20/40 or Better Visual Acuity After Optic Neuritis: Not as Good as We Once Thought?

BACKGROUND: Although patients with acute optic neuritis (ON) recover high-contrast visual acuity (HCVA) to 20/40 or better in 95% of affected eyes, patients with a history of ON continue to note subjective abnormalities of vision. Furthermore, substantial and permanent thinning of the retinal nerve fiber layer (RNFL) and the ganglion cell layer (GCL) is now known to occur early in the course of ON. We measured vision-specific quality of life (QOL) in patients with a history of acute ON and recovery of VA to 20/40 or better in their affected eyes to determine how these QOL scores relate to RNFL

Management of intracranial invasive olfactory neuroblastoma.

BACKGROUND: Olfactory neuroblastoma (ONB) is a rare tumor that often arise from the nasal cavity. The aim of this study was to investigate the clinical characteristics and treatments of intracranial invasive ONB. METHODS: Between July 2001 and August 2005, 5 patients with intracranial invasive ONB were treated in our department. Their clinical features, radiological and pathological characteristics, and surgical treatments were analyzed. Among the 5 patients, 1 received transnasal biopsy, and 4 were operated through the transfrontal or extended bifrontal approaches to reconstruct the skull ba

Binocular Function of Intermittent Exotropia Before and After Surgery.

Purpose: We examined the binocular function of intermittent exotropia before and after surgery, and studied the effect of the surgery and the factor that might be involved in its result.Materials and Methods: 402 intermittent exotropic patients, who underwent their first surgical correction for the strabismus at Teikyo University Hospital, were examined for retinal correspondence and stereo acuity before and after surgery. Results: 367 patients (91.3%) had already had good stereo acuity before surgery when their eyes were straight. Regarding retinal correspondence, though 192 patients (47.8%) w

Sociodemographic aspects and systemic diseases of cataract patients.

Sociodemographic analysis of 351 consecutive cataract patients operated on at the Oulu University Hospital in 1990 revealed that 30.0% of the patients lived alone at home, 62.0% lived with some other person and 7.9% were in institutions. The mean age was 70.6 +/- 12.4 years, the sex distribution (62.7% women, 37.3% men) corresponded with that in the general population of the same age. 78.8% of the patients had had surgery on the first eye, 29.0% of them had low vision or blindness (visual acuity less than 0.3 in the better eye). In the second eye surgery group (21.2% of the patients) low visio

Strategies to Reduce Failure to Rescue after Cardiac Surgery.

Failure to rescue (FTR), defined as mortality after a postoperative complication, is now a central quality metric in cardiac surgery, where high-acuity patients and resource-intensive care increase vulnerability to deterioration. Although complications are common, survival depends on timely recognition, effective escalation, and coordinated multidisciplinary management. This article synthesizes current evidence on the epidemiology, mechanisms, and system-level drivers of FTR, highlighting how patient, provider, and institutional factors influence rescue success. We outline practical, evidence-

Non-Arteritic Ischaemic Optic Neuropathy Associated with Semaglutide Use.

PRESENTATION: A 50-year-old obese male with non-insulin dependent diabetes mellitus (NIDDM) taking Semaglutide, a glucagon-like peptide-1 receptor agonist (GLP-1RA) presented with sudden, painless vision-loss from his right eye. DIAGNOSIS: Right eye visual acuity was 6/24. There was Frisen grade IV optic nerve head (ONH) oedema. This was confirmed at imaging with ocular coherence tomography (OCT). The patient was diagnosed with non-arteritic anterior ischaemic optic neuropathy. TREATMENT: Semaglutide was ceased. DISCUSSION: GLP-1RAs such as Semaglutide are being increasingly recognised as a

Maculopathy due to hypotony: A diagnostic challenge.

