Anterior cruciate ligament reconstruction using autologous hamstring double bundle graft compared with single bundle procedures.

A total of 218 patients with unilateral anterior cruciate ligament deficiency were randomly assigned to one of four groups. In group A an anatomical double bundle anterior cruciate ligament reconstruction was performed; group B were treated by a single bundle using an Endobutton for femoral fixation; in group C by a single bundle using RigidFix cross pins for femoral fixation; and in group D by a single bundle using a bioabsorbable TransFix II screw for femoral fixation. For tibial fixation a bioabsorbable Intrafix interference screw was used for all the groups and the graft was fashioned from

Evaluating the Etiology of Osteochondritis Dissecans of the Knee: The Role of the Articular-Epiphyseal Cartilage Complex.

Introduction: In pathologic form, osteochondritis dissecans (OCD) is a focal end-stage disease of articular cartilage. Comparative anatomy studies have demonstrated that injury to epiphyseal anlage cartilage leads to development of OCD in many species, whereas in humans, the exact etiology is suspected but remains unknown. A potential candidate is the articular-epiphyseal cartilage complex, and our hypothesis is that injury at the junction between the epiphyseal anlage cartilage and the articular cartilage results in childhood OCD. Methods: A retrospective, two-institution assessment of childr

Preoperative MRI abnormalities are not associated with poor outcomes or altered surgical planning in unicompartmental knee arthroplasty: a systematic review and meta-analysis.

PURPOSE: To evaluate the association between preoperative MRI imaging and clinical outcomes following Unicompartmental knee arthroplasty (UKA). METHODS: We searched PubMed, Scopus, Web of Science, and Embase for studies related to preoperative MRI abnormalities and clinical outcomes in patients undergoing UKA. The quality assessment was done by the National Institutes of Health's (NIH) Study Quality Assessment Tools. A random-effects model with Hedges'g (standardized mean difference, SMD) and 95% confidence intervals (CIs) was used to assess the impact of meniscal injury and bone marrow lesio

Area of Skin Numbness After Total Knee Arthroplasty: Does Minimally Invasive Approach Make Any Difference From Standard Approach?

BACKGROUND: Skin numbness after total knee arthroplasty (TKA) was reported in relation to injury of the infrapatellar branch of saphenous nerve (IPBSN). METHODS: Phase I study: A nonrandomized and nonconsecutive selection of 30 patients undergoing unilateral TKA, using minimally invasive surgery approach, and 30 patients having standard approach were included. Area of skin numbness was periodically evaluated and compared until 1-year follow-up (FU). Phase II study: we dissected 15 normal cadaveric knees and followed the course and distribution of IPBSN. RESULTS: Twenty-nine predominantly fem

Evaluation of an intact, an ACL-deficient, and a reconstructed human knee joint finite element model.

The human knee joint has a three-dimensional geometry with multiple body articulations that produce complex mechanical responses under loads that occur in everyday life and sports activities. Understanding the complex mechanical interactions of these load-bearing structures is of use when the treatment of relevant diseases is evaluated and assisting devices are designed. The anterior cruciate ligament (ACL) in the knee is one of four main ligaments that connects the femur to the tibia and is often torn during sudden twisting motions, resulting in knee instability. The objective of this work is

Clinical and radiological outcome of medial patellofemoral ligament reconstruction with a semitendinosus autograft for patella instability.

BACKGROUND: Recurrent patellar instability is a common problem after dislocation. The medial patellofemoral ligament (MPFL) contributes 40-80% of the total medial restraining forces. This study assessed the clinical and radiological outcome after a follow-up of 4 years after linear MPFL reconstruction using an ipsilateral Semitendinosus tendon autograft. STUDY DESIGN AND METHODS: 15 knees in 12 patients were examined with a mean of 47 months after linear reconstruction of the MPFL at a mean age of 30 years. 3 knees underwent previous surgery. 3 patients had mild trochlear dysplasia grade I or

Physiotherapeutic Approach for Septic Arthritis of Knee Joint.

Septic arthritis is an orthopaedic emergency associated with poor prognosis in cases with delayed treatment. The standard routes through which the infection spreads are hematogenous and direct entry. Any delay in medicine could mean the patient facing severe joint destruction, limitation in joint range, and inability to do activities of daily living. Septic arthritis is treated with a multidisciplinary approach in which physiotherapy is essential in making patients functionally independent. This article discusses a 58-year-old male patient with pain and swelling in the right knee joint and dif

Arthroscopic Resection of Symptomatic Tibial Tubercle Ossicles for Recalcitrant Osgood-Schlatter Disease Using a 2-Portal Technique.

