influenced by each of the three dietary factors investigated. In the first study increasing calcium intake reduced the size of the parathyroids by increasing calcium and reducing phosphate absorption. In the second study phosphate intake was linearly related to parathyroid gland size in the uremic animals and associated with rising plasma phosphate levels. In the last study acidosis led dir…

Value of plasma chloride concentration and acid-base status in the differential diagnosis of hyperparathyroidism from other causes of hypercalcaemia.

A study is reported of the estimation of plasma chloride concentration and acid-base status in the differentiation of primary hyperparathyroidism from all other causes of hypercalcaemia. In the two groups of patients studied, all of whom had hypercalcaemia, there was complete separation between the two groups on the basis of plasma chloride concentration and acid-base status. In 16 patients with primary hyperparathyroidism the increase in plasma chloride concentration and associated metabolic acidosis could have been accounted for by the known renal tubular effects of parathyroid hormone. In 1…

Progression of Secondary to Tertiary Hyperparathyroidism with a Coexisting Parathyroid Adenoma in Stage 3b Chronic Kidney Disease: A Case Report.

Tertiary hyperparathyroidism arises when prolonged secondary hyperparathyroidism progresses to autonomous parathyroid hormone secretion with persistent hypercalcemia, most often in chronic kidney disease. Diffuse or nodular multiglandular hyperplasia is the expected pathology; a discrete parathyroid adenoma arising within hyperplastic tissue is uncommon and may be overlooked when imaging is negative early in the disease course. A 60-year-old man with stage 3b chronic kidney disease and long-standing secondary hyperparathyroidism presented with recurrent hypercalcemia and severe vomiting. Labor…

Tertiary hyperparathyroidism with ectopic mediastinal parathyroid adenoma in a patient with chronic kidney disease and cardiovascular complications: a case report.

BACKGROUND: Hyperparathyroidism includes conditions marked by excessive secretion of parathyroid hormone (PTH), resulting in disruptions in calcium-phosphate metabolism. In individuals with chronic kidney disease (CKD), differentiating secondary hyperparathyroidism from tertiary hyperparathyroidism is frequently challenging due to the overlapping biochemical and clinical characteristics. Ectopic mediastinal parathyroid adenomas are uncommon and complicate localisation and therapy further. CASE PRESENTATION: We present a 66-year-old female with chronic kidney disease, hypertension, and ischaem…

Case Report: Multiple skeletal lesions as the initial presentation of primary hyperparathyroidism: A case of parathyroid adenoma mimicking malignancy.

BACKGROUND: Primary hyperparathyroidism (PHPT) is a relatively common endocrine disorder, although advanced skeletal manifestations mimicking metastatic malignancy are uncommon, characterized by autonomous oversecretion of parathyroid hormone. CASE PRESENTATION: We report the case of a 67-year-old female who presented with cough and chest pain and was later found to have extensive osteolytic lesions initially suspected as malignant bone metastases. A multidisciplinary evaluation, including laboratory testing and advanced imaging, was performed. The evaluation revealed primary hyperparathyroid…

A simple preoperative nomogram for predicting high-risk parathyroid lesions: a tool to guide surgical strategy in primary hyperparathyroidism.

BACKGROUND: Distinguishing benign from high-risk parathyroid lesions [atypical parathyroid tumor (APT) and parathyroid carcinoma (PC)] preoperatively is crucial for surgical strategy, yet remains difficult in tertiary referral centers confronting an enriched risk profile. This study sought to develop and validate a practical nomogram for individualized risk quantification in this specific clinical context. METHODS: We conducted a single-center, retrospective cohort study of patients with biochemically active solitary parathyroid lesions who underwent surgery between January 2009 and May 2024

Clinical and Biochemical Correlates of Parathyroid Gland Burden in Patients Undergoing Parathyroidectomy for Secondary Hyperparathyroidism: A Retrospective Observational Study.

Background/Objectives: Secondary hyperparathyroidism (SHPT) is a common and clinically significant complication of advanced chronic kidney disease and may require surgical intervention when medical therapy fails. This study aimed to evaluate the association between parathyroid gland burden, defined by gland number and size parameters, and biochemical disease severity in patients undergoing parathyroidectomy for SHPT, and to assess the concordance between preoperative imaging findings and intraoperative observations. Although parathyroid gland enlargement is a hallmark of SHPT, the clinical rel…

Combination therapy with nutritional vitamin D and calcimimetics in secondary hyperparathyroidism: a mechanism-based approach to restoring PTH regulation.

Secondary hyperparathyroidism (SHPT) in chronic kidney disease (CKD) is commonly interpreted as a compensatory response to disturbances in mineral metabolism, yet this view does not fully account for the biochemical instability and treatment resistance observed in advanced disease. Current therapeutic strategies primarily target individual regulatory pathways. Calcimimetics suppress parathyroid hormone (PTH) secretion through calcium-sensing receptor (CaSR) activation, whereas active vitamin D analogs reduce PTH synthesis via vitamin D receptor (VDR) signaling but are frequently associated wit…

Pharmacological Management of Secondary Hyperparathyroidism in Dialysis Patients: A Narrative Review of Calcimimetics, Vitamin D Analogs, and Phosphate Binders.

