Secondary Cranioplasty After Fronto-Orbital Advancement: Analysis of a National Database.

INTRODUCTION: Fronto-orbital advancement (FOA) is the standard treatment for craniosynostosis patients with fronto-orbital dysmorphology. Few studies examine the incidence of secondary cranioplasty operations after FOA. This retrospective analysis utilizes multicenter billing data to identify risk factors and the incidence of secondary reconstructive cranioplasty operations after FOA. METHODS: A retrospective review was performed on all patients aged 0 to 16 years who underwent primary FOA between 2007 and 2015 in the Pearldriver database. The rates of secondary cranioplasty were analyzed thr

Perineural Spread of Orbital Immunoglobulin G4-Related Disease: A Case Report.

Immunoglobulin G4-related disease (IgG4-RD) is a rare, immune-mediated fibroinflammatory condition that can affect multiple organs. It is characterized by elevated serum IgG4 levels, lymphoplasmacytic infiltration, storiform fibrosis, and obliterative phlebitis. Diagnosis can be challenging due to limited awareness amongst clinicians and its wide range of clinical manifestations. We present a case of an elderly woman with bilateral orbital IgG4-RD and perineural spread, a rare manifestation of the disease. Serological testing showed markedly elevated IgG4 levels, and imaging revealed bilateral

Validation of mixed reality in planning orbital reconstruction with patient-specific implants.

This study aims to evaluate and compare the usability and performance of mixed reality (MR) technology versus conventional methods for preoperative planning of patient-specific reconstruction plates for orbital fractures. A crossover study design was used to compare MR technology with conventional three-dimensional (3D) printing approaches in the planning of maxillofacial traumatology treatments. The primary focus was on user-friendliness and the accuracy of patient-specific reconstruction planning. Secondary outcomes included investigating time differences between the two approaches and evalu

Isolated follicular lymphoma of the inferior oblique muscle presenting as an orbital mass.

A healthy, middle-aged woman presented to the ophthalmology outpatient department with a 12-month history of a palpable, non-tender mass on her right lower eyelid. The patient reported a 3-month history of night sweats. Her medical, family and social histories were unremarkable. On examination, her visual acuity was 6/6 in both eyes. She had a mass located inferior to the medial canthus and tethered to the orbital rim. She did not have diplopia, proptosis or strabismus and her ocular examination was otherwise normal. Imaging, biopsy and immunohistochemistry testing confirmed a diagnosis of fol

Endoscopic-assisted elevation of orbital blowout fracture in children: a case report.

BACKGROUND: Orbital fractures in children are uncommon. The existing literature shows a wide range of estimates for incidence, etiology, management protocols, and outcomes. Nevertheless, it is widely accepted that orbital wall fractures with extraocular muscle entrapment is a surgical emergency due to the risk of permanent diplopia caused by ischemia and necrosis of the muscle. Pediatric orbital floor injuries have distinct mechanistic features and fracture patterns when compared to adults. As a result, the prevalence, typical presenting signs and symptoms, and management considerations for pe

Globe Intussusception Following Orbital Trauma: Case Series and Review of Literature.

The aim of this paper is to report "Globe Intussusception" as an extreme form of globe dislocation outside the orbital pyramid, and provide a literature review. A single-center, retrospective, interventional case series of three patients is presented. A review of the English-language literature from the years 1971 to 2024 was performed using the search terms "traumatic globe dislocation", "maxillary sinus" and "ethmoid sinus". Three cases of globe intussusception are reported. Computed tomography imaging revealed orbital fracture, and globe prolapse into the maxillary sinus with or without inv

Bilateral sino-nasal-orbital fungal necrosis associated with hemophagocytic lymphohistiocytosis.

