Ruptured true aneurysm of the splenic artery: an unusual cause of haemoperitoneum.

True aneurysm of the splenic artery is rare. Two cases of ruptured true splenic artery aneurysms are presented. The first patient was a 62-year-old female who presented within 6 hours of the onset of symptoms. The other was a 27-year-old non-alcoholic male patient who was admitted in a state of shock after 2 days of observation in a peripheral hospital. Both patients had haemoperitoneum and were subjected to exploratory laparotomy. Aneurysmectomy was performed in both the patients in addition to left splenopancreatectomy in the first case and splenectomy in the second. However, due to the prol…

[Ruptured aneurysm of the pancreaticoduodenal artery. A rare etiology of acute abdomen].

Even among the uncommon aneurysms of the visceral arteries the aneurysm of the pancreaticoduodenalis artery is considered a rarity. Etiologically, numerous factors must be taken into account, the most significant one being arteriosclerosis. The clinical presentation is unspecific and ambiguous. CT and, above all, intra-arterial DSA allow for a diagnosis. A generous consideration of indicating operative intervention, even in asymptomatic patients, is especially justified because of the imminent risk of rupture. The preferable therapy consists of elimination of the aneurysm either conventionally…

Salvage of the ruptured spleen in an infant with very low birth weight.

Hemorrhage from the traumatized spleen needs surgical intervention if the patient's cardiovascular status is unstable and not stabilizing with medical support. It is well recognized that salvage of the injured spleen is preferable to splenectomy because of risks of overwhelming postsplenectomy infection (OPSI). The authors report the case of a 746-g newborn with splenic hemorrhage that required surgical intervention to control bleeding. A 2-component fibrin tissue adhesive and a cellulose fabric were used.

Perinatally ruptured dermoid cyst presenting as congenital oculomotor palsy.

An eight-month-old girl presented with congenital exotropia and latent nystagmus. Further evaluation revealed congenital ptosis of the left … wed fibrous tissue containing a hair. Based on the clinical history, the diagnosis of a perinatally ruptured orbital dermoid cyst was made. Review of the previous CT and an additional CT showed enlargement of the left lateral orbital wall with a notch in the lateral wall, indicative of a dermoid cyst.

Ruptured mitral valve aneurysm in a patient with quadricuspid aortic valve.

Mitral aneurysm is commonly associated with infectious endocarditis of the aortic valve. In rare instances, it is associated with other underlying inflammatory or metabolic disorders. A 62-year-old man with mitral valve insufficiency with moderate aortic valve insufficiency underwent operation. Operative findings were the ruptured aneurysm of the mitral valve in association with the calcified quadricuspid aortic valve. There was no evidence of infection in the mitral valve and the aortic valve. The mitral valve was repaired, the aortic valve replaced, and Cox Maze procedure added. A rare combi…

Delayed repair of a ruptured pectoralis major muscle. A case report.

Repair of a pectoralis major muscle rupture at the tendinous insertion into the humerus was successfully performed 13 years after the initial injury. Repair was possible only because the ruptured sternal portion of the muscle was scarred to the intact clavicular portion and therefore had not retracted. The attachment of the avulsed sternal head to the intact clavicular head enabled successful restoration of strength and function, as well as normal contour and appearance of the pectoralis major muscle complex.

Sudden death in an infant caused by rupture of a basilar artery aneurysm.

Ruptured aneurysms of the cerebrovasculature in infancy and early childhood, except for "giant" aneurysms and arteriovenous malformations, are rare. Seizures, loss of consciousness, and apnea are the usual presenting signs in infancy; symptoms such as headache or visual disturbances and signs such as cranial nerve compression or meningeal irritation commonly found in older children or adults are absent in infants. However, the morphologic findings (i.e., subarachnoid and retinal hemorrhage, and occasionally subdural hemorrhage) may be mistaken for inflicted trauma, especially if the aneurysm i…

Special cerebral perfusion in surgery for the ruptured thoracic aortic aneurysm.

BACKGROUND: For surgical treatment of the ruptured thoracic aortic aneurysm (TAA), it is important to control bleeding and to protect the brain, spinal cord, and myocardium. We have developed and performed a new procedure on 6 patients with a ruptured TAA, a true aneurysm in 3 patients and a type A dissection in the other 3. METHOD: Cardiopulmonary bypass is installed with cannulations to the iliac artery and vein and to the common carotid arteries on both sides of the neck before the sternum is divided. For control of bleeding, venous drainage is accelerated, whereas cerebral perfusion is ma…

[Peritonitis caused by spontaneous rupture of pyonephrosis in pregnancy. Report of a case].

