Cole Osteotomy and Variations.

Pes cavus can be a painful and debilitating condition that is often recalcitrant to conservative treatment. Oftentimes surgical reconstruction is necessary for the patient to obtain a pain-free plantigrade foot. An intimate knowledge of pes cavus foot architecture is necessary to select appropriate surgical treatment of the symptomatic patient. Midfoot osteotomies such as the Cole osteotomy can be a powerful procedure to reduce deformities where the apex is located in the midfoot. Midfoot osteotomies are almost never performed in isolation. Midfoot osteotomies including the Cole osteotomy are

Updates From the Other Side of the Drape: Recent Advances in Multimodal Pain Management and Opioid Reduction Among Pediatric Surgical Patients.

As the management of acute pain for children undergoing surgical procedures as well as recognition of the short and long term risks of exposure to opioids has evolved, multimodal and multidisciplinary approaches using organized pathways has resulted in improved perioperative outcomes and patient satisfaction. In this 2023 symposium held at the American Academy of Pediatrics on Surgery meeting, a multidisciplinary discussion on current enhanced recovery after surgery pathways, alternate methods of effective pain control and education and advocacy efforts for opioid reduction were discussed, and

Endoscopic Repair of Partial Gluteus Medius Tear: Staple Configuration With Suture Anchors.

Tears of the gluteus medius and minimus have emerged as significant contributors to lateral hip pain and functional impairment. Surgical intervention reliably delivers pain alleviation and ability to return to activities. Nevertheless, conventional open procedures entail substantial incisions, extensive tissue damage, and protracted recovery. Endoscopic approaches have provided minimally invasive alternative strategies. This Technical Note details a method for addressing partial gluteus medius tears by recreating the hip abductor tendon insertion footprint and employing a bioinductive patch to

Single-Incision Endoscopic Trigger Finger Release.

Trigger finger surgery is a commonly performed open procedure with known potential complications of infection, stiffness, pain, nerve injury, bowstringing, and incomplete release of the A1 pulley. We present a novel single-incision endoscopic trigger finger release technique that moves the incision from the palm to the palmar-digital crease, leading to less pain, scarring, and stiffness. We believe that this technique is technically simple, fast, and may decrease the risk of complications commonly encountered with open trigger finger release. Level of Evidence: Therapeutic IV.

Novel steroid infiltration technique of the carpal tunnel.

This report describes a novel technique of steroid infiltration of the wrist to treat symptomatic carpal tunnel syndrome. Our approach potentially reduces direct trauma to the median nerve when compared with current conventional techniques. The use of a cannula allows infiltration directly into the carpal tunnel and advancement of the blunt tip minimises the risk of sharp trauma to the median nerve and adjacent tendons. This avoids the unpleasant, shooting pain frequently experienced by patients using traditional needle infiltration. We anticipate this would be of particular benefit in reducin

Stump Appendicitis.

Abdominal pain is a frequent problem encountered in the emergency department, and acute appendicitis is a well-recognized diagnosis. Laparoscopic appendectomy has become one of the most common surgical procedures in the United States. Patients with a history of appendectomy may experience recurrent right lower quadrant abdominal pain from an infrequently encountered complication that may occur when the residual appendix becomes obstructed and inflamed. We describe two cases of stump appendicitis in pediatric patients with a review of clinical and imaging findings and surgical management.

Headache in children and adolescents.

The broad field of orofacial pain encompasses pain conditions affecting the head, face, neck and intraoral structures. Much has been written about headache specifically affecting adults, but little is known about the presentation of this problem in children. Dentists may be asked to participate in the differential diagnosis or management of headache in young patients. For their benefit, this article discusses the epidemiology, clinical presentation, diagnosis and treatment of headache in this population. An accurate history and careful examination can allow early diagnosis and the avoidance of

[Differential diagnosis of back pain].

