General and substance-specific predictors of young adult nicotine dependence, alcohol use disorder, and problem behavior: replication in two samples.

BACKGROUND: This paper presents two replications of a heuristic model for measuring environment in studies of gene-environment interplay in the etiology of young adult problem behaviors. METHODS: Data were drawn from two longitudinal, U.S. studies of the etiology of substance use and related behaviors: the Raising Healthy Children study (RHC; N=1040, 47% female) and the Minnesota Twin Family Study (MTFS; N=1512, 50% female). RHC included a Pacific Northwest, school-based, community sample. MTFS included twins identified from state birth records in Minnesota. Both studies included commensurate

[Pathophysiology of radicular pain].

Pathophysiological mechanisms of low back pain and radicular pain produced by lumbar disorders are still controversial. We have made experimental animal models and have elucidated the mechanisms of hyperalgesia, which is a pain related behavior, in these animals. We have not demonstrated that only mechanical compression of the nerve roots in the lumbar spine results in hyperalgesia in rat. We found that application of the nucleus pulposus to the nerve roots produces time-dependent reversible hyperalgesia in the affected hindpaw of the rat, and that the hyperalgesia is related to bioactive subs

Antisocial personality disorder is associated with receipt of physical disability benefits in substance abuse treatment patients.

BACKGROUND: Opioid dependence is growing at an alarming rate in the United States, and opioid dependent patients have substantial medical, as well as psychiatric, conditions that impact their ability to work. This study evaluated the association between antisocial personality disorder (ASPD) and receipt of physical disability payments in methadone maintenance patients. METHODS: Using data from 115 drug and alcohol abusing methadone maintained patients participating in two clinical trials, baseline characteristics of individuals receiving (n=22) and those not receiving (n=93) physical disabili

Overview and epidemiology of substance abuse in pregnancy.

The use of licit or illicit substances during pregnancy has the potential to produce adverse health effects for the maternal-fetal dyad. In the United States, it is estimated that >4.4% of pregnant women abuse 1 or more substances during pregnancy. Social and environmental factors contribute significantly to increases in the prevalence and complexity of substance abuse disorders. Fear of reprisal prevents many women from accurately reporting substance use patterns and receiving appropriate medical and psychological care. This chapter details the epidemiology and risk factors associated with su

Predicting personality pathology among adult patients with substance use disorders: effects of childhood maltreatment.

The purpose of this study was to examine predictive relationships between types of childhood maltreatment and personality disorders in a substance-abusing population. Three hundred thirty-nine drug- or alcohol-dependent patients completed a reliable and valid retrospective measure of childhood trauma, the CTQ, and a self-report inventory that assesses the entire range of DSM-III-R personality disorders, the PDQ-R. As a preliminary step, factor analyses were used to group personality disorders into the three DSM-III-R Axis II clusters (Clusters A, B, and C), although some diagnostic subclusters

Factorial validity of a substance-use stigma scale in methamphetamine-using adults in China.

BACKGROUND: Despite the large number of individuals who use substances of abuse and the stigma of such behaviors in China, scales have not been developed and validated for assessing substance-use stigma. Given its importance for targeting interventions, the aim of the present study was to validate a Chinese substance-use stigma measure including three dimensions of substance-use-disorder-related stigma (personal stigma, perceived stigma and social distance) by modifying a pre-existing scale measuring mental illness-related stigma and social distance. METHODS: A convenience sample of eight-hun

Internet Addiction and Other Behavioral Addictions.

The Internet is increasingly influential in the lives of adolescents. Although there are many positives, there are also risks related to excessive use and addiction. It is important to recognize clinical signs and symptoms of Internet addiction (compulsive use, withdrawal, tolerance, and adverse consequences), treat comorbid conditions (other substance use disorders, attention deficit hyperactivity disorder, anxiety, depression, and hostility), and initiate psychosocial interventions. More research on this topic will help to provide consensus on diagnostic criteria and further clarify optimal

[Clinical characteristics of 5 hospitalized 3,4-methylenedioxypyrovalerone (MDPV) users].

A new illegal psychotropic substance appeared in Hungary during the first months of 2011. Acutely hospitalized patients with psychosis disclosed using a new type of designer drug, previously unknown to clinicians. As the new drug became better known, the cases with acute intoxication were often also transported to toxicology departments. In this study we summarize 5 short case studies that demonstrate the heterogeneous symptoms associated with MDPV abuse, and draw attention to the frequently occurring delusions, and the extended risk of intravenous substance use. Present case studies include p

Substance use and individual assets in urban adolescents: Subgroups and correlates in emerging adulthood.

