Reducing the carbon footprint and costs of laparoscopic cholecystectomy: a modelling study on interventions for sustainable surgery.

BACKGROUND: Reducing the carbon footprint of surgical care requires carbon and economic evaluations to guide and support effective sustainability initiatives. This study aims to quantify the carbon footprint (CO₂-eq.) and financial (euros) impact of targeted interventions in laparoscopic cholecystectomy (LC). METHODS: This modelling study evaluated the carbon and financial impact of sustainability interventions in LC, structured within the 10R circularity framework. Key contributors to emissions (disposables, anaesthesia, and energy) were addressed through seven mitigation strategies switchin

Factors associated with all-cause 60-day readmission in patients with end-stage renal disease on hemodialysis discharged on outpatient parenteral antimicrobial therapy.

PURPOSE: Outpatient parenteral antimicrobial therapy (OPAT) is standard of care in patients who require intravenous antibiotics after hospitalization. There is a lack of data on OPAT in patients with end-stage renal disease on hemodialysis. This study characterized hemodialysis patients discharged on OPAT and identified factors associated with all-cause 60-day hospital readmission.  . METHODS: This institutional review board-approved retrospective cohort study included hemodialysis patients 18 years of age or older who were discharged from January 2020 to August 2022 with at least 1 week of O

A qualitative study on the impact of bleeding disorders on the daily life of Dutch women.

BACKGROUND: Despite growing awareness of the need for better healthcare and research focused on women, sex-based inequalities persist, resulting in poorer health outcomes for women. Particularly, women with congenital bleeding disorders (CBDs) receive less care, are underrepresented in research, and experience diagnostic delays compared to men with CBDs, despite both exhibiting symptoms. OBJECTIVES: To identify lived experiences and care needs of women with CBDs. DESIGN: A qualitative study, including focus groups and semi-structured interviews. METHODS: Women were eligible to participate i

Triage in Action: A Principles-Based Approach to Mass Casualty Management in Tactical Combat Casualty Care.

BACKGROUND: Current triage practices in military mass casualty (MASCAL) events are frequently misaligned with real-world operational needs, leading to delays, confusion, and suboptimal outcomes. Despite the existence of formal triage systems, field responders often default to simplified methods that emphasize speed, clarity, and survivability. This review supports the Committee on Tactical Combat Casualty Care (CoTCCC) recommended transition to a principles-based, two-pass triage system optimized for Role 1 prehospital environments. METHODS: A multi-modal analysis incorporating retrospective

Cost-Effectiveness Analysis of the Technology-Enhanced Community Health Nursing Program for Adolescent Girls and Young Adult Women With Pelvic Inflammatory Disease.

BACKGROUND: Pelvic inflammatory disease (PID) disproportionately impacts adolescents and young adult women. The Technology-Enhanced Community Health Nursing (TECH-N) trial demonstrated the potential benefit of a novel community health intervention for adolescents with PID. We assess the cost-effectiveness of TECH-N compared with standard care. METHODS: We constructed a cohort Markov model to assess the cost-effectiveness of TECH-N for adolescents with PID in an urban setting. The model used nine health states: PID; two states for STIs; four states for PID sequelae, recovery, and deceased stat

Barriers to Care Leading to Fatal Consequences: A Case of Tuberculosis Sepsis Amid Policy Change.

Mycobacterium tuberculosis is an acid-fast bacterium with a diverse range of clinical manifestations and is considered rare in Western countries. Tuberculosis (TB) remains a global health concern and can pose several challenges in diagnosis and treatment. Paired with changing immigration policies, immigrant populations can face several barriers to healthcare. This case illustrates the impact of immigration policy on clinical outcomes and how government policies can act as a barrier to care. We present a case of a 26-year-old Guatemalan man with no known past medical history who came to the eme

A Rapid Response Mobile Application Improves First-Year Resident Clinical Performance During Simulated Care Events: A Randomized Controlled Trial.

BACKGROUND: Rapid response teams (RRTs) are critical to the timely and appropriate management of acutely decompensating patients. In the academic setting, the vital role of RRT leader is often filled by a junior resident physician who may lack the necessary medical knowledge and experience. Cognitive aids help improve guideline adherence and may support resident performance as they transition into leadership roles. OBJECTIVE: This study evaluated the impact of a rapid response mobile application on intern performance during simulated rapid response events. DESIGN: This randomized controlled

Successful Discharge to Community From Home Health Less Likely for People in Late Stages of Dementia.

BACKGROUND AND PURPOSE: Several studies have established the efficacy of home health in meeting the health care needs of people with Alzheimer disease and related dementias (ADRD) and helping them to remain at home. However, transitioning to the community after discharge from home health presents challenges to patient safety and quality of life. The severity of an individual's functional impairments, cognitive limitations, and behavioral and psychological symptoms may compound these challenges. The purpose of this study was to examine the association between dementia severity and successful di

Massive Burn Injuries: Characteristics and Outcomes From a Single Institution.

Massive burn injuries are a unique patient population with unique treatment paradigms. Data from 155 adult patients, admitted from 2009 to 2019, with >50% total body surface area burns (TBSA) were collected and analyzed. Average burn size was 70% TBSA and 63% had a concomitant inhalation injury. Approximately 30% of patients (46/155) transitioned to comfort care-only measures within 24 hours of admission. Standard treatment patients were younger (37 ± 13 vs 60 ± 19 years; p < .00001), male (94% vs 28%; p = .001) and had smaller TBSA (66 ± 13 vs 80 ± 16; p < .00001). Of the standard treatment g

Increasing socioeconomically disadvantaged patients' engagement in breast cancer surgery decision-making through a shared decision-making intervention (A231701CD): protocol for a cluster randomised clinical trial.

INTRODUCTION: Socioeconomic disparities for breast cancer surgical care exist. Although the aetiology of the observed socioeconomic disparities is likely multifactorial, patient engagement during the surgical consult is critical. Shared decision-making may reduce health disparities by addressing barriers to patient engagement in decision-making that disproportionately impact socioeconomically disadvantaged patients. In this trial, we test the impact of a decision aid on increasing socioeconomically disadvantaged patients' engagement in breast cancer surgery decision-making. METHODS AND ANALYS

Initiating gender-affirming hormones for transgender and non-binary people: A qualitative study of providers' perspectives on requiring mental health evaluations.

OBJECTIVE: Gender-affirming hormones (GAH)-the use of sex hormones to induce desired secondary sex characteristics in transgender individuals-is vital healthcare for many transgender people. Among prescribers of GAH, there is debate regarding the value of a universal requirement for an evaluation by a mental health provider prior to GAH initiation. The purpose of this qualitative study was to describe the range of attitudes and approaches to mental health evaluation among GAH providers in the United States. We analyzed the providers' attitudes and base our recommendations on this analysis. ME