Back pain is one of the most frequent clinical pictures encountered in a physician's practice. It can pose a great burden on the individual and in addition have a multifactorial origin. It can be caused by intervertebral discs, vertebral joints, nerve roots, ligaments, sacroiliac joints, or a combination of the above. Back pain, as a symptom of a systemic disease, can also be a warning signal of grave disorders such as malignancies or in the event of aortic aneurysm. The well-considered choice of appropriate diagnostic procedures and suitable treatment requires a thorough knowledge of this mul

Endoscopic removal of a dental prosthesis in the hepatic flexure of the colon.

The diagnosis of impacted foreign body in the colon is usually delayed until the complications such as perforation or abscess formation occur. Here we describe a patient who presented with diffuse abdominal pain due to the impaction of a dental prosthesis in the hepatic flexure of the colon. The dental prosthesis, which was inadvertently swallowed, was successfully removed under colonoscopy. Unexplained abdominal pain should alert the clinician to the possibility of foreign body ingestion and further therapeutic colonoscopy may replace or lessen the need for surgical procedures to extract fore

Chemonucleolysis failures treated by PLIF.

Chemonucleolysis is a procedure in which an enzyme is injected into the intervertebral disc for the purpose of alleviating sciatic pain. This has a reported success rate of 70%-90%. Of the failures, several will benefit from a classic lumbar laminectomy for removal of a free fragment in the spinal canal caused by either improper technique or inadequate enzyme activity. However, in a significant number of cases, the failure is related to either instability or the compression of the nerve roots in the neural foramen. Observations on 17 cases demonstrate that PLIF can relieve pain and other sympt

Juvenile kyphosis.

Scheuermann's disease is a common cause of kyphosis developing in the juvenile period. Untreated, the deformity may progress, producing significant pain and, rarely, neurologic signs and symptoms. Successful treatment is possible with the Milwaukee brace, provided that the child has not reached skeletal maturity. Bracing will produce a superior result in terms of correcting the deformity and reversing vertebral wedging. Surgery may occasionally be necessary for severe kyphosis, especially in the adult who presents with pain and/or neurologic problems. Combined anterior and posterior approach i

Comparison of laparoscopic intraperitoneal onlay mesh (IPOM Plus) and extended totally extraperitoneal (eTEP RS) repair for incisional hernia - a randomized clinical trial.

BACKGROUND: Laparoscopic intraperitoneal onlay mesh repair with a combination of hernial defect closure (IPOM plus) is an effective and common surgical procedure performed in patients with incisional ventral hernia. The introduction of extended-view totally extraperitoneal Rives-Stoppa procedure (eTEP RS) has sparked debate among surgeons regarding the optimal technique for ventral hernia repair. Our randomized clinical study aims to compare IPOM plus and eTEP RS techniques in patients with midline incisional ventral hernias. METHODS: This is a prospective, single-center, single-blind randomi

Genicular Artery Embolization Using Rapidly Resorbable Gelatin-based Microspheres for Osteoarthritis-related Knee Pain.

Background Genicular artery embolization (GAE) is an emerging minimally invasive therapy for osteoarthritis (OA)-related knee pain. Traditional embolic agents such as imipenem-cilastatin and permanent microspheres have distinct limitations. Rapidly resorbable gelatin-based microspheres (RRGMs), designed specifically for GAE, may offer a promising alternative. Purpose To evaluate the safety and clinical outcomes of GAE using RRGMs in participants with OA-related knee pain. Materials and Methods This prospective, single-center observational study included consecutive participants with OA-related

Comparative Analysis of Early and Long-Term Outcomes of Patients with Degenerative Lumbar Spine Disease Using the DIAM Stabilizer and Standard Rehabilitation Program: A Preliminary Prospective Randomized Controlled Trial with 1-Year Follow-Up.

Low back pain (LBP) is a leading cause of disability and work absenteeism. The cause of LBP may be degeneration of the intervertebral disc. LBP is characterized by considerable variability and tends to develop into chronic pain. Treatment of LBP includes conservative and rehabilitative treatments, surgery, and so-called minimally invasive treatment. One of the most commonly performed procedures is interspinous stabilization using a dynamic interspinous DIAM (device for intervertebral assisted motion) stabilizer. There is still no clear, strong evidence for the effectiveness and superiority of
