[Risks associated with childbearing postponement: the relationship between increased mean maternal age and spontaneous abortions in Italy.].

An increased rate of spontaneous abortions has been observed in Italy in recent years, despite a lower fertility rate in the country. In view of the well known and proven link between increasing maternal age and risk of spontaneous abortion, this study examines the relation between increased rate of abortion and progressive postponement of childbearing in Italy. Data was obtained from the Italian National Institute of Statistics (ISTAT) and from the Health For All databases. Findings show that the mean maternal age at delivery has progressively increased in the last two decades, in parallel wi

The influence of mastitis on antibody transfer to infants through breast milk.

Clinical and subclinical mastitis are common among lactating women but their potential influence on programmes to improve infant health through maternal vaccination has not previously been addressed. Maternal antibodies, acquired either through natural infection or through vaccination, enter milk through two main pathways: by transcytosis or paracellularly. Transfer through both pathways is likely to be increased during mastitis, mediated by inflammatory cytokines. The increases are of similar order of magnitude to the variation in milk antibody concentrations between women within a population

How family functioning shapes prenatal bonding: a mediation analysis of depressive symptoms and maternal-fetal attachment.

OBJECTIVE: Pregnancy is a period in a woman's life characterized by profound hormonal, psychological, and social changes, which may lead to increased emotional vulnerability, during which depressive symptoms may negatively affect maternal well-being and maternal-fetal attachment. Although family functioning has traditionally been examined mainly in relation to psychopathology, its protective role during pregnancy remains insufficiently explored. This study examined whether depressive symptoms mediate the association between family functioning and antenatal maternal attachment. METHODS: The sa

Comparative maternal-fetal outcomes associated with different antihypertensive treatment strategies in preeclampsia: a retrospective cohort study.

BACKGROUND: Preeclampsia seriously threatens maternal and infant health. Antihypertensive therapy is key to improving perinatal outcomes, yet comparative maternal-fetal evidence for oral agents remains limited. OBJECTIVE: To compare maternal-fetal outcomes of oral labetalol versus oral nifedipine in preeclampsia, informing individualized clinical treatment. METHODS: This retrospective cohort study included consecutive preeclampsia patients admitted from January 2023 to December 2025, divided into Labetalol (n = 154) and nifedipine (n = 136) groups. Both received magnesium sulfate as needed,

Fetal and Neonatal Complications Associated With Acute Fatty Liver of Pregnancy.

Background Acute fatty liver of pregnancy (AFLP) is a rare but life-threatening obstetric emergency that can quickly endanger both the mother and fetus, typically presenting in the third trimester. Although greater awareness and supportive management have improved maternal survival, perinatal outcomes remain poor, particularly in resource-limited settings. This study aimed to describe fetal and neonatal complications associated with AFLP and identify maternal factors contributing to poor perinatal outcomes. Methods This retrospective study analysed medical records of 73 women diagnosed with AF

Importance of hospital-specific antibiotic policy for ensuring maternal quality of care in India: A narrative review.

Maternal health is pivotal in India, where infections significantly drive maternal morbidity and mortality. While antibiotics are essential for preventing sepsis, neonatal Group B Streptococcus, and surgical site infections (SSI), their misuse-fueled by empirical prescribing, self-medication, and over-the-counter access-has escalated antimicrobial resistance (AMR). This challenge is compounded by India's high maternal mortality, uneven healthcare resources, and cultural practices. Tailored, hospital-specific antibiotic policies, grounded in local epidemiology, resistance patterns, and resource…

Effect of Maternal Coronavirus Disease on Preterm Morbidities.

OBJECTIVE: Coronavirus disease (COVID-19) during pregnancy may have an impact on preterm morbidities due to the inflammatory nature of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Exposure to intrauterine inflammation could result in adverse consequences in preterm infants. We aimed to determine the effect of maternal coronavirus disease on preterm morbidities at a tertiary neonatal intensive care unit. STUDY DESIGN: This observational cohort study compared the clinical outcomes of preterm infants < 37 gestational weeks with and without maternal COVID-19. The study

Prenatal Diet and Children's Trajectories of Anxiety and Depression Symptoms from 3 to 8 Years: The EDEN Mother-Child Cohort.

BACKGROUND: Maternal diet quality during pregnancy has been linked to offspring's physical and mental health outcomes across the lifespan. However, few studies have examined its association with subsequent offspring's anxiety and depression issues. OBJECTIVES: The objective of the study was to examine the relationship between maternal prenatal dietary patterns and offspring's anxiety and depression symptoms from 3 to 8 years. METHODS: We used data from 1242 children enrolled in the French EDEN (Etude des déterminants pré- et postnatals précoces du développement et de la santé de l'enfant) bi

Maternal and perinatal outcomes with COVID-19: A systematic review of 108 pregnancies.

