Obesity-Driven Hypertension: Exploring the Mechanisms and Modern Treatment Strategies.

Obesity and hypertension are interdependent chronic conditions that substantially elevate global cardiovascular risk. Rising obesity prevalence has led to a parallel increase in hypertension, driven by complex physiological disturbances that extend beyond excess body weight alone. This review synthesizes current evidence on the mechanisms linking adiposity to blood pressure elevation, emphasizing the roles of sympathetic nervous system overactivity, insulin resistance, renal sodium retention, and adipose-derived hormonal and inflammatory dysregulation. Particular attention is given to visceral…

Allergy-associated acute coronary syndrome without anaphylaxis in a prospective observational study.

Kounis syndrome is an acute coronary syndrome (ACS) triggered by allergic reactions via mast cell activation, classically accompanied by anaphylaxis. However, ACS associated with mild or subclinical allergic reactions may be underrecognized. We investigated immune markers and autonomic nervous system activity in patients with ACS and characterized allergy-associated ACS without anaphylactic symptoms. Among 375 patients treated for ACS (April 2022 to March 2024), 49 were prospectively enrolled. Patients were classified into non-allergy-associated ACS (ACS-NA, n = 32) and allergy-associated ACS

Single-cell transcriptomic insights into the intrinsic cardiac nervous system: diversity, development, and neuro-cardiac interactions.

The intrinsic cardiac nervous system (ICNS), composed of sympathetic, parasympathetic, and sensory neurons embedded within the heart, plays a pivotal role in regulating cardiac electrophysiology and contractility. Functional imbalance within this network represents a central mechanism driving myocardial infarction, heart failure (HF), atrial fibrillation (AF), and hypertension. Despite decades of investigation, the precise pathophysiological basis of ICNS involvement in cardiovascular disease remains poorly understood. Traditional bulk RNA sequencing, which measures averaged gene expression ac

Autonomic modulation of neuroplasticity in spinal cord injury rehabilitation: insights from a narrative review.

Spinal Cord Injury (SCI) causes persistent autonomic dysregulation, which is not merely a clinical epiphenomenon, but a mechanistic condition shaping the neurochemical, neurovascular, and immuno-endocrine milieu in which plasticity unfolds. Yet, the modulatory role of the Autonomic Nervous System (ANS) in post-injury circuit reorganization remains insufficiently integrated in rehabilitation paradigms. This narrative review synthesizes emerging evidence (2020-2025) describing how sympatho-vagal dynamics constrain or enable adaptive plasticity following SCI, and surveys therapeutic strategies th

Renal denervation attenuates cardiac fibrosis and improves left ventricular function in rats with myocardial infarction.

The sympathetic nervous system (SNS) plays a critical role in regulating inflammatory responses after myocardial infarction (MI). We aimed to investigate whether modulation of the SNS through renal denervation (RDN) influences myocardial remodeling following MI. MI in rats was induced 2 days after bilateral RDN or Sham surgery. Cardiac MRI showed that left ventricular ejection fraction was significantly higher in RDN-treated MI rats compared to Sham-treated MI rats. RNA sequencing of left ventricular tissue revealed that RDN suppresses fibrosis and inflammation related pathways at the whole tr…

Balancing the autonomic scale: the promise of transcutaneous vagus nerve stimulation in combating cardiovascular diseases.

INTRODUCTION: Cardiovascular diseases (CVD) remain a significant burden on global health, often associated with autonomic nervous system (ANS) dysfunction, particularly prolonged sympathetic hyperactivity. Transcutaneous vagus nerve stimulation (tVNS) offers a non-invasive method for modulating vagal tone to achieve autonomic balance, presenting a potentially promising therapeutic approach for hypertension, heart failure and arrhythmia that requires further validation. METHODS: This review elucidates the mechanistic foundations of tVNS and examines its evolving application in the management o

Role of C-Jun N-Terminal Kinases on a Stressed Epithelium: Time for Testing Isoform Specificity.

Biological, physiological, and psychological stressors cause a "stress response" in our bodies. Stressors that are sensorily perceived (either acute or chronic) trigger hormonal responses from the sympathetic nervous system-the SAM and HPA axis-that effect intended organs to alert the individual. Other stressors have a direct effect on the target organ(s) of the body-e.g., physical injury and wounds, toxins, ionizing, and UV radiation. Both kinds of stressors change cell equilibrium, often leading to reactive oxygen species (ROS) accumulation and cellular damage. Among the signaling pathways i…

Critical Role of microRNA-33a/b in Cardiovascular and Metabolic Disease: Molecular Mechanisms and Therapeutic Perspectives.

