Spondylarthropathy and selective IgA deficiency.

No serum IgA was detected in a young male patient suffering from spondylarthropathy (SpA) with bilateral sacroiliitis arthritis, enthesopathy and inflammatory low back pain, whose symptoms occurred in reaction to a sexually induced urethritis. After a period of several months in which the spondylarthropathy was active, disease activity came to a rest. Three years later no progression of the SpA was observed. This finding might be an indication that IgA is not involved in the pathogenesis of spondylarthropathy.

[Hyperostotic bone changes of the spine in diabetes mellitus].

The diffuse idiopathic skeletal hyperostosis, especially with manifestation at the vertebral column, is often associated with metabolic disorders, such as diabetes mellitus or hyperuricaemia. Clinical, pathogenetic and radiological aspects are discussed. Routine thoracic x-ray give important informations on the diagnosis of this osteoplastic diathesis.

[Laser therapy of seronegative spondyloarthritis].

Multimodality therapy of 29 patients with Bekhterev's disease and Reiter's disease was performed. Of them 19 received external helium-neon laser therapy of the vertebral column and joints. A course of laser therapy consisted of 20-25 sessions. Therapeutic efficacy in this group of patients was assessed on the basis of clinicolaboratory indices and compared to the therapeutic results in the control group. A certain therapeutic effect of the use of laser therapy was obtained in patients with Bekhterev's disease and Reiter's disease proving the appropriateness and promise of further investigation

Spinal cord function in Paget's disease of spine.

The adverse influence of diminished spinal cord blood flow has been well documented. There is increased skeletal blood flow in Paget's disease of bone, with a corresponding reduction in the size of the anterior spinal artery. Use of salmon calcitonin results in rapid reduction in the bone perfusion. The recovery of cord function following calcitonin therapy in patients with paraplegia due to Paget's disease of the vertebral column is likely to be due to enhanced spinal cord perfusion.

[Discitis in small children (author's transl)].

Discitis was diagnosed in 5 children under 3 years of age, the initial clinical manifestations being difficulty in walking and abdominal pains in one case. Diagnosis was not made before periods varying from 8 days to 3 months, and no etiological basis for the disease was discovered. Pinching of the disc was always present in the first radiographic image, and the vertebral plate was ill-defined in 3 cases. Repeat radiological examinations were carried out in 3 children after 6 months, 2 and 5 years respectively. There was partial restauration of the disc space in 2 cases; the last one presented

Axial spondyloarthritis may protect against poor outcomes in COVID-19: propensity score matched analysis of 9766 patients from a nationwide multi-centric research network.

INTRODUCTION: The outcomes of COVID-19 in patients with axial spondyloarthritis (ax-SpA) have not been explored in detail. Tumour necrosis factor inhibitors (TNFi) are commonly used for ax-SpA patients, and how they influence outcomes may have implications on COVID-19 management. METHODS: A nationwide multi-centric research network was queried for patients with ax-SpA, including ankylosing spondylitis (AS) and non-radiographic SpA (nr-SpA) who had developed COVID-19. An equal number of propensity score(PS) matched controls were extracted from the database amongst patients with COVID-19 who di…

A Bizarre Bone Scan of Fibrodysplasia Ossificans Progressiva.

A 27-year-old woman had progressive bony ankylosis over both hands since she was 5 years old. Bony ankylosing over the peripheral joints and spine slowly progressed, and scoliosis surgery was performed at age 14. Recently, she developed difficulty walking, changing position, and opening her mouth. A neurologist was consulted and ruled out the suspicion of narcolepsy. Chest CT showed ankylosing spondylitis. A bone scan showed a bizarre pattern with radiouptake to the left maxilla, bilateral sacroiliac joints, left middle third femur, and soft tissue uptake over bilateral arms, legs, and pelvis.…

Etanercept treatment-related c-ANCA-associated large vessel vasculitis.

Anti-tumor necrosis factor (TNF) agents have become central players in the management of autoimmune and rheumatic disease. With the wide use of anti-TNF agents today, we have become aware of rare autoimmune complications as systemic lupus erythematosus and psoriasis, yet rarely has large vessels vasculitis been described. We herein describe a case of cytoplasmic anti-neutrophil cytoplasmic antibody (c-ANCA) (with myeloperoxidase (MPO) antibodies)-associated large vessel vasculitis (aortitis) that developed during anti-TNF treatment for ankylosing spondylitis. Awareness of this rare, but seriou…

Paraplegia after gastrectomy in a patient with cervical disc herniation: a case report and review of literature.

Paraplegia is a rare postoperative complication. We present a case of acute paraplegia after elective gastrectomy surgery because of cervical disc herniation. The 73-year-old man has the medical history of cervical spondylitis with only symptom of temporary pain in neck and shoulder. Although the patient's neck was cautiously preserved by using the Discopo, an acute paraplegia emerged at about 10 hours after the operation. Severe compression of the spinal cord by herniation of the C4-C5 cervical disc was diagnosed and emergency surgical decompression was performed immediately. Unfortunately th…