Hypotony maculopathy is a potential complication of glaucoma filtering surgery, clinically relevant as it can cause vision loss. Classically, it produces choroidal folds, vascular tortuosity, and optic disc edema. However, in optical coherence tomography (OCT), it may also present as intraretinal cysts (IRC). We present the case of a patient who, 10 years after undergoing glaucoma surgery, developed ocular hypotony, IRC, and decreased visual acuity (VA). Given the patient's history of diabetes mellitus, it was initially diagnosed as diabetic macular edema (DME), which delayed appropriate treat

Microsurgical Management of Large Superior Hypophyseal Artery Aneurysm Presented with Visual Impairment.

Superior hypophyseal artery aneurysms are rare. Symptomatic superior hypophyseal artery aneurysm commonly presents with subarachnoid hemorrhage, uncommonly with cranial nerve deficits, and very rarely with visual impairment. Microsurgical management of such aneurysms is quite complex considering the anatomical structures in the paraclinoid region. A 43-year male presented with decreased visual acuity in both eyes. The patient was diagnosed with left supraclinoid partially thrombosed unruptured aneurysm with mass effect over optic chiasm and optic nerves on magnetic resonance imaging of the bra

Optic Neuritis As the Initial Presentation of Syphilis.

Syphilis is transmitted via sexual contact and spreads through the lymphatic and hematologic systems. Ocular syphilis is an uncommon yet serious manifestation of syphilis that can cause significant vision impairment. This report presents a case of ocular syphilis in a 41-year-old otherwise healthy woman who presented with left-sided blurry vision and ocular discomfort. Examination confirmed decreased visual acuity and optic disc swelling in the left eye. Magnetic resonance imaging findings suggested optic neuritis. After ruling out other causes of optic neuritis and considering the presentatio

Cystoid Macular Edema: A Rare Adverse Reaction to Rituximab.

Membranous glomerulonephritis is the leading cause of nephrotic syndrome in non-diabetic Caucasian adults. For patients at risk of progressing to end-stage renal disease, immunosuppression, particularly rituximab, is the recommended treatment. While extremely rare, cases of cystoid macular edema associated with rituximab have been documented in the literature. In this report, we present the case of a 54-year-old male with membranous glomerulonephritis at a high risk of progressing to end-stage renal disease who experienced cystoid macular edema hours after receiving rituximab infusion. Followi

Complications of Bacterial Keratitis Impacted by Social Determinants of Health: A Case Report.

INTRODUCTION: The emergency department commonly evaluates eye pain and vision loss. Typically, these conditions can be managed outpatient; however, delays can lead to advanced pathology. CASE REPORT: A 48-year-old homeless male presented with left-eye vision loss and pain. His exam revealed monocular decreased visual acuity, corneal ulcer, and hypopyon. The patient was diagnosed with bacterial keratitis and admitted for treatment but left against medical advice. He returned and was admitted for further treatment but was lost to follow-up thereafter. CONCLUSION: Our case features complicated

Rare Orbital Involvement Originating from Extranodal Marginal Zone Lymphoma.

Ocular adnexa region (OAR) primary lymphomas are uncommon, accounting for 1-2% of non-Hodgkin lymphomas and 8% of extranodal lymphomas. Extranodal marginal zone lymphoma (EMZL) originates from several epithelial tissues, including the stomach, salivary gland, lung, small intestine, thyroid gland, and ocular adnexa region. Here, we report a 66-year-old female patient who was diagnosed with EMZL of OAR. In consideration of the possible side effect of radiotherapy, such as conjunctivitis, visual acuity impairment, and even retinal complications, she received six cycles of triweekly targeted chemo

Subthreshold Micropulse Laser Acute Exudative Maculopathy in Central Serous Chorioretinopathy.

