An observational survey assessing the extent of PrEP and PEP furnishing in San Francisco Bay Area pharmacies.

BACKGROUND: Human immunodeficiency virus (HIV) remains prevalent in the United States despite medications that reduce the risk of infection, primarily pre-exposure prophylaxis (PrEP) and postexposure prophylaxis (PEP). In 2019, California authorized pharmacists to furnish PrEP and PEP under Senate Bill 159 (SB-159). OBJECTIVE: Assess implementation of SB159 in San Francisco Bay Area community and mail-order pharmacies. METHODS: We conducted an observational, cross-sectional survey of independent community and mail-order pharmacies in the 9-county San Francisco Bay Area to identify those that

Antioxidant dressing therapy versus standard wound care in chronic wounds (the REOX study): study protocol for a randomized controlled trial.

BACKGROUND: A wound that does not heal in the orderly stages of the healing process or does not heal within 3 months is considered a chronic wound. Wound healing is impaired when the wound remains in the inflammatory stage for too long. A range of factors can delay the healing process: imbalance between proteases and protease inhibitors in the wound bed; bacterial colonization and the presence of biofilm; and oxidative stress. Recently, wound management has improved significantly. A new antioxidant dressing has been developed, which combines an absorbent matrix obtained from locust bean gum ga

Microarchitectural adaptations in aging and osteoarthrotic subchondral bone issues.

The human skeleton optimizes its microarchitecture by elaborate adaptations to mechanical loading during development and growth. The mechanisms for adaptation involve a multistep process of cellular mechanotransduction stimulating bone modelling, and remodeling resulting in either bone formation or resorption. This process causes appropriate microarchitectural changes tending to adjust and improve the bone structure to its prevailing mechanical environment. Normal individual reaches peak bone mass at age between 25 and 30 years, and thereafter bone mass declines with age in both genders. The b

Outcomes after endovascular aneurysm repair conversion and primary aortic repair for urgent and emergency indications in the Society for Vascular Surgery Vascular Quality Initiative.

OBJECTIVE: Open conversion after endovascular aortic aneurysm repair (EVAR-c) is performed nonelectively in up to 60% of cases. EVAR-c has been reported to have significantly greater risk of postoperative morbidity and mortality than primary aortic repair, but few data exist on outcomes for symptomatic or ruptured presentations. This study determined outcomes and identified predictors of postoperative major adverse cardiac events (MACEs) and mortality for patients undergoing nonelective EVAR-c compared with nonelective primary aortic repair (PAR) in the Vascular Quality Initiative (VQI). Integrating Gait Analysis and AI Into Knee Osteoarthritis Care: Protocol for a 3-Phase Participatory Qualitative Study.

BACKGROUND: Knee osteoarthritis (OA) leads to pain, disability, and reduced quality of life. For advanced stages, knee arthroplasty surgery is the standard treatment, yet dissatisfaction and persistent mobility deficits remain common. Current surgical decision-making processes seldom incorporate objective predictors of outcomes, such as biomechanical data. Advances in computer vision, wearable sensors, and artificial intelligence now enable efficient capture and interpretation of clinically prognostic gait features in real-world settings. However, the clinical adoption of such innovations rema

Mitii™ ABI: study protocol of a randomised controlled trial of a web-based multi-modal training program for children and adolescents with an Acquired Brain Injury (ABI).

BACKGROUND: Acquired brain injury (ABI) refers to multiple disabilities arising from damage to the brain acquired after birth. Children with an ABI may experience physical, cognitive, social and emotional-behavioural impairments which can impact their ability to participate in activities of daily living (ADL). Recent developments in technology have led to the emergence of internet-delivered therapy programs. "Move it to improve it" (Mitii™) is a web-based multi-modal therapy that comprises upper limb (UL) and cognitive training within the context of meaningful physical activity. The proposed s

The Association of Aortic Stenosis Severity and Symptom Status With Morbidity and Mortality.