Peritonitis after spontaneous rupture of pyonephrosis into the peritoneal cavity is a rare complication, usually diagnosed intraoperatively. We report a case of a woman presenting with left lumbar pain and fever during pregnancy. On admission, ultrasonography showed a pregnancy with fetal activity for 16 weeks, and pyonephrosis in the left kidney, but on a normal right kidney. After antibiotic therapy and upper urinary, tract stenting renal drainage revealed purulent urine, fever persisted with acute abdomen. Clinical and radiological assessment showed features of acute peritonitis with pyonep…

[Rupture in the palm of a flexor tendon in a young man with osteogenesis imperfecta].

Flexor tendon rupture in the palm of the hand is very infrequent. It usually occurs in association with an underlying pathological condition. We report a case of hand tendon rupture in a patient with osteogenesis imperfecta (OI). This disease is typically characterised by brittle bones but other tissues rich in collagen, such as tendons, may also be involved. The ruptured tendon was diagnosed one month post-injury and was treated by tendon graft. Six months later, tenolysis was performed and one year post-injury flexion was still limited at the distal interphalangeal joint. Few cases of tendon…

[Ruptured aortic aneurysm as an unexpected finding in laparotomy for acute abdomen].

The ruptured abdominal aortic aneurysm as an incidental finding in emergency laparotomy for acute abdominal symptoms is a rare event. For this reason it is more important to know the necessary diagnostic and therapeutic strategies. Nowadays sonography facilitates the preoperative diagnosis. The performance of an additional computed tomography or angiography depends on the clinical appearance of the patient. In hemodynamically instable patients with a ruptured aneurysm, an immediate laparotomy is mandatory. If intraoperatively the aortic aneurysm has a diameter of more than 5 cm and shows no si…

Pectoralis major ruptures postsuction lipectomy for surgical management of gynecomastia.

Over the last decade, suction-assisted lipoplasty has been shown to be a safe and reliable procedure in the hands of trained individuals. A large number of plastic surgeons employ liposuction as an adjunct to surgery for gynecomastia with excellent results and low morbidity. An unusual problem, traumatic rupture of the pectoralis major muscle, has been observed by us in a patient who had undergone the procedure. The physical deficit arising from the injury may go unnoticed in all but the most physically active people, and surgeons should be wary of the possibility of this previously unreported…

Rupture of the left aortic sinus into the pulmonary artery in a Dalmation dog.

An 18-month-old male Dalmatian dog was presented for veterinary evaluation after the pet owner observed a sudden onset of weakness. An acquired continuous cardiac murmur was detected on physical examination. Two-dimensional echocardiographic examination revealed structural abnormalities of the proximal aorta and pulmonic valve. Doppler echocardiographic studies and cardiac catheterization demonstrated the presence of a proximal aorticopulmonary shunt. A diagnosis of left aortic sinus rupture and aorticopulmonary fistula was made. Antemortem diagnosis of aortic sinus rupture in the dog has not,…

[A case of spontaneous repair of the ureteral rupture caused by ureteral carcinoma].

We report a 47-year-old male patient with a spontaneous ureteral rupture caused by ureteral carcinoma. Drip infusion pyelography (DIP) and computerized tomography (CT) demonstrated the extravasation of contrast medium around the dilated upper ureter. Nephrostography showed the extinction of extravasation 4 days after the DIP. A total nephroureterectomy with the excision of a cuff of bladder was performed for the transitional cell carcinoma of the right lower ureter. Meticulous pathological examination of the specimen revealed discontinuity of the muscle layer, edema and fibrin deposition in th…

Ruptured dissecting aneurysm as a cause of subarachnoid hemorrhage of unverified etiology.

BACKGROUND AND PURPOSE: The clinical features of "aneurysmal" subarachnoid hemorrhage (SAH) of angiographically unverified etiology were rev … y, the lesions were located on the anterior circulation in 6 and on the posterior circulation in 4. Ruptured dissecting aneurysms were confirmed by exploratory surgery or autopsy in 6 patients. Subsequent rupture occurred in 6 of the 10 patients (60%), and all 6 of these patients died as a result. CONCLUSIONS: The incidence (6/30) of dissecting aneurysms …