Secondary hyperparathyroidism (sHPT) is a prevalent complication of end-stage renal disease, driven by phosphate retention, vitamin D deficiency, hypocalcemia, and calcium-sensing receptor dysregulation. Despite international guidelines, optimal biochemical control remains challenging in real-world practice. This narrative review synthesizes current evidence on the pharmacological management of sHPT in dialysis patients, with attention to the underrepresented Middle Eastern and Gulf Cooperation Council (GCC) context. A narrative review was conducted using PubMed and Scopus, identifying origina…

Plasma Parathyroid Hormone Concentration as a Predictor of Post-Operative Hypocalcemia in Dogs Diagnosed With Primary Hyperparathyroidism and Treated With Parathyroidectomy.

BACKGROUND: Hypocalcemia is a relatively common complication after parathyroidectomy for treatment of primary hyperparathyroidism. OBJECTIVES: To retrospectively evaluate clinical variables in dogs with primary hyperparathyroidism to determine whether or not an association exists between pre-surgical variables and the development of post-surgical hypocalcemia. ANIMALS: One hundred three dogs diagnosed with primary hyperparathyroidism and treated by parathyroidectomy in seven referral hospitals between 2010 and 2021. MATERIAL AND METHODS: Data collected from medical records included signalme…

Persistent Hyperparathyroidism in Vitamin D-Dependent Rickets Type 2A Does Not Prevent Normalization of Hypophosphatemia or Healing of the Rickets.

INTRODUCTION: Vitamin D-dependent rickets type 2A (VDDR2A) is a rare, autosomal recessive disorder caused by pathogenic variants of the VDR gene encoding the vitamin D receptor. It has been proposed to be a form of parathyroid hormone (PTH)-dependent rickets. Here, we describe in detail a girl with VDDR2A who developed a long-standing, tertiary hyperparathyroidism that did not prevent healing of the rickets nor normalization of hypophosphatemia. CASE PRESENTATION: A girl who presented with seizures at 9 months of age was diagnosed with VDDR2A. She had poor growth, alopecia, severe hypocalcemi…

Evaluation of Radiofrequency Ablation Safety and Efficacy in Primary Hyperparathyroidism: A Single-Center Retrospective Study in Taiwan and Literature Review.

Radiofrequency ablation (RFA) is increasingly recognized as a minimally invasive option for primary hyperparathyroidism (PHPT). This study aimed to evaluate the effectiveness and safety of ultrasound (US)-guided RFA for the treatment of PHPT in Taiwan. We conducted a retrospective study of patients with PHPT who underwent RFA at a single medical center between March 2020 and January 2023. Serum biochemical samples, changes in parathyroid nodule volume, symptomatic scores, and complications were analyzed at follow-up after RFA. The volume reduction ratio (VRR) and treatment response at 1 year w…

A Pregnant Woman with Primary Hyperparathyroidism Who Underwent Parathyroidectomy in the Second Trimester of Her Third Pregnancy: A Case Report.

Primary hyperparathyroidism (PHPT) during pregnancy is rare, causing severe pregnancy complications. We report a pregnant woman with PHPT and a pregnancy complication history who underwent a single-gland parathyroidectomy during the second trimester and delivered at term. A 28-year-old pregnant woman, gravida 3, para 2, was referred to our hospital at 9 gestational weeks (GWs) because of previous intrauterine fetal death, preterm birth, and fetal growth restriction. She was diagnosed with PHPT during her second pregnancy, and her baby experienced neonatal hypocalcemia. However, she received no…

"Metabolic Superscan" Pattern in a Case of Secondary Hyperparathyroidism With Parathyroid Hyperplasia Detected on 68 Ga-Trivehexin PET/CT.

Parathyroid hyperplasia frequently causes secondary hyperparathyroidism (SHPT) in chronic kidney disease (CKD), necessitating bilateral neck exploration. Although preoperative imaging is limited, it is essential for reducing recurrent or persistent disease. This case highlights the potential of 68 Ga-trivehexin PET/CT in SHPT, demonstrating better identification of hyperfunctioning parathyroid glands, potentially assisting surgical planning. In addition, diffuse skeletal tracer uptake revealed a "metabolic superscan" pattern consistent with renal osteodystrophy and rapid bone-turnover. 68 Ga-t…

A prospective cross-sectional study on hypocalcemia after total thyroidectomy in patients with Graves' disease: insights on secondary hyperparathyroidism.

PURPOSE: To investigate the parathyroid function and calcium (Ca) levels in the secondary hyperparathyroidism (SHPT) state in patients with Graves' disease. METHODS: We examined 31 consecutive patients with Graves' disease without chronic kidney disease, who were treated with total thyroidectomy. The patients were divided into a normal parathyroid hormone (PTH) group (NPTH group; n = 19) with a PTH level ≤ 65 pg/mL, and a secondary hyperparathyroidism group (SHPT group; n = 12), with a PTH level > 65 pg/mL. The PTH and Ca-related parameters were examined and the risk factors for postoperative…