Arthroscopic resection of symptomatic unfused tibial tubercle ossicles causing chronic anterior knee pain due to recalcitrant Osgood-Schlatter disease has been shown to be a reproducible arthroscopic technique. Although a number of other surgical techniques have been described, including ossicle excision and tubercle-plasty, drilling of the tibial tubercle, bone peg insertion to induce fusion, open excision of loose fragments, direct bursoscopic ossicle resection, and closing-wedge tubercle osteotomy, with varied outcomes, this technique offers a minimally invasive approach with low risk. Experimental haemophilic arthropathy in a mouse model of a massive haemarthrosis: gross, radiological and histological changes.

Recurrent haemarthrosis results in chronic synovitis and destructive arthropathy. The long-term effect of a single haemorrhage is not known. To investigate the histopathological changes following a single, but major joint haemorrhage, an animal model of massive haemarthrosis without mechanical trauma was developed and is described in this manuscript. The knee joint capsule of mice deficient in coagulation factor VIII or IX and non-haemophilic wild type mice was punctured to induce a one time, but massive haemorrhage. The single joint puncture resulted in acute haemarthrosis in both types of ha

Osteochondritis dissecans of the trochlea of the femur.

This article defines the characteristic features of osteochondritis dissecans of the trochlea of the femur, and indicates that important differences distinguish it from the more familiar femoral condylar form. The clinical features in 16 knees included: gradual onset of symptoms, pain with running and jumping, no significant history of injury; inconstant tenderness of the trochlea, and pain with resisted extension at 20 to 45 degrees. Diagnosis was usually difficult, and was often delayed because of subtle radiographic changes. Treatment depended on the stage of presentation. Nonsurgical treat

Comparison of Blood Flow Restriction Interventions to Standard Rehabilitation After an Anterior Cruciate Ligament Injury: A Systematic Review.

BACKGROUND: Blood flow restriction training (BFR-t) data are heterogeneous. It is unclear whether rehabilitation with BFR-t after an anterior cruciate ligament (ACL) injury is more effective in improving muscle strength and muscle size than standard rehabilitation. PURPOSE: To review outcomes after an ACL injury and subsequent reconstruction in studies comparing rehabilitation with and without BFR-t. STUDY DESIGN: Systematic review. Level of evidence, 3. METHODS: A search of English-language human clinical studies published in the past 20 years (2002-2022) was carried out in 5 health scienc

Acute reconstruction results in less sick-leave days and as such fewer indirect costs to the individual and society compared to delayed reconstruction for ACL injuries.

PURPOSE: To compare the total number of sick-leave days caused by the knee injury from the day of injury and over the first year between acute (within 8 days) and delayed (6-10 weeks) anterior cruciate ligament reconstruction (ACLR) and also assess other clinical outcomes during this period. METHODS: Seventy patients with an acute ACL injury and Tegner level of 6 or more were randomized to acute (within 8 days) or delayed (after 6-10 weeks) ACLR. Patient-reported outcomes; objective IKDC and manual stability measurements were assessed at 6 and 12 months. With data from the Swedish Social Insu

Racial Disparities in Limb Amputations After Traumatic Vascular Injury.

OBJECTIVES: The influence of race or ethnicity on limb loss after traumatic vascular injury is unclear. We sought to determine whether there were racial differences in rates of amputation between American Indians, blacks, Asians, and Hispanics compared to white patients following arterial axillosubclavian vessel injury (ASVI), femoral artery injury (FAI), or popliteal artery injury (PAI). As black race has been identified as an independent prognostic factor for postsurgical complication in trauma-associated lower extremity amputation, we further hypothesized that black race would be associated

Outcome at 12 to 22 years of 1502 tibial shaft fractures.

Fractures of the tibial shaft are common injuries, but there are no long-term outcome data in the era of increased surgical management. The aim of this prospective study was to assess the clinical and functional outcome of this injury at 12 to 22 years. Secondary aims were to determine the short- and long-term mortality, and if there were any predictors of clinical or functional outcome or mortality. From a prospective trauma database of 1502 tibial shaft fractures in 1474 consecutive adult patients, we identified a cohort of 1431 tibial diaphyseal fractures in 1403 patients, who fitted our in

Teriparatide as a chondroregenerative therapy for injury-induced osteoarthritis.

There is no disease-modifying therapy for osteoarthritis, a degenerative joint disease that is projected to afflict more than 67 million individuals in the United States alone by 2030. Because disease pathogenesis is associated with inappropriate articular chondrocyte maturation resembling that seen during normal endochondral ossification, pathways that govern the maturation of articular chondrocytes are candidate therapeutic targets. It is well established that parathyroid hormone (PTH) acting via the type 1 PTH receptor induces matrix synthesis and suppresses maturation of chondrocytes. We r