INTRODUCTION: The pandemic caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has exposed vulnerable populations to an unprecedented global health crisis. The knowledge gained from previous human coronavirus outbreaks suggests that pregnant women and their fetuses are particularly susceptible to poor outcomes. The objective of this study was to summarize the clinical manifestations and maternal and perinatal outcomes of COVID-19 during pregnancy. MATERIAL AND METHODS: We searched databases for all case reports and series from 12 February to 4 April 2020. Multiple terms

Placental DNA methylation at term reflects maternal serum levels of INHA and FN1, but not PAPPA, early in pregnancy.

BACKGROUND: Early detection of pregnancies at risk of complications, such as intrauterine growth restriction (IUGR) and preeclampsia (PE), is critical for improved monitoring and preventative treatment to optimize health outcomes. We predict that levels of placental-derived proteins circulating in maternal blood reflect placental gene expression, which is associated with placental DNA methylation (DNAm) profiles. As such, placental DNAm profiling may be useful to distinguish pregnancies at risk of developing complications and correlation between DNAm and protein levels in maternal blood may gi

Bridging the Gap in Health Disparities between Low-Middle Income Countries (LMIC) and High-Income Countries (HIC).

Good health is the basic right of every human being. Health inequality and disparities between low-middle-income countries (LMICs) and high-income countries (HICs) presents a substantial global health challenge. The differences in life expectancy, maternal health indicators, neonatal and child mortality indicators, burden of infectious diseases clearly point to the disparities between developed, developing and underdeveloped countries. This paper attempts to explore the reasons for these disparities, examines current approaches to mitigate them, and proposes the measures to enhance equity and …

Community-Acquired Pyelonephritis in Pregnancy Caused by KPC-Producing Klebsiella pneumoniae.

Carbapenem-resistant Enterobacteriaceae (CRE) usually infect patients with significant comorbidities and health care exposures. We present a case of a pregnant woman who developed community-acquired pyelonephritis caused by KPC-producing Klebsiella pneumoniae. Despite antibiotic treatment, she experienced spontaneous prolonged rupture of membranes, with eventual delivery of a healthy infant. This report demonstrates the challenge that CRE may pose to the effective treatment of common infections in obstetric patients, with potentially harmful consequences to maternal and neonatal health.

Infant mortality: a call to action overcoming health disparities in the United States.

Among all of the industrialized countries, the United States has the highest infant mortality rate. Racial and ethnic disparities continue to plague the United States with a disproportionally high rate of infant death. Furthermore, racial disparities among infant and neonatal mortality rates remain a chronic health problem in the United States. These risks are based on the geographical variations in mortality and disparities among differences in maternal risk characteristics, low birth weights, and lack of access to health care.

Improving maternity and neonatal care in England: protocol for a formative evaluation of the implementation of the core competency framework to enhance multi-professional practice.

BACKGROUND: Concern exists about the variation in maternity services across England, highlighted by various high-profile investigations into care failures. In response, NHS England developed the Core Competency Framework (CCF) to standardise training and reduce unwarranted discrepancies in maternity and neonatal care. The current version of the framework (CCFv2) outlines minimum training requirements, targets key areas of harm, and supports multi-professional education while allowing local adaptation. This study aims to explore and enhance the rollout and impact of the CCFv2 and to strengthen

Happy mothers, healthy minds: Maternal welfare and children's early development in the global south.

In low- and middle-income countries (LMICs), maternal well-being remains critical yet understudied in relation to early childhood development, particularly where poverty and systemic discrimination persist. This study examines the heterogeneous impact of mothers' subjective well-being, as measured by self-reported life satisfaction, on the developmental outcomes of children under the age of five. Leveraging data from the sixth round of the Multiple Indicator Cluster Surveys (MICS6) from 2018 to 2020 on approximately 160,000 mother-child dyads across 31 LMICs, the analysis exploits a novel inst

Implementing risk-appropriate maternity care-based triage model at a tertiary care teaching institute: an organisational quality improvement initiative to optimise risk and resources.

BACKGROUND: The annual births in our hospital (a regional perinatal centre for the southeastern coastal region) had increased to nearly 19 000 in 2019, straining the resources. Reduced low-risk childbirths due to the restrictions during the COVID-19 pandemic gave us an impetus to design and implement a risk-appropriate triage model referral system. We report its implementation process and examine its effect on birth rates and quality of care. METHODS: Initially, the data on childbirths (2019) and the districts where the majority belonged were analysed. We discussed the need for triaging and t
