Nonfluent Variant Primary Progressive Aphasia on FDG, 11 C-PIB, and 18 F-APN-1607 PET Imaging.

A 61-year-old right-handed man presented with decreased cognitive function, short-term memory, fluent speech disorders, and grammatical errors for 1 year. The patient underwent PET imaging with 11 C-PIB, 18 F-FDG, and 18 F-APN-1607. The 11 C-PIB PET showed no amyloid accumulation; the 18 F-FDG PET showed hypometabolism in the bilateral frontal lobe, temporal lobe, and midbrain; and the 18 F-APN-1607 PET showed tau accumulation in the brainstem, basal ganglia, and left inferior frontal gyrus. These findings suggested a diagnosis of nonfluent variant primary progressive aphasia. This case emphas…

Sylvian fissure arteriovenous malformations.

We have operated on 16 cases of arteriovenous malformation (AVM) in and around the sylvian fissure. We call these lesions "sylvian fissure AVMs" and classify them into four subdivisions, namely, pure, lateral, medial, and deep AVMs. By others, they have been variously called AVMs of the basal ganglia, insula, anterior choroidal artery, frontal lobe, or temporal lobe. These sylvian fissure AVMs showed similar angiographic findings: the feeders in all cases were branches of the middle cerebral artery; in some cases, additional feeders from the anterior and posterior choroidal and posterior commu…

The importance of imaging in diagnosis of infected otogenic pneumatoceles.

OBJECTIVE: Spontaneous otogenic pneumatocele is a rare entity resulting from a pressure gradient between a dehiscent temporal bone and the intracranial space. Secondary infection can ensue in patients with concurrent otomastoiditis. The current study discusses the clinical presentation and imaging characteristics of two cases of secondarily infected otogenic pneumatoceles. STUDY DESIGN: Case series. RESULTS: Two patients were diagnosed with a temporal lobe abscess in the setting of otogenic pneumatocele. Diagnosis was aided by both CT and MRI demonstrating a diffusion restricting lesion with…

Spontaneous otogenic intracerebral pneumocephalus: case report and review of the literature.

Pneumocephalus is commonly associated with head and facial trauma, ear infection or surgical interventions. We describe the rare case of a spontaneous pneumocephalus arising from lateral mastoid air cells. A 48-year-old man presented with a 10-day history of sudden, repetitive, 'hammering-like' acoustic sensations in his left ear that were followed by word-finding difficulties and loss of vision in the right visual field. Imaging revealed a large, left temporal pneumatocele associated with a small acute intracerebral hemorrhage. Left temporal and subtemporal craniotomy and decompression were p

A Case Report of Early-Onset Alzheimer's Disease Using 18F-FDG PET and 18F-FBB PET.

We describe a 40-year-old female patient who presented with sleep disturbance, intermittent headache, and gradual subjective cognitive decline. 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) showed mild FDG hypometabolism in bilateral parietal and temporal lobes. However, 18F-florbetaben (FBB) amyloid PET revealed diffuse amyloid retention in the lateral temporal cortex, frontal cortex, posterior cingulate cortex/precuneus, parietal cortex, and cerebellum. This finding supports the clinical significance of amyloid imaging in diagnostic work-up of early-onset Alzheimer's diseas…

Phonemic word fluency is related to temporal and striatal gray matter volume in healthy older adults.

Word fluency (WF) tasks that tap verbal and executive function show deteriorating performance by advancing age. To address the scarcely stud … as associated with smaller GM volume in the left inferior frontal cortex, the right anterior medial temporal lobe, and the right striatum. There were no significant associations for semantic WF. The fact that our temporal and subcortical findings were bilateral and right-lateralized, may reflect age-related compensation by these brain areas.

Epilepsy surgery for a patient with neurofibromatosis type 1 concomitant with moyamoya syndrome.

BACKGROUND: Epilepsy associated with neurocutaneous disorders is well known, and the prevalence of epilepsy among patients with neurofibromatosis type 1 (NF1) ranges from 4% to 13%. However, epilepsy surgery for patients with NF1 is not commonly performed. CASE DESCRIPTION: We report the case of a patient with medically intractable mesial temporal lobe epilepsy (mTLE) concomitant NF1 and moyamoya syndrome (MMS) who had already undergone bilateral superficial temporal artery-middle cerebral artery anastomosis at 17 years old. As the standard temporal approach was unavailable, we evaluated the …

Combined semantic dementia and apraxia in a patient with frontotemporal lobar degeneration.

In this study we report neuropsychological and brain-imaging findings in a patient with frontotemporal lobar degeneration. Brain imaging using registration of (18)fluorodeoxyglucose (FDG)-PET data to three-dimensional (3-D) magnetic resonance imaging showed atrophy and highly significant hypometabolism of the left temporal lobe and both frontal lobes. Volumetric measurements of the hippocampi/amygdala showed a reduction in volume of 25% on the left compared to right within cortical areas. Neuropsychological testing revealed semantic dementia with severe anomia as well as apraxia with impairmen…

Memory Impairment in Epilepsy: A Comprehensive Review of Structural and Pharmacological Influences.

Epilepsy affects 50 million people worldwide and is frequently associated with memory impairment, contributing significantly to reduced quality of life. Memory difficulties are commonly reported across epilepsy syndromes and may coexist with anxiety, reduced self-esteem and social withdrawal. While temporal lobe epilepsy is often highlighted due to the crucial role of mesial temporal structures in memory processing, memory functions also rely on distributed neural networks extending beyond the temporal lobe. A common concern among patients with epilepsy, discussed during clinic visits, relates…

Anterior temporal lobectomy and selective AmygdaloHippocampectomy complications across Europe: review, meta-analysis, and Delphi consensus.

INTRODUCTION: Epilepsy is a neurological disorder affecting over 50 million people globally, with around 30 % of them classified as having drug-resistant epilepsy (DRE). Temporal lobe epilepsy (TLE) is the most frequently encountered type of surgically treated epilepsy. The primary surgical approaches for TLE include anterior temporal lobectomy (ATL) and selective amygdalohippocampectomy (selAH). RESEARCH QUESTION: This study sought to gather expert European consensus on surgical strategies and complication rate for ATL and selAH, in both adult and pediatric patients. MATERIALS AND METHODS: …

The changes in neural complexity and connectivity in thalamocortical and cortico-cortical systems after propofol-induced unconsciousness in different temporal scales.

Existing studies have indicated neural activity across diverse temporal and spatial scales. However, the alterations in complexity, functional connectivity, and directional connectivity within the thalamocortical and corticocortical systems across various scales during propofol-induced unconsciousness remain uncertain. We analyzed the stereo-electroencephalography (SEEG) from wakefulness to unconsciousness among the brain regions of the prefrontal cortex, temporal lobe, and anterior nucleus of the thalamus. The complexity (examined by permutation entropy (PE)), functional connectivity (permuta…

The psychological basis of hunger and its dysfunctions.

This article describes a new and emerging psychological perspective on hunger, together with the implications of that perspective, which is … ugments the expected - remembered - pleasure for any food. Both hungers are supported by the medial temporal lobe memory system. Damage to this system can occur from eating a Western-style diet and, longer-term, from obesity and its consequences. Medial temporal lobe memory damage may cause deficits in specific hunger, but most especially in general hunger, res …
