The computer user syndrome.

Most conventional computer hardware systems have been inadequately designed for the functional comfort and efficient usage by human operators. The effects of excyclotorsion, inadequate depression-of-gaze, excessive hand-wrist pronation, and ulnar abduction are discussed. The neurology of ocular movements, synkinesis, saccades, pursuits, and orthopedic hand-wrist biomechanics are reviewed. The interrelationships of these ocular and orthopedic phenomena have been synthesized into a comprehensive hypothesis, in an effort to create a computer configuration which permits a greater integration of th

Associated reactions in the hemiplegic arm.

Of 40 stroke patients attending a rehabilitation department, 32 (80%) had associated reactions affecting the hemiplegic arm. These involuntary movements nearly always occurred in association with yawning and less frequently with stretching, coughing, sneezing and laughing. In many patients the pattern of these reactions differed from previous descriptions. In the majority of patients they raised false hopes about the recovery of arm function, but sometimes they were a cause of distress. All 8 patients who did not have associated reactions had some voluntary movement of the affected arm and 4 o

Treatment of tic douloureux with a new anticonvulsant (clonazepam).

Twenty-five patients affected by 30 episodes of tic douloureux were treated with a new anticonvulsant, clonazepam. In 40% there was complete control of the neuralgia and an additional 23.3% were significantly helped by the drug. Sixteen patients had previously been resistant to carbamazepine, and eight of them were completely and one partially relieved by clonazepam. The side-effects, somnolence and unsteadiness of gait, were present to some extent in 80 and 88% of the cases respectively, being severe in about half of them. It is concluded that clonazepam can be considered as a second choice f

[Neuropsychological aspect of normal pressure hydrocephalus (author's transl)].

The authors report ten cases of normal pressure hydrocephalus treated by ventricular drainage. They pay particular attention to the neuropsychological and psychometric aspects and endeavour to identify the most characteristic nosological features by comparing their own clinical observations with those reported in the literature.

Paratonia in Advanced Dementia: Deconstructing Scientific, Regulatory, and Health System Barriers to Botulinum Toxin A (BoNT-A) Treatment of Involuntary Muscle Resistance.

Paratonia, a movement disorder characterized by involuntary muscle resistance, affects nearly all people with advanced dementia and contributes to caregiver burden, hygiene complications, pressure injuries, and pain. With Alzheimer's disease and related dementias affecting 7.2 million Americans and projected to reach nearly 14 million by 2060, the clinical impact of paratonia will expand proportionally. Because the involuntary muscle resistance of paratonia is frequently misinterpreted as deliberate noncompliance, individuals may receive inappropriate pharmacologic management rather than targe

Occult breast cancer with CSF TBA positivity and abnormal involuntary movements.

Occult breast cancer (OBC) is a rare and special type of breast cancer. The initial symptom is an enlarged axillary lymph node. Paraneoplastic neurological syndromes (PNS) are a group of syndromes that are caused by malignant tumors and damage to the nervous system in distant parts. Most patients have neurological symptoms before the primary tumors. Early diagnosis and treatment are beneficial to improve the therapeutic effect of patients. This case report describes a 54-year-old woman presenting with a one-year history of gradual-onset abnormal involuntary movements, psychiatric disorders, me

Complexities in Managing Psychosis in a Patient With Stiff-Person Syndrome: A Case Report.

Stiff-person syndrome (SPS) is an uncommon autoimmune neurological disorder marked by painful muscle stiffness, muscle spasms, and limb weakness. Neurological symptoms in SPS can mimic a psychogenic movement disorder in which symptoms are triggered by sudden movement and emotional distress, which might delay proper treatment. However, psychiatric symptoms are far less common, and there is limited understanding regarding the co-occurrence of psychiatric conditions. Psychiatric symptoms include nonspecific anxiety, agoraphobia, and depression, which can be triggered by sudden movement, noise, or

Comparison of computerized methods for rapid eye movement sleep without atonia detection.

Rapid eye movement (REM) sleep without atonia detection is a prerequisite for diagnosis of REM sleep behavior disorder (RBD). As the visual gold standard method is time-consuming and subjective, several automated methods have been proposed. This study aims to compare their performances: The REM atonia index (RAI), the supra-threshold-REM-activity metric, the Frandsen index, the short/long muscle activity indices, and the Kempfner index algorithms were applied to 27 healthy control participants (C), 25 patients with Parkinson's disease (PD) without RBD (PD-RBD), 29 patients with PD and RBD (PD

Diffusion tensor imaging correlates of saccadic reaction time in children with fetal alcohol spectrum disorder.

