PURPOSE: To validate the glenoid track concept in a cadaveric bipolar bone loss model and to test whether "on-track" and "off-track" lesions can be stabilized with Bankart repair (BR) with or without Hill-Sachs remplissage (HSR).
METHODS: Eight fresh-frozen cadaveric shoulders were tested in a custom apparatus with passive axial rotation and then progressive translational loading (10 to 40 N) at mid-range (60°) and end-range external rotation (90°). Injury conditions included glenoid bone loss of 15% with on-track (15%) and off-track (30%) Hill-Sachs lesions. Repair conditions included BR wit

Lipoma arborescens is a very rare intra-articular lesion characterized by villous lipomatous proliferation of the synovium, usually involving the knee joint. To date, in the literature, there has been only one reported case of lipoma arborescens in the glenohumeral joint. But, the authors treated the patient conservatively and reported only the magnetic resonance imaging (MRI) findings. We report a case of a patient with lipoma arborescens involving the glenohumeral joint. To our knowledge, this is the first reported case of lipoma arborescens involving the glenohumeral joint managed by arthro

Currently, there is little information on the outcome of humeral head replacement for steroid-associated osteonecrosis of the humeral head. The purpose of this study was to evaluate the outcome of patients who underwent humeral head replacement for steroid-associated osteonecrosis to determine the results, risk factors for an unsatisfactory outcome, and rates of revision surgery. Between 1980 and 2000, 32 shoulder hemiarthroplasties were performed for steroid-associated osteonecrosis. We included 31 hemiarthroplasties in 25 patients with a minimum 2-year follow-up (mean, 12.0 years) in the stu

Patients with rotator cuff tear arthropathy can be treated successfully with a reverse total shoulder prosthesis. In patients with significant glenoid bone loss, achieving stable bone fixation can be challenging, as the surgeon must know when bone grafting is necessary and when the plan to implant the reverse total shoulder prosthesis should be abandoned because of the likelihood of early implant loosening. The purposes of this study were (1) to determine the initial stability of a metal glenoid implant fixed in a glenoid with a central cavitary defect and (2) to determine whether an altered s

Recurrent instability in patients over forty years of age is felt to occur primarily as a result of an associated rotator cuff tear. This is often referred to as the "posterior mechanism." We reviewed our patients over the age of forty who underwent an anterior shoulder repair to identify the incidence of capsulolabral detachments and the role of an "anterior mechanism" in this patient population. A retrospective review of all patients from 1985 to 2000 was performed to identify patients who had surgery for recurrent instability that began after forty years of age. Of the 265 patients records

The purpose of this study was to evaluate 4 cases in which axillary nerve injury was observed after radiofrequency capsular shrinkage of the shoulder. These cases were used to evaluate the clinical circumstances under which axillary nerve injury occurred, the time frame over which these injuries recovered, and the ultimate outcome observed in these patients. The case histories of each of these 4 patients were carefully evaluated. In 2 of the 4 cases, purely sensory findings were found, and in 2 cases both sensory and motor injuries occurred. In each case the inferior axillary recess was treate

PURPOSE: The purpose of this study was to determine the temperatures along the course of the axillary nerve during radiofrequency (RF) capsulorrhaphy of the shoulder in a cadaver model. The hypothesis of this project was that temperatures capable of damaging neural tissue would be recorded during simulated RF capsulorrhaphy.
TYPE OF STUDY: Basic science evaluation.
METHODS: This study used 9 fresh-frozen cadaver shoulder capsules removed en bloc along with the underlying axillary nerve and soft tissue. The axillary nerve was instrumented with 4 thermocouples placed along the course of the ne

Were reviewed 200 fractures of humeral diaphysis, two cases have been excluded because they were incomplete; 175 patients out of 200 have been reexamined a long time after; 23 have not been reexamined but held a sufficient follow up. Most of the transversal fracture were met among young adults (56 per 100 traffic accidents). The intraosseous vascularisation was studied elucidating partly the greatest frequency of pseudarthrosis in the part 1/3 middle-1/3 inferior especially after exaggerated periostal elevation. Among 107 cases treated by hanging cast were noted 4 pseudarthrosis, 12 shoulder's

