Ergonomics in gynecologic surgery.

PURPOSE OF REVIEW: Work-related musculoskeletal disorders (WMSDs) are prevalent among surgeons and result in significant disability. We aimed to review the English-language literature regarding ergonomic risk, prevalence of WMSDs, and unique ergonomic considerations of gynecologic surgery. RECENT FINDINGS: Surgeon WMSDs are prevalent, with rates ranging from 66 to 94% for open surgery, 73-100% for conventional laparoscopy, 54-87% for vaginal surgery, and 23-80% for robotic-assisted surgery. Risk factors for injury in open surgery include use of loupes, headlamps, and microscopes. Unique risks

Hip rotational velocities during the full golf swing.

Since labral pathology in professional golfers has been reported, and such pathology has been associated with internal/external hip rotation, quantifying the rotational velocity of the hips during the golf swing may be helpful in understanding the mechanism involved in labral injury. Thus, the purpose of this study was to determine the peak internal/external rotational velocities of the thigh relative to the pelvis during the golf swing. Fifteen female, collegiate golfers participated in the study. Data were acquired through high-speed three dimensional (3-D) videography using a multi-segment

Treatment of posterior sternoclavicular dislocation with locking plate osteosynthesis.

Posterior dislocations of the sternoclavicular joint are rare injuries, representing approximately 3% of all major insults to the shoulder girdle. Despite their relative infrequency, they must be diagnosed and treated early because of the proximity of the sternoclavicular joint to sensitive anatomic structures in the superior mediastinum. Physical examination can yield limited findings, and a high index of suspicion is warranted for this injury pattern especially with impingement signs and symptoms: dyspnea, dysphagia, dysphonia, brachial plexus injury, and vascular alterations. Evaluation wit

Risk Factors for Infection After Rotator Cuff Repair.

PURPOSE: To identify risk factors for infection after rotator cuff repair. We hypothesized that patient characteristics and surgical technique would affect the rate of infection. METHODS: The records of 1,824 rotator cuff repairs performed by a single surgeon from 1995 to 2010 were reviewed retrospectively. Fourteen patients had an early deep postoperative wound infection that was treated with surgical irrigation and debridement. One hundred eighty-five control patients who were treated with rotator cuff repair and did not develop an infection were selected randomly for comparison and statist

Clinical significance of the arthroscopic drive-through sign in shoulder surgery.

PURPOSE: During arthroscopy of the shoulder, the ability to pass the arthroscope easily between the humeral head and the glenoid at the level of the anterior band of the inferior glenohumeral ligament is considered a positive drive-through sign. The drive-through sign has been considered diagnostic of shoulder instability and has been associated with shoulder laxity and with SLAP lesions. The goal of this study was to examine the prevalence of the drive-through sign in patients undergoing shoulder arthroscopy and to determine its relationship to shoulder instability, shoulder laxity, and to SL

In Vivo Response to Electrocompacted Tendon-Derived Thread Scaffolds During Tendon Repair.

Tendon injuries remain a major clinical challenge due to the tissue's limited regenerative capacity and poor remodeling of current allograft-based therapies. To address these limitations, we developed a tendon-derived thread (TDT) scaffold by electrocompacting decellularized tendon powder into aligned threads, which were subsequently woven into a porous overlay patch. Biochemical analyses of TDT confirmed efficient decellularization (DNA < 50 ng/mg) and the preservation of collagen and proteoglycans. Compared to en bloc human tendon graft (HTG), the TDT scaffold exhibited significantly higher

Scald Burns From Hair Braiding.

Only one previous case report has described scald burns secondary to hair braiding in pediatric patients. The present case study is the largest to date of scald burns as a result of hair braiding in children and adults. Charts of all 1609 female patients seen at a single burn center from 2008 to 2014 were retrospectively reviewed to identify patients with scald burns attributed to hair braiding. Demographics, injury severity, injury patterns, and complications were analyzed. Twenty-six patients (1.6%) had scald burns secondary to hair braiding with median TBSA 3%. Eighty-five percent of patien

Motor impairment factors related to brain injury timing in early hemiparesis. Part I: expression of upper-extremity weakness.

BACKGROUND: Extensive neuromotor development occurs early in human life, but the time that a brain injury occurs during development has not been rigorously studied when quantifying motor impairments. OBJECTIVE: This study investigated the impact of timing of brain injury on the magnitude and distribution of weakness in the paretic arm of individuals with childhood-onset hemiparesis. METHODS: A total of 24 individuals with hemiparesis were divided into time periods of injury before birth (PRE-natal, n = 8), around the time of birth (PERI-natal, n = 8), or after 6 months of age (POST-natal, n

Does hypoxia-reperfusion injury occur in osteoarthritis of the temporomandibular joint?

AIMS: To determine the available evidence in the literature for whether hypoxia-reperfusion injury plays a role in the pathogenesis of joint diseases in general and of osteoarthritis (OA) of the temporomandibular joint (TMJ) in particular. METHODS: The electronic databases CENTRAL, PubMed, and EMBASE were systematically searched. The search strategy combined thesaurus terms "reperfusion injury" and "joints" and excluded "tourniquet," which possibly induces iatrogenic reperfusion injury. Inclusion and exclusion criteria were applied, data were extracted, and quality was assessed. RESULTS: Fou

Neuromuscular adaptations in shoulder function and dysfunction.

Neuromuscular activity, organized in coordinated patterns, forms the basis of task-specific function in sports and exercise. The content and extent of these patterns may be variable, but include elements of activation/inhibition, co-activation, concentric/eccentric activation, proximal-to-distal activation, plyometric activation, and preactivation stiffness. They may be based on inherent neuromuscular architecture, but are commonly affected by positive or negative adaptations to imposed functional demands. Positive neuromuscular adaptations improve the efficiency of performing the task, which

Influence of body mass index on severity of rotator cuff tears.

BACKGROUND: Research on the relationship between obesity and rotator cuff tears (RCTs) has been limited to the impact of obesity on the results of arthroscopic repair of RCTs; thus, a need for rigorous research controlling for other factors affecting RCTs is warranted, especially to better understand the impact of body mass index (BMI) on RCT severity. METHODS: A retrospective study of admission records contained in electronic medical records pertaining to patients who were admitted for RCT repair on 1 shoulder between January 2018 and July 2022 was conducted. In total, 386 patients were incl

Upper limb kinematics after cervical spinal cord injury: a review.

Although a number of upper limb kinematic studies have been conducted, no review actually addresses the key-features of open-chain upper limb movements after cervical spinal cord injury (SCI). The aim of this literature review is to provide a clear understanding of motor control and kinematic changes during open-chain upper limb reaching, reach-to-grasp, overhead movements, and fast elbow flexion movements after tetraplegia. Using data from MEDLINE between 1966 and December 2014, we examined temporal and spatial kinematic measures and when available electromyographic recordings. We included fi