BACKGROUND: Eye movement tasks provide a simple method for inferring structural or functional brain deficits in neurodevelopmental disorders. Oculomotor control is impaired in children with fetal alcohol spectrum disorder (FASD), yet the neuroanatomical substrates underlying this are not known. Regions of white matter have been shown by diffusion tensor imaging (DTI) to be different in FASD and thus may play a role in the delayed saccadic eye movements. The objective of this study was to correlate oculomotor performance with regional measures of DTI-derived white matter anisotropy in children

Peripheral and central changes combine to induce motor behavioral deficits in a moderate repetition task.

Repetitive motion disorders, such as carpal tunnel syndrome and focal hand dystonia, can be associated with tasks that require prolonged, repetitive behaviors. Previous studies using animal models of repetitive motion have correlated cortical neuroplastic changes or peripheral tissue inflammation with fine motor performance. However, the possibility that both peripheral and central mechanisms coexist with altered motor performance has not been studied. In this study, we investigated the relationship between motor behavior changes associated with repetitive behaviors and both peripheral tissue

Hypothetical membrane mechanisms in essential tremor.

BACKGROUND: Essential tremor (ET) is the most common movement disorder and its pathophysiology is unknown. We hypothesize that increased membrane excitability in motor circuits has a key role in the pathogenesis of ET. Specifically, we propose that neural circuits controlling ballistic movements are inherently unstable due to their underlying reciprocal innervation. Such instability is enhanced by increased neural membrane excitability and the circuit begins to oscillate. These oscillations manifest as tremor. METHODS: Postural limb tremor was recorded in 22 ET patients and then the phenotype

Sleep dysfunction in heart failure.

Chronic congestive heart failure is a highly prevalent and progressive disorder associated with excess morbidity and mortality; it has huge economic impact. Left heart failure may be systolic or may occur as isolated diastolic dysfunction. The diastolic form predominates in older people. Sleep disorders are frequent in both types. Most systematic studies have been performed in patients with systolic heart failure. Prospective studies show the presence of obstructive and central sleep apnea, periodic limb movements, and significant alterations in sleep architecture, characterized by poor effici

[Thinking of the treatment for lumbar spinal stenosis with high-dense silven needle therapy based on the theory of six-meridian differentiation].

The paper introduces the thinking of the diagnosis and treatment with high-dense silver needle therapy for lumbar spinal stenosis (LSS) based on the theory of six-meridian differentiation. According to the severity of LSS and the depth of illness location, LSS is differentiated as six syndromes/patterns, including taiyang disorder, yangming disorder, shaoyang disorder, shaoyin disorder, jueyin disorder and taiyin disorder. The high-dense silver needle therapy is used. The main points include the bilateral Jiaji points (EX-B 2) from L1 to L5 and the acupoints of the bladder meridian of foot-tai

Factors associated with constipation in infants from six to 12 months of age: a case-control study.

Brain-gut axis disorders (BIGDs) are expressed as a combination of recurrent gastrointestinal symptoms without the presence of anatomical abnormalities, inflammation, or tissue damage. From 6-12 months of age there is a change in the defecatory pattern, which is characterized by a gradual decrease in the frequency of bowel movements and an increase in the consistency of the stool. Factors associated with functional constipation in this age group have not been well identified.

Primary dystonia: moribund or viable.

With increasing understanding of dystonia genetic etiologies and pathophysiology there has been renewed scrutiny and reappraisal of dystonia classification schemes and nomenclature. One important category that includes both clinical and etiologic criteria is primary dystonia. This editorialized review discusses the impact of recent findings on primary dystonia criteria and argues that it remains useful in clinical and research practice. © 2013 Movement Disorder Society.

Psychogenic propriospinal myoclonus.

We report a case of probable psychogenic propriospinal myoclonus (PSM) in a patient who developed a sudden onset of disabling axial flexor myoclonus following a cosmetic surgical procedure. The electrophysiological findings were consistent with previous reports of PSM. Spontaneous remissions and disappearance of the jerks, sustained for 2 years, following removal of superficial surgical screws support the diagnosis of a psychogenic movement disorder.
