Role of female reproductive hormones and genetics in temporomandibular joint disorders.

The disorders of temporomandibular joint manifest clinically with disruptions in its movement and facial pain. Women exhibit a three-fold higher propensity for developing temporomandibular joint disorders compared to men. There are several studies describing the effects of female reproductive hormones on temporomandibular joint structures and pain perception, shedding light on the genetic influence underlying these conditions. Several polymorphisms have been studied and documented in the literature, shedding light on the genetic background of temporomandibular joint disorders. This review aims

Joint surgery rates in lupus: a long-term cohort study.

AIM: With scarce data on the need and type of joint surgery in SLE, we investigated the long-term rates and underlying causes for arthroplasty, arthrodesis and synovectomy in patients with SLE. METHODS: Procedure dates for arthroplasty, arthrodesis or synovectomy were retrieved from the state-wide Hospital Morbidity Data Collection between 1985 and 2015 for patients with SLE (n=1855) and propensity-matched controls (n=12 840). Patients with SLE with ≥two additional diagnostic codes for rheumatoid arthritis were classified as rhupus. ORs and incidence rates (IRs) per 100 person-years for joint

The Contribution of the Distal Oblique Band to Distal Radioulnar Joint Stability.

PURPOSE: The distal radioulnar joint (DRUJ) is supported by an array of dynamic and static stabilizers, of which the triangular fibrocartilage complex (TFCC) is the most important, and the distal interosseous ligament is next in importance. The distal oblique band (DOB) is an identifiable component of the distal interosseous ligament, found in a subset of the population. Our objective was to determine the contribution of the DOB to DRUJ stability in the presence of a disrupted TFCC. METHODS: Twenty-three above-elbow specimens were prepared by removing the TFCC and the DRUJ joint capsule, pres

Bilateral Subluxation of the Proximal Tibiofibular Joint: A Rare Case Report.

INTRODUCTION: Proximal tibiofibular joint (PTFJ) subluxations are rare and can occur following trauma or sporting events. Idiopathic PTFJ subluxations are more common in females and in patients with generalized ligament laxity. The proper history and clinical examination together with imaging can help in establishing the diagnosis in the majority of cases. CASE REPORT: We report a rare case of bilateral subluxation of PTFJ and describe the detailed clinical features and magnetic resonance imaging findings. The patient was managed conservatively with good results. CONCLUSION: Bilateral sublux

Implantation of S1AIS has priority as a sacroiliac joint fixation technique.

PURPOSE: The sacral alar-iliac screw (SAIS) fixation technique has evolved from spinopelvic fixation which originated from S2AIS to sacroiliac joint fixation, with more reports regarding its application of S2AIS than S1AIS. However, there is a lack of comparative evidence to determine which technique is superior for sacroiliac joint fixation. This study aimed to determine which of the screws was superior in terms of implantation safety and biomechanical stability for sacroiliac joint fixation. METHODS: CT data of 80 normal pelvises were analyzed to measure the insertable range, trajectory len

[Arthrodesis of the Distal Interphalangeal Joint of the Finger Using an Intramedullary Implant].

PURPOSE OF THE STUDY: Osteoarthritis (OA) of the joints of the hand is one of the most common musculoskeletal disorders in the elderly population. It is a complex, degenerative disease affecting all components of the joint. Surgical treatment is indicated when conservative therapy fails. The ultimate solution is arthrodesis of the distal interphalangeal (DIP) joint. Various methods of arthrodesis have been described, ranging from the use of K-wires or compression screw to different types of intra-articular (intramedullary) implants. The aim of this study is to evaluate the surgical outcomes of…

Predictive parameters for spontaneous joint bleeding during emicizumab prophylaxis.

Emicizumab is approved for prophylaxis of patients with hemophilia A (HA). Despite its efficacy in reducing bleeding, some patients on emicizumab still experience hemarthrosis, but no tool is yet available to identify those at a higher risk of spontaneous joint bleeding. This study aimed to evaluate whether laboratory measurements (global coagulation assays and emicizumab concentration) and/or arthropathy scores can distinguish patients at higher risk of spontaneous joint bleeding while on emicizumab prophylaxis. A thrombin generation assay was performed upon the addition of tissue factor and

Structural alterations and pain perception at the ankle joint in patients with haemophilia.

BACKGROUND: Patients with haemophilia (PwH) suffer from chronic pain due to joint alterations induced by recurring haemorrhage. OBJECTIVES: This study aimed to investigate the relationship between structural alterations and pain perception at the ankle joint in PwH. PATIENTS/METHODS: Ankle joints of 79 PwH and 57 healthy controls (Con) underwent ultrasound examination (US) and assessment of pain sensitivity via pressure pain thresholds (PPT). US discriminated between joint activity (synovitis) and joint damage (cartilage and/or bone degeneration) applying the HEAD-US protocol. Based on US-fi…

Patellar osteochondritis dissecans: maturation-dependent patellofemoral joint characteristics.

