Dravet syndrome: diverse seizure phenotypes with various triggers and developmental outcome- A case series from a LMIC.

Dravet syndrome (DS) is a severe childhood epilepsy characterised by drug-resistant seizures, developmental delays, and behavioural disturbances, often linked to de novo mutations in the SCN1A gene. This retrospective case series from Aga Khan University Hospital, Karachi, describes five patients with varying ages of seizure onset and difficult-to-control seizures despite conventional antiseizure medications. Seizure types included focal clonic evolving into myoclonic, atonic, and generalised clonic seizures, with fever identified as a trigger in three cases. Developmental delays were universa

Suzetrigine (a NaV1.8 inhibitor) versus placebo for acute postoperative pain: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: NaV1.8 channels, expressed in peripheral nociceptors, mediate sustained pain signaling. Their inhibition offers a potential opioid-sparing strategy for postoperative pain, although efficacy and safety remain incompletely defined. We synthesized randomized evidence for suzetrigine versus placebo in postoperative pain. METHODS: We conducted a Preferred Reporting Items for Systematic Reviews and Meta-Analyses-compliant meta-analysis of randomized controlled trials comparing suzetrigine (a NaV1.8 inhibitor) versus placebo in surgical patients. The primary outcome was 24-hour pain with

Deep learning architectures for EEG-based classification of Dravet syndrome: A comparative study of pre-trained and non-pretrained hybrid CNN-LSTM models.

OBJECTIVE: This study explores the potential of artificial intelligence (AI) using a hybrid deep learning Convolutional Neural Network-Long Short-Term Memory (CNN-LSTM) framework, for EEG-based classification and analysis of Dravet Syndrome (DS). METHOD: The study cohort comprised nine pediatric patients with DS, confirmed through either a heterozygous pathogenic mutation in the SCN1A gene or a clinical diagnosis consistent with established diagnostic criteria. In addition, EEG recordings from age-matched healthy control subjects and pediatric patients with non-Dravet epilepsy ("abnormal" EEG

Empagliflozin mitigates doxorubicin-induced cardiac dysfunction in association with preserved t-tubule integrity and improved Ca2+ handling: a structural mechanistic insight.

BACKGROUND: Clinical and experimental studies have suggested that sodium-glucose cotransporter type-2 inhibitors (SGLT2i) may exert a preventive effect against doxorubicin (Dox)-induced heart failure; however, the underlying mechanisms remain incompletely understood. This study aimed to investigate whether empagliflozin (Emp) protects against Dox-induced cardiotoxicity by preserving transverse tubule (t-tubule) structure and improving Ca2+ handling. METHODS: Adult male C57BL/6 mice were randomly assigned to four groups: Control, Emp-only, Dox-only, and Dox+ Emp. Emp was administered to the Em

Design and structural optimization of lappaconitine derivatives as Nav1.7 inhibitors with improved analgesic efficacy and reduced lethality in mice.

Lappaconitine (LA), a diterpenoid alkaloid with analgesic properties, is limited clinically by its narrow safety margin. To overcome this, we synthesized novel LA derivatives modified at the 20-N position to enhance Nav1.7 potency and reduce lethality. Systematic SAR studies identified representative compound 39 for further characterization. Compound 39 inhibited human Nav1.7 (IC50 = 2.87 ± 0.19 μM), exhibiting an 8.6-fold increase in potency over LA (IC50 = 24.71 ± 1.64 μM). This translated to superior suppression of sodium currents and action potential firing in DRG neurons. In a mouse posto

Development of a preclinical testing platform for clinically relevant therapy for Dravet syndrome.

OBJECTIVE: Patients with drug-resistant epilepsy, including Dravet syndrome, are frequently prescribed multiple antiseizure medications. Nevertheless, people with Dravet syndrome often have inadequate seizure control, and there is an ongoing unmet clinical need to identify novel therapeutics. As a proof-of-principle study to further validate and characterize the Scn1aA1783V/WT mouse model and identify a drug-testing paradigm with face, construct, and predictive validity, we assessed the efficacy of subchronic administration of stiripentol add-on to clobazam and valproic acid at clinically rele

NHE3 inhibitor tenapanor maintains intestinal barrier function, decreases visceral hypersensitivity, and attenuates TRPV1 signaling in colonic sensory neurons.

The pathogenesis of irritable bowel syndrome (IBS) is multifactorial, characterized in part by increased intestinal permeability, and visceral hypersensitivity. Increased permeability is associated with IBS severity and abdominal pain. Tenapanor is FDA-approved for the treatment of IBS with constipation (IBS-C) and has demonstrated improvements in bowel motility and a reduction in IBS-related pain; however, the mechanism by which tenapanor mediates these functions remains unclear. Here, the effects of tenapanor on colonic pain signaling and intestinal permeability were assessed through behavio

A novel mutation in the SCN9A gene associated with congenital insensitivity to pain, anhidrosis, and mild cognitive impairment.