Photodynamic acute exudative maculopathy is a transient increase in subretinal fluid (SRF) within the first days after treatment in different chorioretinal diseases and tumors. Recently, this entity has attracted much attention. However, no cases of acute exudative maculopathy have been described after subthreshold laser (STL). This report presents the case of a 35-year-old male with chronic central serous chorioretinopathy with an extrafoveal serous retinal pigment epithelium detachment (PED) who underwent STL. One day after treatment, the patient noted an acute decrease in visual acuity and

[Reversible bilateral blindness associated with alcoholic ketoacidosis: a case report].

A 51-year-old male with a history of chronic alcoholism presented to the emergency department with an abrupt onset of complete bilateral blindness lasting for one hour. Funduscopic examination yielded unremarkable findings. Systemic evaluations revealed the presence of severe ketoacidosis. The patient spontaneously regained light perception after experiencing total blindness for 3 hours; however, he subsequently developed hypothermia and entered a state of shock. Following treatment with sodium bicarbonate and aggressive fluid resuscitation, his condition stabilized, and there was a rapid impr

UNILATERAL CHEMOSIS, BULLOUS SEROUS RETINAL DETACHMENT, AND PRESUMED BACILLARY LAYER DETACHMENT IN SEVERE PREECLAMPSIA.

BACKGROUND/PURPOSE: To report a case of unilateral chemosis, serous retinal detachment (SRD), and presumed bacillary layer detachment in pregnancy. METHODS: Clinical examination and multimodal imaging findings were reviewed and analyzed. RESULTS: A 17-year-old female patient with severe preeclampsia was found to have unilateral chemosis and bullous SRD. Postpartum optical coherence tomography revealed thickened choroid and presumed bacillary layer detachment. Two weeks later, chemosis and SRD resolved, leaving behind mottled hyperpigmentation, drusen, and subretinal drusenoid deposits. CONC

Foveal Hypoplasia Related to Congenital Rubella.

Normal development of the fovea begins before midgestation and continues for several years after birth. Foveal hypoplasia is a condition in which the foveal pit and the foveal avascular zone do not fully develop. Several diseases are known to be associated with foveal hypoplasia; however, a direct association between foveal hypoplasia and congenital rubella has not been stated so far. This report describes a case of foveal hypoplasia detected during adulthood in a patient with known fetal exposure to maternal rubella infection and several ocular features of congenital rubella syndrome, includi

Venous sinus stenting for the treatment of acute blindness in a patient with idiopathic intracranial hypertension.

We present a case of acute onset of blindness treated with venous sinus stenting in the hyperacute period (24-48 h). The patient had not been diagnosed previously with idiopathic intracranial hypertension and presented at an outside facility with a short history of headache, nausea, vomiting and visual deficits. Initial management included lumbar punctures for pressure relief. Unfortunately, the patient's condition deteriorated and she experienced the rapid onset of blindness. Within 24-48 h of blindness onset, she was transferred to our institution for emergent endovascular stenting of the ve

Pre-Descemetic Placement of Intracorneal Ring Segments for Keratoconus after Mechanical Implantation.

We report the case of a 23-year-old male who was referred to our clinic for a routine follow-up examination. The patient was treated for keratoconus 3 years ago in Colombia, where intracorneal ring segments were implanted mechanically in both eyes to improve his visual acuity. Surprisingly, we discovered a pre-descemetic placement of intracorneal ring segments, which could lead to potential complications, under circumstances. We illustrate this impressive finding with slit-lamp photography and high-resolution anterior segment OCT and advocate for the use of femtosecond laser as the method of c

Bilateral Sequential Paracentral Acute Middle Maculopathy.

We aim to present a case with bilateral sequential paracentral acute middle maculopathy (PAMM). A 57-year-old man presented with paracentral scotoma in the left eye. The patient's multimodal imaging findings were consistent with PAMM in the left eye. Extensive systemic work-up revealed hypertension and a history of cerebrovascular event. One year after initial presentation, the patient had a subsequent decrease in visual acuity in the right eye and developed optical coherence tomography findings consistent with PAMM, whereas the left eye showed resolved PAMM findings. Although rare, PAMM can o