Hemophagocytic lymphohistiocytosis (HLH) is a rare hyperinflammatory condition caused by hyperactive T cells and macrophages, which release pro-inflammatory cytokines. The diagnosis is based on eight criteria: fever, bi- or tri-lineage cytopenias, splenomegaly, hyperferritinemia, hypofibrinogenemia or hypertriglyceridemia, elevated soluble CD25 levels, low or absent NK cell cytotoxicity, and hemophagocytosis. We report a 1-month-old male infant who was being treated in the pediatric intensive care unit for neonatal infection when he developed six of the eight criteria for HLH, along with bilat

The meningo-orbital band from an endoscopic transorbital approach: an anatomical study.

INTRODUCTION: The meningo-orbital band (MOB) is an intricate dural structure extending between the periorbita, the frontal dura, and the temporal dura. The endoscopic transorbital approach (ETOA) provides a more thorough understanding of its anatomy. MATERIALS AND METHODS: Anatomical dissections were performed on 15 human head specimens (30 orbits) at the Laboratory of Surgical Neuroanatomy (LSNA) at the University of Barcelona. The specimens were preserved using a Cambridge solution for optimal fixation. An endoscopic transorbital approach (ETOA) was used to isolate the meningo-orbital band

Trichosporon asahii and Candida guilliermondii as a Source of Orbital Infection in an Immunocompromised Individual.

A 47-year-old male with a history of immunosuppression and recent intensive care unit admission presented with progressive orbital swelling and pain. Blood cultures grew Trichosporon asahii and Candida guilliermondii, with matching positive cultures from peripheral and central venous samples. Given his fungemia and worsening orbital involvement, induction therapy with amphotericin B and isavuconazole was initiated. Within weeks, clinical improvement was noted, prompting a transition to long-term consolidation therapy with oral posaconazole and isavuconazole. At follow-up, the patient demonstra

Preoperative Administration of Amphotericin B in Orbital Mucormycosis Management: A Case Report.

This case report presents a 29-year-old male with diabetes mellitus who developed rhino-orbito-cerebral mucormycosis (ROCM) that was successfully treated with liposomal amphotericin B orbital injections. Despite emergent endoscopic debridement, the patient's disease progressed intracranially and intraorbitally, but he declined further surgical intervention. Subsequently, due to rapid acute vision loss, we initiated transcutaneous retrobulbar amphotericin B (TRAMB) injections. Following these injections, visual acuity, motility, and intraorbital fungal burden improved despite intracranial progr

Correcting Sunken Eyelid by Orbital Fat Transposition During Subbrow Lift.

BACKGROUND: The subbrow approach is reportedly more effective than upper blepharoplasty for improving lateral hooding. Additionally, it allows for the removal of thick periorbital fat while simultaneously tightening weakened and sagging orbicularis oculi muscles. This study introduces a method for repositioning and redistributing the medial fat pad to the central area, restoring fullness in the central area, and improving medial upper eyelid bulging. METHODS: From January 2023 to June 2024, 74 patients were followed up preoperatively, and 1 week and 6 months postoperatively. The medial and ce

Revolutionizing orbital reconstruction: Spectacle -retained prosthesis for post-mucormycosis defect.

The incidence of Mucormycosis has been seen an uptrend especially after the COVID-19 epidemic. The number of cases reporting to Prosthodontists has seen an uptick, with increasing demand to rehabilitate the patients with large intraoral and extra oral defects. Mucormycosis is an aggressive fungal infection that can result in severe complications, including involvement of the orbit, often requiring urgent surgical intervention such as enucleation of the affected eye and other structures. This case report details the management of a 57-year-old male patient who had his right eye enucleated due t

Comparison of transconjunctival and transnasal approaches for orbital decompression: a randomized controlled trial.

This randomized controlled trial aimed to compare the effectiveness of two surgical approaches to endoscopic orbital decompression for thyroid-associated ophthalmopathy (TAO). In total, 48 moderate-to-severe or sight-threatening TAO orbits were enrolled and randomly assigned into the transconjunctival group or transnasal group. The main outcome was the rate of effectiveness. If the postoperative proptosis was below 18 mm or the value of proptosis reduction was over 8 mm, this was defined as effective. The secondary outcomes included surgical accuracy, best-corrected visual acuity (BCVA), ocula