BACKGROUND: The relationship between aortic stenosis (AS) severity, AS-related symptoms, and clinical outcomes is poorly understood. OBJECTIVES: The purpose of this study was to evaluate whether potential AS-related symptoms at diagnosis are associated with outcomes, including death and AS-related hospitalization. METHODS: In this retrospective cohort study from a large, integrated health care system serving >4.5 M individuals, we applied validated natural language processing algorithms to echocardiogram reports to identify physician-assessed AS severity and potential AS-related symptoms (eg

Co-design and implementation of nursing handover improvement tool in Kenyan newborn units: a pilot study.

BACKGROUND: Transfer of care and responsibility for patients from one healthcare provider to another during hospitalisation is critical to them receiving the right interventions and overall quality and safety of care, and is dependent on the accurate transfer of patient information. Sick newborns are particularly dependent on health workers being able to meet their needs, but previous work suggests that nursing handovers in newborn units (NBUs) may be poor in Kenya. Initiatives aimed at improving nursing handovers have mainly been tested and implemented in High-Income Countries (HICs), with ve

Social support and the cancer patient. Conceptual and methodologic issues.

In this paper, an attempt has been made to provide an overview of the major issues facing cancer researchers who are interested in the conceptualization and measurement of social support. Taken as a whole, the literature reflects a growing appreciation for the complexity of the social-support construct, and a need for greater conceptual and research specificity. The following issues were discussed: (1) the importance of measuring support in a way that will permit assessment of distinct types of support (e.g., emotional support, advice) by distinct providers (e.g., spouse, physician) since avai

Maternal health planning and prioritization in Chad: developing a supportive tool.

The Republic of Chad has one of the highest rates of maternal mortality in the world. With scarce resources to respond to competing demands, pragmatic evidence-based planning tools are needed to aid planning and support priority setting. This action research aimed to develop a tool to support maternal health (MH) planning and prioritization decisions and identify priority regions/provinces for intervention in Chad based on aggregate MH coverage gap scores (Target-Coverage = Coverage Gap). A rapid review was conducted to identify key indicators and relevant national targets. The 2019 Multiple I

Analyzing electronic medical records to extract prepregnancy morbidities and pregnancy complications: Toward a learning health system.

INTRODUCTION: Preexisting and pregnancy-related medical conditions frequently co-occur, leading to multimorbidity (≥2 morbidities) in pregnant women, and much of this information is in semi-structured format in electronic medical records (EMRs). The aim was to advance the learning health system as a platform for automating information extraction from EMRs and to uncover the prevalence of common morbidities during pregnancy and their association with pregnancy-related complications. METHODS: This study included 48 502 pregnant women attending Monash Health maternity hospitals from 2016 to 2021

Acute and long-term mental and physical sequelae in the aftermath of traumatic exposure - some remarks on "the body keeps the score".

Traumata, by definition, refer to exterior events that expose a person to experiences of overwhelming threat and catastrophe and elicit feelings of death anxiety, panic, horror, helplessness, loss of personal control, and intractability. Most affected persons respond with at least some distressing symptoms of trauma-related memory intrusions, autonomic hyperarousal, dissociation, and depression in the acute aftermath. Fortunately, the majority of traumatized individuals succeed in coping with this major stress quite well during the following weeks and months unless the process of recovery is h

Emotion regulation and social information processing as targets of a guided selective prevention app for emerging adults with a history of adverse childhood experiences: A randomised controlled trial.

BACKGROUND: Adverse childhood experiences (ACEs) are associated with increased risk for psychopathology and reduced psychosocial functioning across the life course. However, digital selective prevention targeting individuals with a history of ACEs remains scarce. Impairments in emotion regulation and social information processing were confirmed as mechanisms linking ACEs with mental health and potential targets for interventions. This randomised controlled trial evaluated the uptake, efficacy, and differential component effects of the guided FACE app, a theory-based preventive intervention for

Improving hospital-based opioid substitution therapy (iHOST): protocol for a mixed-methods evaluation.

BACKGROUND: Opioid substitution therapy (also known as 'opioid agonist therapy' or 'medication treatment of opioid use disorder') is associated with improved health and social outcomes for people who use heroin and other illicit opioids. It is typically managed in the community and is not always continued when people are admitted to hospital. This causes opioid withdrawal, patient-directed discharge, and increased costs. We are establishing a project called iHOST (improving hospital opioid substitution therapy) to address these problems. This is an applied health research project in which we w
