Differential regulation of SOCS signalling and immunoproteasome pathways by JAK inhibitors and TNF inhibitors in rheumatoid arthritis.

BACKGROUND: Janus kinase (JAK) inhibitors and TNF inhibitors (TNFi) are established therapies in rheumatoid arthritis (RA), yet their differential effects on the JAK/STAT/SOCS/proteasome axis remain incompletely understood. This pilot study systematically characterised treatment-specific effects of baricitinib, tofacitinib, and TNFi upon molecular signatures in RA patients. METHODS: Seventy-five patients with RA were enrolled across two prospective cohorts: an initial inception cohort treated with tofacitinib (n = 15) and a subsequent confirmatory cohort treated with baricitinib or TNFi (n =

Trials in progress: CARMAN - study protocol of a randomized controlled, international, multicenter, open-label phase II trial evaluating early treatment intensification in patients with high-risk mantle cell lymphoma using CAR-T-cell treatment after an abbreviated induction therapy with rituximab and ibrutinib and 6 months ibrutinib maintenance as compared to standard of care induction and maintenance.

BACKGROUND: Brexucabtagene-autoleucel (brexu-cel) is an anti-CD19 chimeric antigen receptor T-cell (CAR-T) product approved for relapsed/refractory Mantle Cell Lymphoma (MCL) after two prior treatment lines, including Bruton tyrosine kinase inhibitors (BTKi). Patients with high-risk (hr) disease-defined by high-intermediate or high-risk MIPI-c, p53 overexpression, or TP53 alterations-have a poor prognosis, underscoring the need for improved first-line strategies. The European Mantle Cell Lymphoma Network therefore designed a phase II trial to investigate the incorporation of brexu-cel into fir

[Recent advances and future perspectives in the treatment of chronic lymphocytic leukemia].

The treatment of chronic lymphocytic leukemia (CLL) has rapidly evolved from traditional chemoimmunotherapy to molecular targeted therapies. Current recommended first-line treatment options include covalent BTK inhibitor (cBTKi: ibrutinib, acalabrutinib±obinutuzumab, and zanubrutinib)-based regimens and BCL2 inhibitor (BCL2i: venetoclax)-containing regimens (venetoclax+obinutuzumab and venetoclax+ibrutinib). The former approach relies on continuous treatment with cBTKi for long-term disease control, and the latter is a time-limited approach using BCL2i aiming for long-term treatment-free remis

Mathematical Modeling of HDV RNA, HBV DNA, and HBsAg Dynamics During Lonafarnib-Based Therapy: Insights from the LOWR HDV-1 Study.

Lonafarnib (LNF) is an investigational drug targeting hepatitis delta virus (HDV) but not hepatitis B virus (HBV), providing a unique opportunity to model HDV kinetics and how changes in HDV affect HBV. We performed a detailed kinetic analysis and developed a mathematical model to explain serum HBV DNA, HDV RNA and hepatitis B surface antigen (HBsAg) kinetics in 15 HBV/HDV coinfected patients receiving LNF-based treatment. After a delay of 0-2 days, patients experienced a rapid 1st-phase HDV-decline followed by either a viral plateau, 2nd slower-decline phase, or viral breakthrough (VB). LNF m

The glutamyl-prolyl-tRNA synthetase 1 inhibitor halofuginone exerts synergistic antitumor effects with bortezomib in colon cancer cells.

Glutamyl-prolyl-tRNA synthetase 1 (EPRS1) is a key enzyme in protein synthesis and is implicated in various diseases, including inflammation, fibrosis, and cancer. Here, we found that EPRS1 is highly expressed in patients with colon cancer and is positively correlated with poor prognosis. Halofuginone, a derivative of febrifugine extracted from the traditional Chinese herb Dichroa febrifuga, was previously identified as a specific inhibitor of EPRS1. Our research found that halofuginone significantly inhibit the growth of colon cancer cells. Mechanistic investigations demonstrated that halofug

Real-world utilization of Bruton tyrosine kinase inhibitors in relapsed/refractory chronic lymphocytic leukemia in Australia.

The relapsed/refractory chronic lymphocytic leukemia treatment landscape has continued to evolve with Bruton's tyrosine kinase inhibitor (BTKi) emergence. Alongside extensive clinical trial results, real-world evidence for BTKi use in clinical practice is useful. Yet, published Australian real-world evidence is limited. This retrospective study describes ibrutinib and acalabrutinib use in the Australian real-world context, using data from December 2017 to August 2023 from the Pharmaceutical Benefits Scheme (PBS) 10% sample dataset. Acalabrutinib and ibrutinib were primarily dispensed in the se

Evaluation of imipenem-relebactam, meropenem-vaborbactam, aztreonam-avibactam and cefepime-zidebactam activities on a wide collection of French clinical Enterobacterales isolates.

