Conservative Prosthodontic Rehabilitation for Geriatric Patients With Cleft Lip and Palate, and an Edentulous Mandible: A Clinical Report.

Cleft lip and palate affects facial esthetics, mastication, swallowing, and speech functions. We report the case of a 52-year-old woman with congenital cleft lip and palate treated using a palatal obturator prosthesis and removable partial denture. The clinical examination revealed a residual palatal defect communicating with the nasal cavity, tooth loss, and a severely resorbed alveolar ridge. Functional and cosmetic rehabilitation was achieved using a palatal obturator prosthesis and a removable partial denture. At the 1-year follow-up, the patient was satisfied with the functional and esthe

Functional reconstruction of lip defects.

The lip is a crucial structure in the oral and maxillofacial region, serving vital physiological functions such as speech, swallowing, chewing, and expression. Due to the complexity of lip anatomy, function, and the various types of defects, the functional restoration and reconstruction of lip defects remain complex and challenging tasks. In this article, we summarize several methods for functional restoration and reconstruction of lip defects using local flaps that carry the depressor anguli oris muscle, as well as some free flaps. We also introduce methods for repairing extensive defects in

Pharmacologic Management of Eosinophilic Esophagitis.

Proton pump inhibitors (PPIs), swallowed topical corticosteroids (STSs), and dupilumab are highly effective therapies for the treatment of eosinophilic esophagitis. Shared decision-making informs the choice of therapy and factors such as ease of use, safety, cost, and efficacy should be addressed. PPIs are the most common medication utilized early in the disease course; however, for nonresponders, STSs are an excellent alternative. Dupilumab is unlikely to replace PPIs or STSs as first-line therapy, except in highly specific circumstances. Identification of novel biologic pathways and the deve

Extracorporeal membrane oxygenation in 34 days old infant with SARS-COV-2 associated pneumonia.

We present a case of successful veno-arterial extracorporeal membrane oxygenation (VA-ECMO) in infant with SARS-COV-2 associated pneumonia. To the best of our knowledge, this patient was the youngest infant successfully discharged after the VA-ECMO for COVID-19. We report one complication of ECMO support, which was bleeding from the site of the radial artery cannulation. We followed up this patient for 6 months after the discharge: delayed psychomotor development and inability to swallow were noted. Follow-up CT 3 months after the discharge showed linear fibrosis of S5 lung segment.

Foreign body penetrating duodenum into kidney: A case report.

The case of penetrating injury of the kidney caused by a foreign body mistakenly swallowing through the duodenum is rare. A 22-year-old male patient found that a strip of the foreign body penetrated the descending duodenum - the lower pole of the right kidney through an abdominal CT examination. After Multi-Disciplinary treatment, the patient underwent extracorporeal ultrasound-assisted endoscopic foreign body removal and hemostatic clamp suture. Extracorporeal ultrasound monitoring and intravenous pyelography showed that there was no leakage of contrast medium around the right kidney. No hema

Trichobezoar-A rare cause of hyperemesis in a pregnant woman.

Bezoars consist of ingested foreign material or organic matter which forms a mass in the gastrointestinal tract, usually in the stomach. Trichobezoars formed by swallowed hair may present with vomiting, malnutrition, weight loss or abdominal pain with signs of gastrointestinal obstruction. There are limited case reports of Trichobezoar during pregnancy. We present a case of young pregnant women who was admitted on few occasions with vomiting in early pregnancy and was treated as hyperemesis gravidarum. Failure of response to conventional management prompted further investigation, revealing tri

Dysphagia in a patient with Plumer Vinson syndrome: An innovative surgical dilatation technique after failed endoscopy.

For the last five years, a 43-year-old female presented with progressive dysphagia for solids and liquids. She was treated for iron deficiency anemia by systemic and oral iron therapy. Gastroenterologists failed to pass endoscopes through the upper esophagus. Gastrogrifin swallow revealed a critical narrowing of the esophagus. She was referred to a thoracic surgeon for further management. The cervical esophagus was exposed through the neck incision along the anterior border of the left sternocleidomastoid muscle, and esophageal dilatation was achieved using the silicon foleys catheter. It is a

Multiple Epidermal Inclusion Cysts of Epiglottis.

Epiglottic cysts are benign laryngeal lesions. Though congenital cysts are often life-threatening, they are usually painless and symptomless in adults, or present with slight, though perceptible, interference with either swallowing or speaking or respiration. Early recognition and diagnosis by routine mirror laryngoscopy is advocated. Prompt removal or draining is the treatment. This is a case report of 42-year-old woman with complaints of feeling of something in throat, sleep apnoea and hoarseness, who on routine mirror laryngoscopy was found to have two large cysts arising from the free bord

Bezoar: an uncommon cause of upper gastrointestinal bleeding.

