Protocol for an 'efficient design' cluster randomised controlled trial to evaluate a complex intervention to improve antibiotic prescribing for CHIldren presenting to primary care with acute COugh and respiratory tract infection: the CHICO study.

INTRODUCTION: Respiratory tract infections (RTIs) in children are common and present major resource implications for primary care. Unnecessary use of antibiotics is associated with the development and proliferation of antimicrobial resistance. In 2016, the National Institute for Health Research (NIHR)-funded 'TARGET' programme developed a prognostic algorithm to identify children with acute cough and RTI at very low risk of 30-day hospitalisation and unlikely to need antibiotics. The intervention includes: (1) explicit elicitation of parental concerns, (2) the results of the prognostic algorit

Effect of whole-body vibration exercise in preventing falls and fractures: a systematic review and meta-analysis.

OBJECTIVE: To investigate the effect of whole-body vibration exercise (WBV) on fracture risk in adults ≥50 years of age. DESIGN: A systematic review and meta-analysis calculating relative risk ratios, fall rate ratio and absolute weighted mean difference using random effects models. Heterogeneity was estimated using I2 statistics, and the Cochrane Collaboration's risk of bias tool and the GRADE approach were used to evaluate quality of evidence and summarise conclusions. DATA SOURCES: The databases PubMed, Embase and the Cochrane Central Register from inception to April 2016 and reference li

Early, Goal-Directed Therapy for Septic Shock - A Patient-Level Meta-Analysis.

BACKGROUND: After a single-center trial and observational studies suggesting that early, goal-directed therapy (EGDT) reduced mortality from septic shock, three multicenter trials (ProCESS, ARISE, and ProMISe) showed no benefit. This meta-analysis of individual patient data from the three recent trials was designed prospectively to improve statistical power and explore heterogeneity of treatment effect of EGDT. METHODS: We harmonized entry criteria, intervention protocols, outcomes, resource-use measures, and data collection across the trials and specified all analyses before unblinding. Afte

Cost-effectiveness Analysis of Rivaroxaban in the Secondary Prevention of Acute Coronary Syndromes in Sweden.

BACKGROUND: Worldwide, coronary heart disease accounts for 7 million deaths each year. In Sweden, acute coronary syndrome (ACS) is a leading cause of hospitalization and is responsible for 1 in 4 deaths. OBJECTIVE: The aim of this analysis was to assess the cost-effectiveness of rivaroxaban 2.5 mg twice daily (BID) in combination with standard antiplatelet therapy (ST-APT) versus ST-APT alone, for the secondary prevention of ACS in adult patients with elevated cardiac biomarkers without a prior history of stroke/transient ischemic attack (TIA), from a Swedish societal perspective, based on cl

Reattribution to Mind-Brain Processes and Recovery From Chronic Back Pain: A Secondary Analysis of a Randomized Clinical Trial.

IMPORTANCE: In primary chronic back pain (CBP), the belief that pain indicates tissue damage is both inaccurate and unhelpful. Reattributing pain to mind or brain processes may support recovery. OBJECTIVES: To test whether the reattribution of pain to mind or brain processes was associated with pain relief in pain reprocessing therapy (PRT) and to validate natural language-based tools for measuring patients' symptom attributions. DESIGN, SETTING, AND PARTICIPANTS: This secondary analysis of clinical trial data analyzed natural language data from patients with primary CBP randomized to PRT, p

Use of metformin to treat pregnant women with polycystic ovary syndrome (PregMet2): a randomised, double-blind, placebo-controlled trial.

BACKGROUND: Women with polycystic ovary syndrome (PCOS) have an increased risk of pregnancy complications. Epi-analysis of two previous randomised controlled trials that compared metformin with placebo during pregnancy in women with PCOS showed a significant reduction in late miscarriages and preterm births in the metformin group. The aim of this third randomised trial (PregMet2) was to test the hypothesis that metformin prevents late miscarriage and preterm birth in women with PCOS. METHODS: PregMet2 was a randomised, placebo-controlled, double-blind, multicentre trial done at 14 hospitals i

Artificial intelligence tools for the assessment and management of dysphagia: protocol for a scoping review.