BACKGROUND: The likelihood of healing of osteochondritis dissecans decreases with skeletal maturity and there are theories that abnormal biomechanical forces contribute to the development and progression of these lesions. OBJECTIVE: To characterize, according to regional skeletal maturity, the morphology and alignment indices of the patellofemoral joint on MRI in patients with patellar osteochondritis dissecans. MATERIALS AND METHODS: MRI examinations of patients with patellar osteochondritis dissecans obtained between January 2008 and May 2023 were retrospectively reviewed to determine regi

Evaluation of temporomandibular joint involvement in juvenile idiopathic arthritis patients.

OBJECTIVE: Juvenile idiopathic arthritis (JIA) is a common, chronic and inflammatory rheumatological disease of childhood. The disease can affect all synovial joints in the body. Temporomandibular joints (TMJs) are important areas of involvement in JIA, which are frequently involved but often not noticed because the involvement is usually asymptomatic. The aim of this study is to determine the frequency and risk factors of TMJ joint involvement in juvenile idiopathic arthritis patients admitted to our clinic, and to guide for early diagnosis and treatment. METHODS: Patients who applied to thi…

Non-surgical interventions for treating osteoarthritis of the big toe joint.

BACKGROUND: Osteoarthritis (OA) affecting the first metatarsophalangeal joint (hallux rigidus) is common and painful. Several non-surgical treatments have been proposed; however, few have been adequately evaluated. Since the original 2010 review, several studies have been published necessitating this update. OBJECTIVES: To determine the benefits and harms of non-surgical treatments for big toe OA. SEARCH METHODS: We used standard, extensive Cochrane search methods. The latest search was February 2023. SELECTION CRITERIA: We included randomised trials that compared any type of non-surgical t

Is It Necessary to Repair the Plantar Plate in the Management of Metatarsophalangeal Joint Instability?

Lesser metatarsophalangeal joint (MTPJ) instability is a common cause of metatarsalgia and is associated with plantar plate pathology. The instability is often due to chronic attenuation and attritional changes of the plantar plate and collateral ligaments, leading to eventual rupture. The plantar plate and collateral ligaments serve as the primary stabilizers of the MTPJ, and rupture or attenuation of these ligaments leads to instability. Surgical intervention is often necessary for plantar plate tears, with various approaches and techniques available.

Treatment of temporomandibular joint internal derangement using MESNA injection.

Источник пометил публикацию как отозванную

INTRODUCTION: The development of temporomandibular disorders specifically emphasizes the biochemical changes occurring in the synovial fluid at different stages of temporomandibular joint disease. Research has indicated that inflammation may be a primary reason behind the pain and dysfunction in temporomandibular joint diseases. Since its clearance several years ago, MESNA (sodium 2-mercaptoethanesulfonate) has been used in various formulations as a mucolytic drug in the respiratory domain. It operates by disrupting the disulfide bonds present between polypeptide chains within mucus. MESNA exh

A Bayesian joint model for mediation analysis with matrix-valued mediators.

Unscheduled treatment interruptions may lead to reduced quality of care in radiation therapy (RT). Identifying the RT prescription dose effects on the outcome of treatment interruptions, mediated through doses distributed into different organs at risk (OARs), can inform future treatment planning. The radiation exposure to OARs can be summarized by a matrix of dose-volume histograms (DVH) for each patient. Although various methods for high-dimensional mediation analysis have been proposed recently, few studies investigated how matrix-valued data can be treated as mediators. In this paper, we pr

Delayed Diagnosis of Temporomandibular Joint Dislocation in Severe Stroke Patients.

A 79-year-old woman with a history of left cerebral infarction developed altered consciousness and left hemiplegia. CT of the head revealed a putaminal hemorrhage. She underwent tracheal intubation followed by a tracheostomy for long-term airway management. Despite improved consciousness, the patient continued to experience dysphagia and was fed via a nasal tube. Subsequent axial CT and 3D CT scans revealed an empty glenoid fossa in both temporomandibular joints (TMJs) with the condyles positioned anteriorly, consistent with chronic bilateral anterior TMJ dislocation. After an unsuccessful att…

Ossicular joint histopathology in cases of age-related hearing loss.

OBJECTIVES: Age-related hearing loss (presbycusis) is a prevalent condition traditionally attributed to inner ear dysfunction. Little is known about age-related changes in the ossicular joints or their contribution to presbycusis. Herein, we performed an otopathologic evaluation of the ossicular joints in cases of presbycusis without a clear sensorineural explanation. METHODS: Histopathologic analysis of the incudomallear (IM) and incudostapedial (IS) joints was performed in specimens from the National Temporal Bone Registry with audiometrically confirmed presbycusis but without histologicall…