Bezoars are aggregates of non-digestible material that accumulate in the gastrointestinal tract. They can be classified according to their composition, and phytobezoars (composed of vegetable or fruit fiber) is the most common type. They most often occur in patients with some risk factors, namely prior gastric surgery, neuropsychiatric, endocrine or other disorders that lead to abnormal gastric function, or poor gastric peristalsis. Bezoars may be asymptomatic but most commonly cause abdominal discomfort or pain, nausea, vomiting, fullness, difficulty in swallowing, or anorexia and weight loss

Medical Comorbidities and Complications Associated with Poliomyelitis and Its Sequelae.

Survivors of poliomyelitis may experience long-term sequelae that put them at increased risk for injury, pain, cardiovascular deconditioning, and functional decline. Osteoporotic fractures and entrapment neuropathies, in particular, may result in greater impairments in one's mobility and ability to perform activities of daily living. Dysphagia may necessitate the use of compensatory swallow strategies to minimize aspiration risk. Comorbid conditions, including hypertension, dyslipidemia, obesity, and stroke, are also very prevalent in this population. Risk factor modification, including diet,

Patency Capsule Aspiration.

A 77-year-old man with anemia who had undergone 2 abdominal surgeries for colon and gastric cancer experienced dyspnea after swallowing a patency capsule before endoscopy for investigating the cause of anemia. Chest radiography and computed tomography revealed that the patency capsule was located within the bronchus intermedius. It was successfully removed by flexible bronchoscopy. The balloon was placed over the capsule and inflated. Subsequently, the catheter was pulled, while thus dragging the capsule with it and preventing its destruction. In cases of patency capsule aspiration, the capsul

Emergency CT of abdominal complications of ingested fish bones: what not to miss.

Fish bones are the most commonly ingested structures and the most common cause of foreign body perforation of the gastrointestinal tract (GIT). Clinical presentation of foreign body GIT perforation is nonspecific, in many cases with clinical signs of acute abdomen, which can mimic appendicitis, diverticulitis, ulcer peptic disease, and other common inflammatory conditions. Besides, patients commonly do not refer that a fish bone was swallowed. Since this condition is usually not suspected by referring physicians of the emergency department (ED), radiologists play a key role in this diagnosis;

Importance of orthodontic intervention of the Class III malocclusion in mixed dentition.

INTRODUCTION: Supervising the development of occlusion, managing problems during the transition from mixed to permanent dentition, as well as controlling environmental factors that contribute to establishing malocclusion, are important actions to achieve a Class I occlusion with facial balance. Among these problems, the malocclusions associated with dysfunctions such as mouth breathing or obstructive sleep apnea syndrome (OSAS), atypical swallowing and abnormal tongue position, open bites, crossbites and maxillomandibular discrepancies, and especially the Class III malocclusion can be listed.

Neonatal Aerodigestive Disorders: Epidemiology and Economic Burden.

Aerodigestive disorders, those affecting the upper and lower airway or upper gastrointestinal tract, are interrelated anatomically during fetal development and functionally after birth. Successful respiration and feeding requires careful coordination to promote effective swallowing and prevent aspiration. I describe the epidemiology, including the prevalence of the most common aerodigestive disorders. The ability of an infant to feed by mouth at discharge, without a surgically placed feeding tube, is an important neurodevelopmental marker. Therefore, aerodigestive disorders have a high potenti

Accidental ingestion of hairpin in an adolescent girl.

Ingestion of headscarf pin is a particular type of foreign body ingestion sometimes experienced by adolescent girls and even older women while putting on their headscarf. We report on a 13-year-old girl who presented at the emergency unit at our hospital within 48 hr after accidentally swallowing a headscarf pin. The pin was successfully retrieved from the ascending colon through colonoscopy. Headscarf pin ingestion is a serious problem among women who wear headscarves for various reasons. However, the retrieval of such pins from the colon through colonoscopy has been infrequently reported.

Over-the-scope-clip treatment of gastrobronchial fistula following minimally invasive oesophagectomy: a novel approach.

Gastrobronchial fistulae (GBF) following minimally invasive oesophagectomy (MIO) is a rare entity, with an estimated incidence of 0.3-1.5% according to the published literature. It could present with persistent cough (Ohno's sign), chest pain, haemoptysis and recurrent pneumonia. Barium swallow examination remains the most sensitive investigation in 78% of the cases; the main stay of management is surgical and in some cases endoscopic (stent insertion). We report a rare case of a GBF 1 month after two-stage MIO for cancer of the gastro-oesophageal junction, which was successfully treated for t

[A case of non-cardiac chest pain caused by esophageal motility disorder observed on esophageal high-resolution manometry].

A 20-year-old man was referred to our hospital with dysphagia and chest pain. Heart disease was denied. No abnormality was observed in upper esophagogastroduodenoscopy and fluoroscopy;furthermore, no gastric acid-related symptoms were observed on combined esophageal multichannel intraluminal impedance and pH monitoring. Esophageal high-resolution manometry (HRM) performed by liquid swallow revealed normal peristalsis;however, HRM performed while the patient was taking solid meals showed abnormal contraction, and the patient simultaneously complained of chest pain. Therefore, we diagnosed this