INTRODUCTION: Dysphagia, or difficulty in swallowing, significantly impacts the quality of life of the affected individuals. Diagnostic approaches, including video fluoroscopic swallowing studies and flexible endoscopic evaluation of swallowing, are the most commonly used methods for assessing swallowing function. Recent advancements have led to the development of artificial intelligence (AI), including machine learning (ML) and deep learning (DL), which will provide innovative approaches to dysphagia diagnosis and treatment planning. There is a limited synthesis of literature on AI tools in d

Development of a core outcome set for studies involving patients undergoing major lower limb amputation for peripheral arterial disease: study protocol for a systematic review and identification of a core outcome set using a Delphi survey.

BACKGROUND: The development of a standardised reporting set is important to ensure that research is directed towards the most important outcomes and that data is comparable. To ensure validity, the set must be agreed by a consensus of stakeholders including patients, healthcare professionals and lay representatives. There is currently no agreed core outcome set for patients undergoing major lower limb amputation for peripheral arterial disease (PAD) for either short- or medium-term research outcomes. By developing these sets we aim to rationalise future trial outcomes, facilitate meta-analysis

Non-Surgically Treated Distal Radius Fractures in the Adult Population: A Systematic Review and Meta-analysis.

Distal radius fracture (DRF) is the most common upper extremity fracture, and cast immobilization is the most widely used treatment for DRF. However, prospective outcome data on conservatively treated DRF remain limited. The aim of this systematic review and meta-analysis was to report the pooled patient-reported outcome measures (PROMs) in adult patients with non-surgically treated DRFs. PubMed (Medline), Web of Science, and the Cochrane Central Register of Controlled Trials databases were searched. The primary outcome was to analyze PROMs at 3-month, 6-month, and 12-month follow-ups. The sec

Walking While Acting Sad and Happy Emotions Influences Risk Factors of Knee Osteoarthritis.

Greater knee adduction moment is associated with increased risk and progression of knee osteoarthritis, and this biomechanical risk factor is modulated through kinematic gait modifications. Emotions are known to influence walking kinematics and speed, but the effect of different emotions on knee mechanics is unclear. To test this, 20 healthy participants walked while instrumented gait data was recorded. Participants initially walked naturally (baseline) and then acting 4 emotional walking conditions: Anger, Happy, Fear, and Sad, in randomized order. Statistical parametric mapping with an analy

Repetitive negative thinking and suicidal ideation in internalizing psychopathologies: A replication study.

Suicidal ideation (SI), a risk factor for suicide, is prevalent in internalizing psychopathologies, including depression and anxiety. Rumination and worry are well-studied repetitive negative thinking (RNT) constructs implicated in internalizing psychopathologies. These constructs have shared and distinct characteristics. However, the relationship between rumination and worry and their associations with SI are not fully understood in clinical samples. The present study used correlational and regression analysis to evaluate these relationships as a secondary data analysis in treatment-seeking p…

Investigating the impact of isthmocele and its surgical repair on fertility: results from a systematic review and meta-analysis.

The aim of this systematic review was to quantify the impact of isthmocele and its treatment on fertility. The review was conducted according to the PRISMA guidelines, and the meta-analysis according to the MOOSE guidelines. The meta-analysis of data from 29 studies showed that 43% (rate 0.43, 95% CI 0.33-0.54; n = 2038) of women with a symptomatic isthmocele reported secondary infertility as a presenting symptom (quality of evidence [QOE)]: very low). In the IVF setting, women with an isthmocele were shown to have a lower live birth rate (LBR) than women with a previous Caesarean section with

A hierarchical framework for distributed resilient control of islanded AC microgrids under false data injection attacks.

In this paper, we propose a control scheme to ensure the microgrid control layers are resilient to cyberattacks. The studied microgrid consists of several distributed generation (DG) units and we consider the hierarchical control structure that is common for microgrids. The use of communication channels among DGs has made microgrids more vulnerable and this is where cybersecurity issues arise. In this work we added three algorithms, reputation-based, Weighted Mean Subsequence Reduced (W-MSR) and Resilient Consensus Algorithm with Trusted Nodes (RCA-T), to the secondary control layer of the mic