In recent years, novel β-lactam-inhibitor combinations (imipenem-relebactam (I/R), meropenem-vaborbactam (M/V), aztreonam-avibactam (A/A)) have been commercialized and some are not yet on the market (cefepime-zidebactam (C/Z)). The objective of this study was to evaluate the efficacy of these β-lactam-β-lactamase inhibitors (BL/BLIs) combinations against a wide collection of French clinical multiresistant Enterobacterales isolates and to assess the performance of the E-test MIC method for I/R and M/V. BL/BLIs MICs were determined by broth microdilution on a collection of 200 ESBL-producing and

Upshot of Some Bioactive Compounds on Angiogenesis in Retinal Pigment Epithelial Cells.

Nowadays, the use of monoclonal antibodies to target angiogenic signalling pathways is common, but, unfortunately, the clinical activity of these agents is limited. Thus, the development of approaches targeting multiple pathways for anti-angiogenic effect will lead to increase the clinical benefit. For this purpose, oleuropein, hesperidin, piperine, proanthocyanidins and retinoic acid, which have previously been proven to be bioactive components, anti-angiogenic performances were experimentally tested in retinal pigment epithelial cells. Bioactive ingredients were applied to retinal pigment ep

5-Hydroxytryptamine 1F receptor loss reduces renal vasculature and prevents lasmiditan-induced recovery following moderate-severe acute kidney injury in mice.

Kidney disease (KD) has emerged as a major global health crisis and leading cause of morbidity and mortality worldwide, impacting over 850 million individuals. Pathophysiological hallmarks of KD encompass renal tubular cell injury/necrosis, tubulointerstitial fibrosis, vascular dysfunction/rarefaction, and mitochondrial dysfunction, all of which are implicated in disease initiation/progression. Unfortunately, there remains a general lack of effective Food and Drug Administration (FDA)-approved therapeutics for the treatment of KD. Thus, the identification of novel and/or repurposed treatment s

Venous thromboembolism events associated with tofacitinib in rheumatoid arthritis patients: a real-world study from FAERS database.

Tofacitinib, a Janus kinase (JAK) inhibitor approved in 2012, has become a pivotal oral treatment for rheumatoid arthritis (RA). However, the risk of venous thromboembolism (VTE) with tofacitinib in RA patients remains uncertain. This study aims to assess the association between tofacitinib use and the risk of VTE events in patients with RA using data from the U.S. Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS). Additionally, this analysis sought to compare the reporting odds ratio (ROR) of VTE associated with tofacitinib and tumor necrosis factor inhibitors (TNFi).

Radiation-guided nanoparticles enhance the efficacy of PARP inhibitors in primary and metastatic BRCA1-deficient tumors via immunotherapy.

Poly (ADP-ribose) polymerase inhibitors (PARPi) have revolutionized the treatment landscape for patients suffering from BRCA1-mutated breast and ovarian cancers. However, responses are not durable. We demonstrate that treatment with PARPi, niraparib, increases programmed death-ligand 1 (PD-L1) expression in BRCA1-deficient cancer cells, contributing to immune evasion. To circumvent this, we developed P-selectin-targeted poly (lactic-co-glycolic) acid (PLGA)-poly (ethylene glycol) (PEG)-based nanoparticles (NPs) encapsulating PARP and PD-L1 inhibitors at a synergistic ratio. To further enhance

Old drugs with new applications: Tofacitinib-mediated inhibition of M1-like macrophage polarization in acute pancreatitis by the JAK/STAT and emerging NOD-like receptor pathways.

BACKGROUND: Given the potential for persistent inflammation to induce severe systemic inflammatory consequences, the development of new drugs for the early inflammatory cascade of acute pancreatitis (AP) is particularly important. The aim of this study was to investigate the activity of the JAK inhibitor tofacitinib (TOFA) against AP. METHODS: In vivo, TOFA efficacy against AP was assessed in a mouse model induced by the intraperitoneal injection of caerulein and retrograde pancreatic duct infusion of sodium taurocholate (STC). In vitro, we evaluated the effect of TOFA on the polarization of