PURPOSE: The goal of this anatomical clinical study is to clarify whether a simple pisiformectomy can provide sufficient decompression of the ulnar nerve along its entire course within the ulnar canal.
METHODS: A simple pisiformectomy was performed in 20 cadaveric hands, followed by complete release of the ulnar nerve. The anatomical findings were subsequently correlated with clinical outcomes in 17 patients presenting with ulnar nerve irritation and pisotriquetral joint pain.
RESULTS: Our anatomical results demonstrated that additional division of fibrous structures was not necessary in any

Rheumatoid arthritis (RA) is a chronic inflammatory disease characterized by synovial hyperplasia and joint destruction. Synovectomy and synovial biopsies are valuable procedures for diagnostic and research purposes. Synovectomy may also provide symptomatic relief in selected cases when medical therapy is insufficient. Arthroscopic-guided techniques offer a minimally invasive approach with enhanced accuracy in tissue sampling. This technique provides the distinct advantage of direct visualization, allowing for targeted biopsy of specific areas within the joint and the collection of tissue, inc

Distal clavicle insufficiency, or a loss of the bony structure of the distal clavicle, generally after an injury or surgical procedure, may result in shoulder pain, dysfunction, scapular dyskinesia, and chronic acromioclavicular joint instability. If more than 15 to 20 mm of the distal clavicle is missing, chronic posterior instability of the distal clavicle may ensue and impinge on the deltotrapezial fascia and scapular spine. Although soft tissue reconstruction of the acromioclavicular-coracoclavicular (AC-CC) ligaments is a reliable procedure, if there is 15 to 20 mm or more of distal clavi

Textile sensors transform our everyday clothing into a means to track movement and biosignals in a completely unobtrusive way. One major hindrance to the adoption of "smart" clothing is the difficulty encountered with connections and space when scaling up the number of sensors. There is a lack of research addressing a key limitation in wearable electronics: Connections between rigid and textile elements are often unreliable, and they require interfacing sensors in a way incompatible with textile mass production methods. We introduce a prototype garment, compact readout circuit, and algorithm t

UNLABELLED: This study assessed the outcome of open reduction and internal fixation of proximal interphalangeal joint fracture-dislocations through a shotgun approach, while keeping hemi-hamate arthroplasty as a back-up plan. After using the shotgun approach, fixation was carried out when the anterior fragment was large enough to accept two 1.5 mm screws. Sixty-three fracture-dislocations were treated in 61 patients, of whom 30 underwent internal fixation. Twenty-one of these were assessed in the final study. At a mean follow-up of 29 months, the mean range of joint motion was 80° and 102° for

Chronic acromioclavicular (AC) instability is a rare posttraumatic shoulder condition that can lead to undesirable symptoms like persistent pain, muscle fatigue, loss of strength, or even scapular dyskinesis. It is well known that in these cases the superior functional results depend on the restoration of the anatomy and stability of the AC joint in both vertical and horizontal planes. Considering the ligaments degeneration and atrophy in chronic AC joint dislocations, we present an arthroscopic-assisted reconstruction of both the coracoclavicular and AC ligaments using autograft augmentation.

Osteonecrosis of the femoral head is an entity which occurs mainly in young and active patients aged between 20 and 50. The success of hip joint preserving treatments ranges from 15% to 50% depending on the stage and amount of osteonecrotic lesion. Total hip replacement is indicated in late post-collapse hips but it has unsatisfactory survival because of the wear and osteolysis in young and active patients. Osteochondral allografts have been reported in the treatment of large articular lesions with defects in underlying bone in knee, talus and shoulder. By combining osteoconductive properties

In this retrospective study of nine dogs exhibiting bilateral medial trochlear ridge talar osteochondritis dissecans (MTRT-OCD) and unilateral hind limb lameness, we compared subchondral lesion size in limbs with visible lameness, with contralateral lesions that were not associated with any visible lameness. All MTRT-OCD lesions were imaged by radiography and computed tomography (CT). The dimensions of subchondral bone lesions were measured (length, width and depth) using CT software. Similar to a method used in humans, the estimated volume (length x width x depth) and cross sectional area (le

Most cases of Pseudomonas pyarthrosis affecting the sternoclavicular joint have been reported in immunosuppressed intravenous drug users. We report a case of Pseudomonas pyarthrosis in a man who was otherwise immunocompetent, except for his age. A 66-year-old white man presented to the clinic with a 1-month history of right-sided shoulder and arm pain associated with swelling of the upper part of the chest in the region of the right sternoclavicular joint. The chest radiograph revealed opacity in the right superior mediastinum. Computed tomography scan of the chest confirmed a mass in the righ
