Pediatric Simulation-Based Prehospital Training Course in Botswana.

AUDIENCE: This simulation-based training focuses on the most common and high risk pediatric prehospital scenarios in low- and middle-income countries (LMIC). The curriculum was developed based on a needs assessment to train Ministry of Health and Wellness (MOHW) prehospital providers in Botswana specifically for pediatric resuscitation and could be used for emergency medical services (EMS) providers in other LMIC. After participating in this curriculum, providers should enhance their assessment and interventions in acutely ill pediatric prehospital patients. LENGTH OF CURRICULUM: The entire c…

Fatal Haemophilus influenzae type a sepsis in an infant.

Haemophilus influenzae type a can cause severe sepsis, as demonstrated by the case described. Epidemiology of sepsis in childhood is changing. Regardless of the pathogen involved, management of children with septic shock involves resuscitative measures and empiric antibiotics. The following case of H. influenzae type a sepsis proved fatal in spite of appropriate therapy.

Rigid Esophagoscopy-Induced Aortic Rupture During Oral Gastric Tube Placement in Patient With Esophageal Stricture: A Case Report.

This is the first account of significant aortic injury during diagnostic rigid esophagoscopy in an adult with an esophageal stricture. We describe the resultant hemothorax and hemodynamic collapse and the successful treatment with massive volume resuscitation, vasopressors, and timely surgical intervention including thoracic endovascular aortic repair. We discuss the importance of rapid diagnosis, relevant anatomy, treatment modalities, and communication as cornerstones for learning.

Successful Use of Ketamine as a Prehospital Analgesic by Pararescuemen During Operation Enduring Freedom.

Effective analgesia is a crucial part of the care and resuscitation of a traumatically injured patient. These secondary effects of pain may increase morbidity and mortality in the acutely injured patient. When ketamine is administered appropriately in the clinical setting, it can provide analgesia, anxiolysis, and amnesia for patients with less respiratory depression and hypotension than equivalent doses of opioid analgesics.

Contemporary management of pyloric stenosis.

Hypertrophic pyloric stenosis is a common surgical cause of vomiting in infants. Following appropriate fluid resuscitation, the mainstay of treatment is pyloromyotomy. This article reviews the aetiology and pathophysiology of hypertrophic pyloric stenosis, its clinical presentation, the role of imaging, the preoperative and postoperative management, current surgical approaches and non-surgical treatment options. Contemporary postoperative feeding regimens, outcomes and complications are also discussed.

[Fast treatment of septic shock saves lives. Although "early goal-directed therapy"-protocol does not show any effect in new studies].

Three recent randomised trials have shown a lack of effect of EGDT for the treatment of severe sepsis and septic shock. Notably, control group care was excellent in all trials, including early recognition, early and aggressive fluid resuscitation and prompt administration of antibiotics, and was reflected in a lower than expected mortality. The discussion emphasizes the need for continued vigilance and early aggressive management.

[Sudden cardiac arrest as the first symptom of coronary heart disease--case report].

We describe the patient at whom sudden cardiac arrest was the first symptom of coronary heart disease. In the paper, we describe factors which influence the effectiveness of resuscitation in out of hospital cardiac arrest. Applying the cardiac massage by a bystander, the presence of defibrillation rhythm and short period of time to the first defibrillation, determined the survival of the patient.

Sigmoid volvulus complicating pregnancy.

Sigmoid volvulus complicating pregnancy is an extremely rare complication with fewer than 76 cases reported in literature. We report a case of sigmoid volvulus complicating pregnancy. The sigmoid colon was resected and Hartman's colostomy was performed. The patient had a successful recovery. Aggressive resuscitation followed by early surgical intervention should be undertaken to reduce maternal and fetal morbidity and mortality.

Peptic ulcer perforation: management of high-risk cases by percutaneous abdominal drainage.

Sixty-six patients were selected as high-risk cases of duodenal ulcer perforation. After resuscitation with intravenous fluids and nasogastric suction, a widebore percutaneous intra-abdominal drain was put in under local anaesthesia. There were three (4.5%) deaths; 58 (87.8%) patients improved satisfactorily. High-risk peptic ulcer perforation patients can be managed by putting in an intra-abdominal drain supported by conservative treatment.

Embolism during pregnancy: thrombus, air, and amniotic fluid.

Pulmonary embolism is the primary cause of acute respiratory decompensation during pregnancy. Regardless of the nature of the embolism, a high index of suspicion, early diagnosis, and aggressive resuscitation need to be instituted to achieve a successful maternal and fetal outcome. Several clinical characteristics will assist practitioners to distinguish among the different forms of embolism and to institute specific measures of treatment.

Behavioral reactions of rats in the early postresuscitation period after systemic circulatory arrest of different duration.

Rats were exposed to systemic circulatory arrest for 10, 12, and 15 min. During the first 10 days after resuscitation exploratory activity of animals depended on the duration of systemic circulatory arrest and was determined by two opposite factors: high reactivity of the central nervous system contributing to intensification of exploratory activity, and impaired general state of survivors (inhibition of animal behavior).

Emergency ligation of anomalous left coronary artery arising from the pulmonary artery.

We report two cases of successful emergency ligation of anomalous left coronary artery arising from the pulmonary artery (ALCAPA) in patients with previous cardiac arrest. Both patients had regained marginal cardiac output after cardiopulmonary resuscitation and had maximal doses of inotropic support. The ALCAPA ligation was then performed as a life-saving procedure in the absence of any kind of mechanical circulatory support.

Complete fusion of the vocal cords; an unusual case.

A case is presented of a woman who sustained a 35% body surface area mixed depth cutaneous burn, together with a significant inhalational injury. The patient required emergency resuscitation with endotracheal intubation and subsequently tracheostomy. This resulted in an unfortunate complication of a total adhesion between the vocal cords which extended into the subglottic area, causing complete occlusion of the airway.

Premature "closing" of the foramen ovale in transposition of the great arteries with intact ventricular septum: rare cause of sudden neonatal death.

We report a male neonate who developed severe cyanosis and bradycardia at birth unresponsive to resuscitation. At autopsy he was found to have premature "closure" of the foramen ovale together with transposition of the great arteries and an intact ventricular septum. Reviewing the literature, we found only one case report describing a similar neonate with this lethal combination of cardiac malformations.