INTRODUCTION: To investigate patterns of divergence in adolescent adjustment, this study examined the co-occurring patterns of adolescents' individual assets (e.g., school engagement, values) and substance use, and whether the co-occurring patterns were associated with later functioning in emerging adulthood. METHODS: Participants were 358 (54% females), predominantly African American, urban adolescents, recruited at birth for a prospective study on the effects of prenatal substance exposure in the Midwest United States. Individual assets, using the Developmental Assets Profile, substance use

State Laws on Substance Use Treatment for Incarcerated Pregnant and Postpartum People.

BACKGROUND: Incarcerated perinatal populations report high rates of substance use in the United States (US). Despite this, substance use disorder (SUD) treatment is not routinely available in carceral settings and state policies related to the provision of substance use screening and treatment are unknown. METHODS: We conducted a systematic search in WestLaw through the end of the 2020 legislative session combining the terms "pregnant" and "postpartum" with terms for incarceration and related terms. The search returned 453 statutes from 43 states. A deductive codebook of various maternal heal

Substance use assessment: comparing self-reports with objective data in a research setting.

Accurate assessment of substance use is essential in public health, clinical, and research settings. While self-reports are widely used, they are prone to biases such as social desirability and recall errors. Objective biological measures, such as hair toxicology, offer a longer detection window and may improve data validity. This study examined the concordance between self-reported substance use and hair toxicology results, with a particular focus on cannabis. It also explored the prevalence and predictors of underreporting, and the relationship between cannabis use patterns and THC detection

Detection of hidden populations using illicit substances.

BACKGROUND: The prevalence of hidden populations using illicit substances may lead to underestimation of the health and social impacts. This study aimed to precisely estimate the prevalence and characteristics of hidden populations using illicit drugs. METHODS: We linked national databases in Taiwan, including drug-related autopsy data and police arrest data. This study included individuals who died, underwent autopsy and showed positive drug results during autopsy, covering heroin, methamphetamine, ketamine, ecstasy and new psychoactive substances (NPS), from 2013 to 2019. Harm reduction and its monitoring in Europe, from EMCDDA to EUDA.

BACKGROUND: Harm Reduction, conceptualized by Russell Newcombe in the late 1980s, has revolutionized how drug use, individuals who use drugs, and drug policies are understood globally. Emerging from the HIV/AIDS crisis, Harm Reduction sought to address the dire rates of drug-related infections and the overwhelming burden on healthcare providers. Early initiatives, such as Opioid Substitution Treatment (OST) and needle exchange programs, were met with resistance but gradually established new standards of care, transforming attitudes toward people who use drugs and prioritizing human-centered, r

At the tipping point: Patient experiences of addiction to benzodiazepines and motivation to seek treatment.

INTRODUCTION: Addiction to benzodiazepines is a serious problem, and it is important to better understand how individuals can be motivated to quit. Few studies have investigated patients' perceptions and experiences of addiction to benzodiazepines and there is a need to better understand the influence of motivational factors on treatment seeking. METHODS: In this qualitative study, nineteen adults undergoing treatment for addiction to benzodiazepines participated in semi-structured individual interviews which were analyzed using reflexive thematic analysis. RESULTS: Participants both defined

Polysubstance Use and Related Risk Behaviors among People Who Inject Drugs in Kenya Preparing for Hepatitis C Virus Treatment.

Polysubstance use (PSU), injection drug use (IDU), and equipment sharing are associated with bloodborne infection (BBI) transmission risk, particularly Hepatitis C Virus (HCV), yet data on PSU in low- and middle-income countries (LMICs) is limited. We report on baseline PSU, medication-assisted treatment (MAT) engagement, and motivation to reduce IDU among 95 people who inject drugs (PWID) who accessed needle and syringe programs (NSP) in Nairobi and Coastal Kenya prior to HCV treatment. Bivariate and multivariate logistic regression were used to examine the associations between PSU and behavi

Integrating harm reduction and clinical care: Lessons from Covid-19 respite and recuperation facilities.

In response to the novel coronavirus 2019 (Covid-19) pandemic, many people experiencing homelessness and substance use disorders entered respite and recuperation facilities for care and to isolate and prevent subsequent SARS-CoV-2 transmission. However, because drug use was officially prohibited in these facilities, we observed people who use substances leaving isolation temporarily or prematurely. The initial Covid-19 surge magnified the need for harm reduction access for those who use substances to ensure their safety and well-being and that of their local communities. In this commentary, we