Congenital insensitivity to pain (CIP) is a rare phenotype characterized by the inability to perceive pain stimuli with subsequent self-injuries, whereas CIP associated with anhidrosis (CIPA) is an overlapping phenotype mainly characterized by insensitivity to noxious stimuli and anhidrosis. CIP is primarily associated with pathogenetic variants in the SCN9A gene while CIPA is associated with pathogenetic variants in NGF and NRTK genes. However, in recent years, a significant overlap between these two disorders has been observed highlighting the presence of anhidrosis in SCN9A variants. We rep

Physiological Roles of the Rapidly Activated Delayed Rectifier K+ Current in Adult Mouse Heart Primary Pacemaker Activity.

Robust, spontaneous pacemaker activity originating in the sinoatrial node (SAN) of the heart is essential for cardiovascular function. Anatomical, electrophysiological, and molecular methods as well as mathematical modeling approaches have quite thoroughly characterized the transmembrane fluxes of Na+, K+ and Ca2+ that produce SAN action potentials (AP) and 'pacemaker depolarizations' in a number of different in vitro adult mammalian heart preparations. Possible ionic mechanisms that are responsible for SAN primary pacemaker activity are described in terms of: (i) a Ca2+-regulated mechanism ba

Age-dependent epileptic encephalopathy associated with an unusual co-occurrence of ZEB2 and SCN1A variants.

Mowat-Wilson syndrome is a genetic disorder associated with a variable phenotype including peculiar facial features associated with intellectual disability, epilepsy, language impairment, and multiple congenital anomalies caused by heterozygous mutation of the ZEB2 gene. The ZEB2 protein is a complex transcription factor that encompasses multiple functional domains that interact with the regulatory regions of target genes including those involved in brain development. Recently, it has been documented that ZEB2 regulates the differentiation of interneuron progenitors migrating from the medial g

Sleep-related hypermotor epilepsy (SHE): Contribution of known genes in 103 patients.

PURPOSE: Genetics of Sleep-related Hypermotor Epilepsy (SHE) includes mutations in several genes that cumulatively account for 30 % of families. This approximate estimate comes from different case-series, each focused on the screening of a single gene. We systematically investigated a large cohort of SHE patients to estimate the frequency of pathogenic variants in the main genes thus far implicated in this epilepsy syndrome. METHODS: We selected familial and isolated cases diagnosed with clinical/confirmed SHE who underwent genetic analysis by comparable next generation sequencing (NGS) techn

Graded Transmission without Action Potentials Sustains Rhythmic Activity in Some But Not All Modulators That Activate the Same Current.

Neurons in the central pattern-generating circuits in the crustacean stomatogastric ganglion (STG) release neurotransmitter both as a graded function of presynaptic membrane potential that persists in TTX and in response to action potentials. In the STG of the male crab Cancer borealis, the modulators oxotremorine, C. borealis tachykinin-related peptide Ia (CabTRP1a), red pigment concentrating hormone (RPCH), proctolin, TNRNFLRFamide, and crustacean cardioactive peptide (CCAP) produce and sustain robust pyloric rhythms by activating the same modulatory current (IMI), albeit on different subset

De novo mutations in SCN1A are associated with classic Rett syndrome: a case report.

BACKGROUND: Rett syndrome (RTT) is a neurodevelopmental disorder. In more than 95% of females with classic RTT a pathogenic mutation in MECP2 has been identified. This leaves a small fraction of classic cases with other genetic causes. So far, there has not been reported any other gene that may account for the majority of these cases. CASE PRESENTATION: We describe two females who fulfill the diagnostic criteria for classic RTT, with pathogenic de novo mutations in SCN1A, which usually leads to Dravet syndrome. The developmental history and clinical features of these two females fits well wit

Sympathetic mechanisms in an animal model of vasovagal syncope.

PURPOSE: Individuals predisposed to vasovagal syncope may have different autonomic nervous system control mechanisms from those without predisposition to vasovagal events. To test this hypothesis, we investigated different sympathetic responses in a canine model of vasovagal syncope. METHODS: Left thoracotomy was performed on 20 mongrel dogs. The heart was exposed and a bolus of veratridine (15 μg/kg), a neurotoxin which prevents the inactivation of sodium ion channels, was injected into the left atrium to induce a Bezold-Jarisch reflex-mediated vasovagal event, characterized by bradycardia,