MicroRNAs (miRNAs) serve as fundamental post-transcriptional regulators of gene expression, among which the miR-33 family, consisting of miR-33a and miR-33b, has emerged as a critical modulator in the pathogenesis of cardiovascular and metabolic diseases. These miRNAs are embedded within the intronic regions of SREBF genes and play pivotal roles in cholesterol homeostasis, fatty acid metabolism, and inflammatory regulation. Notably, miR-33a is highly conserved across various species, whereas miR-33b is found primarily in primates and some other mammals, complicating the development of relevant

Exploring the complex relationship between psychosocial stress and the gut microbiome: implications for inflammation and immune modulation.

There is growing interest in understanding the complex relationship between psychosocial stress and the human gastrointestinal microbiome (GIM). This review explores the potential physiological pathways connecting these two and how they contribute to a proinflammatory environment that can lead to the development and progression of the disease. Exposure to psychosocial stress triggers the activation of the sympathetic nervous system (SNS) and hypothalamic-pituitary axis (HPA), leading to various physiological responses essential for survival and coping with the stressor. However, chronic stress…

'Wired up about self' - narcissistic traits predict elevated physiological arousal during self-disclosure in conversation.

Individuals vary in their self-disclosure motivations and physiological responses. It is unclear, however, whether the content of a person's self-view accounts for this variation. In this paper we explore the impact of self-disclosure on autonomic nervous system activity in participants with high and low levels of grandiose narcissistic traits. Three conversational experiments were conducted to simulate different contexts of self-disclosure: getting acquainted (Experiment 1), talking about emotional life experiences (Experiment 2), and telling emotional stories with varying self-relevance (Exp

High-Fructose-Induced Salt-Sensitive Hypertension: The Potential Benefit of SGLT4 or SGLT5 Modulation.

Hypertension is an important risk factor for cardiovascular diseases. High salt intake when consumed with excess fructose enhances hypertension and resultant cardiovascular disease. Usually, the small intestine absorbs dietary fructose, and the proximal tubule of kidney reabsorbs filtered fructose into the circulation with the help of different transporters including SGLT4 and SGLT5. Very recently, SGLT5 mRNA has also been found to be expressed in the heart. High-fructose diet stimulates the sympathetic nervous system and renin-angiotensin-aldosterone (RAAS) activity, of which both are respons…

Investigating the course and predictors of desire to void based on heart rate variability.

Urinary incontinence is closely related to the motor ability and toileting tasks. Some nursing home residents with limited mobility who cannot reach the bathroom in time highly depend on caregivers for toileting assistance. However, nursing home staffing is often insufficient to meet the needs of all residents. Monitoring the desire to urinate is vital to minimize functional dependence and improve the quality of life of older people. Improved reliability of the desire to void monitoring requires exploring more effective monitoring methods. In this paper, we observed the changes in heart rate v

Safety and Efficacy of Renal Denervation for the Treatment of Resistant Hypertension in Patients with Chronic Kidney Disease: A Narrative Review of the Literature.

Arterial hypertension is highly prevalent among individuals with chronic kidney disease (CKD), exhibiting a bidirectional association and playing a critical role in the progression of renal dysfunction. CKD affects approximately 10-12% of the global population and is often a common comorbidity in patients with true resistant hypertension. The sympathetic nervous system (SNS) plays a key role in the pathophysiological cascade of CKD-mediated hypertension. Current pharmacological therapies do not directly target SNS overactivity, highlighting the need for alternative approaches. Renal denervatio…

Interplay Among Gut Microbiota-Derived TMAO, Autonomic Nervous System Dysfunction, and Heart Failure Progression.

The gut microbiota is crucial for metabolic homeostasis and cardiovascular health. Dysbiosis triggers a gut-brain-heart axis dysfunction: vagal signaling promotes neuroinflammation and cerebral damage, which in turn impairs cardiac function. This bidirectional cycle is further exacerbated by reduced cerebral perfusion. Trimethylamine-N-oxide (TMAO), a metabolite of dietary choline and L-carnitine, acts as a primary mediator in this network. Elevated TMAO levels-resulting from bacterial conversion and hepatic oxidation-are linked to atherosclerosis and heart failure. Mechanistically, TMAO activ
