The post-thrombotic syndrome: a 2012 therapeutic update.

Post-thrombotic syndrome (PTS) refers to chronic manifestations of venous insufficiency following a deep-vein thrombosis (DVT). It is a frequent, chronic, burdensome and costly disease for which therapeutic options are limited. Above all, the optimal management of PTS consists of preventing its occurrence: first, by preventing DVT, and second, by preventing development of PTS after a DVT. Prevention of DVT is challenging, particularly in the case of nonsurgical hospital inpatients, where physician's adherence to recommended thromboprophylaxis is often low. In our opinion, this adherence should

Patient reported barriers and facilitators to using a self-management booklet for hip and knee osteoarthritis in primary care: results of a qualitative interview study.

BACKGROUND: To enhance guideline-based non-surgical management of hip or knee osteoarthritis (OA), a multidisciplinary, stepped-care strategy has been implemented in primary care in a region of the Netherlands. To facilitate this implementation, the self-management booklet "Care for Osteoarthritis" was developed and introduced. The aim of the booklet was to educate patients about OA, to enhance the patient's active role in the treatment course, and to improve the communication with health care providers. To successfully introduce the booklet on a large scale we assessed barriers and facilitato

[Spanish Society of Anaesthesia (SEDAR) guidelines for pre-anaesthesia checking procedures].

We present this document as a guide to preparing a specific institutional pre-anaesthesia checklist, as recommended in the Helsinki declaration on patient safety in anaesthesiology. Also, the recently recommended WHO "safe surgery check-list" includes a check-list for anaesthesia. A working group was established in accordance with the charter of the Spanish Society of Anaesthesiology and Resuscitation (Sociedad Española de Anestesiología y Reanimación [SEDAR]). The new patient safety culture introduced into medicine, and the recommendations of European anaesthesia societies has led us to desig

The whole is greater than the sum of the parts: recognising missed opportunities for an optimal response to the rapidly maturing TB-HIV co-epidemic in South Africa.

BACKGROUND: Despite widely acknowledged WHO guidelines for the integration of TB and HIV services, heavily burdened countries have been slow to implement these and thus significant missed opportunities have arisen. DISCUSSION: The individual-centred, rights-based paradigm of the SA National AIDS Policy, remains dissonant with the compelling public-health approach of TB control. The existence of independent and disconnected TB and HIV services results in a wastage of scarce health resources, an increased burden on patients' time and finances, and ignores evidence of patients' preference for an

Rationale for the hypertension guidelines for primary care in South Africa.

New South African guidelines are proposed by the Hypertension Society of Southern Africa for the management of hypertension by primary health care services in South Africa. Specific South African guidelines are appropriate for hypertension, which is now recognised as one of the five major diseases that must be given priority by the new Government. Furthermore, patient participation and empowerment in blood pressure (BP) control become feasible through the new concept of lifestyle modification. This article gives the rationale underpinning these guidelines. The correct methods to measure blood …

Association Between Advance Care Planning and Place of Death: A Systematic Review and Meta-Analysis.

BACKGROUND: Place of death is a key indicator of quality in end-of-life care, reflecting how well care aligns with patients' values and preferences. Advance care planning (ACP) has been proposed to support goal-concordant care, yet evidence regarding its association with actual place of death remains inconsistent. OBJECTIVE: To examine the association between participation in ACP and place of death and to explore whether this association varies according to ACP implementation method. METHODS: In this systematic review and meta-analysis, we searched five databases (PubMed, Embase, PsycInfo, C

An Age-Adapted Co-Design Methodology for Community Health Research Involving Older Adults With Type 2 Diabetes.

INTRODUCTION: Co-design is increasingly used in health and social care research, but its application with older adults in community settings may raise methodological challenges. Age-adapted co-design methods are needed to support meaningful participation. METHODS: This study developed and examined an age-adapted co-design methodology in a community setting involving older adults with Type 2 diabetes mellitus (T2DM). A qualitative approach was adopted, which included exploratory interviews, design probes, clarification interviews, a co-design workshop and evaluation interviews. A total of 24 p
