Cataract surgery in the context of age-related macular degeneration: challenges and considerations.

Age-related macular degeneration (AMD) and cataracts commonly coexist in the aging population, posing unique diagnostic and therapeutic challenges for ophthalmologists. Although cataract surgery can substantially improve visual function, outcomes are often influenced by the type and severity of underlying macular pathology. This narrative review explores current evidence on the impact of cataract surgery in patients with AMD, including preoperative assessment strategies, intraoperative considerations, and postoperative visual expectations. The role of multimodal imaging in detecting subclinica

Verapamil-sensitive fascicular ventricular tachycardia precipitated by promethazine overdose.

Verapamil-sensitive fascicular tachycardia is a type of ventricular tachycardia (VT) involving the specialised cardiac conducting tissue. Drug overdose is an important cause to consider when assessing patients with a new presentation of a broad complex tachycardia. Multiple antihistamines have been implicated as pro-arrhythmic, but these tend to precipitate Torsades de Pointes via drug-induced QT prolongation. We present an educational case of a Verapamil-sensitive fascicular VT with typical electrocardiogram (ECG) characteristics, which developed immediately following an overdose of promethaz

Type 2 diabetes mellitus: Isn't it time to update the terminology?

There is a paradigm shift in the approach to the treatment of type 2 diabetes mellitus (T2DM) based on recent research. "Twin cycle hypothesis" and remission of diabetes proved that diabetes is a disease due to excess fat accumulation above the personal fat threshold. Hence, weight reduction and, whenever possible, achievement of remission has become therapeutic target for the management of T2DM. Increasing evidence suggests that T2DM is a part of the spectrum of diseases caused by excess fat accumulation. Hence, it indeed is the need of the hour to update the terminologies to indicate that T2

Immunotherapy in Type 1 Diabetes: Emerging Therapies and Future Directions.

Type 1 Diabetes Mellitus (T1D) is an autoimmune disorder marked by the destruction of insulin-producing pancreatic β-cells. While insulin therapy remains the standard of care, advancements in immunotherapy present promising alternatives aimed at halting disease progression and reducing insulin dependence. This brief review highlights key immunomodulatory therapies, including teplizumab, which has demonstrated the ability to delay the onset of T1D, and rituximab, known for preserving β-cell function, though its effects tend to be transient. Emerging treatments, such as stem-cell therapies, also

Emergence of Bictegravir Resistance in a Treatment-Experienced PWH on Functional Monotherapy and Rapid Replacement by an Ancient Wild-Type Strain Following Transient Treatment Interruption.

A treatment-experienced, highly adherent person living with HIV for over 25 years developed resistance mutations against all four major ART classes, including bictegravir (BIC). This led to viral failure on a quadruple regimen including BIC and doravirine (DOR). Resistance emergence was associated with M184V, thymidine analog mutations (TAMs), NNRTI mutations (108I, 234I, 318F), and INSTI mutations (T97A, G140S, Q148H, G149A), likely driven by suboptimal BIC levels due to divalent cation interactions. During a two-month ART interruption, the resistant virus was rapidly replaced by an ancient w

Cycles of Neglect: A Statutory Review of State Policies Regarding Feminine Hygiene Product Provision During Incarceration.

Historically, American correctional institutions were designed for men. An increase in incarcerated females has required states to grapple with issues related to menstrual hygiene, including menstrual cycles. While the federal government requires that women and persons who menstruate in federal prisons be supplied with minimal menstrual hygiene supplies, there is no such mandate for menstruators incarcerated in state prisons. We conducted a review and analysis of state legal statutes and Department of Corrections' (DoC) policies pertaining to menstrual product provision to incarcerated individ

LpX and LpZ associated hypercholesterolemia in a patient with primary sclerosing cholangitis - A case report.

This case report highlights the complex interplay between cholestatic liver disease and lipoproteins in a 45-year-old male with primary sclerosing cholangitis who presents with severe hyper-cholesterolemia. Nuclear magnetic resonance lipoprofile revealed marked elevations in lipoprotein X and lipoprotein Z (LpZ), with LpZ being the predominant abnormal lipoprotein. Treatment with evolocumab, a proprotein convertase subtilisin/kexin type 9 (PCSK9) monoclonal antibody, effectively reduced LpZ levels, with a consequent decrease in low-density lipoprotein particle concentration. A novel deep learning approach to classify 3D foot types of diabetic patients.

Diabetes mellitus is a worldwide epidemic that leads to significant changes in foot shape, deformities, and ulcers. Precise classification of diabetic foot not only helps identify foot abnormalities but also facilitates personalized treatment and preventive measures through the engineering design of foot orthoses. In this study, we propose a novel deep learning method based on DiffusionNet which incorporates a self-attention mechanism and external features to classify the foot types of diabetic patients into six categories by using simple 3D foot images directly. Our approach achieves a high a

[Update on asthma 2024 - what the ENT specialist needs to know].

The lifetime prevalence of 8.6% of asthma in Germany reflects the high medical and socioeconomic impact of the disease. Asthma treatment goals have changed during the last decades: from symptom control to symptom prevention, with highly effective, disease-modifying anti-asthmatic drugs (DMAADs) aiming at asthma remission. In order to achieve this goal, phenotyping of patients (including an evaluation of allergies and type 2 biomarkers) is crucial for personalized treatment. The identification and effective treatment of concomitant diseases, such as allergic rhinitis or chronic rhinosinusitis w

Ethical implications of defining longstanding anorexia nervosa.

The label severe and enduring anorexia nervosa (SE-AN) is widely used in the literature on longstanding anorexia nervosa (AN). However, the process of constructing the criteria and the use of the label SE-AN has ethical implications that have not been taken into account. Through combining existing literature and lived experience perspective, this paper addresses to what extent the current criteria do and do not reflect the lived experience. Arguments are presented on why the process of constructing the criteria for SE-AN and the application of the label can be both identified as, and give rise

Acute Appendicitis in an 86-Year-Old Patient: Uncommon Age for a Common Disease.

Appendicitis, an inflammation of the vermiform appendix, is one of the most common causes of acute abdomen and one of the most frequent indications for emergency abdominal surgery worldwide. Any person older than 65 years old is considered elderly. The elderly population constitutes only 5-10% of total appendicitis cases. The symptoms depend on the location of the appendix. Generally, lower abdominal pain and anorexia are known to be the most common symptoms of appendicitis. Although young adults have a higher prevalence of appendicitis, the elderly have a higher complication rate, 37.5% versu

Thiamine-responsive megaloblastic anaemia in a young adult with acute pancytopenia.

Thiamine-responsive megaloblastic anaemia (TRMA) is a rare autosomal recessive disorder characterised by the clinical triad of megaloblastic anaemia, sensorineural hearing loss and diabetes mellitus (DM) in young patients. We present a case of a young man with type 1 DM who presented with pancytopenia of unclear aetiology, initially attributed to a COVID-19 infection. After obtaining a bone marrow biopsy and pursuing genetic testing, two pathogenic variants of the SLC19A2 gene consistent with TRMA were discovered in this patient. Treatment with 100 mg of thiamine oral supplementation daily led

Treatment strategy for patient with non-syndromic tooth agenesis: a case report and literature review.

BACKGROUND: Non-syndromic tooth agenesis (NSTA) is a type of ectodermal dysplasia (ED) in which patients with non-syndromic oligodontia may only affect teeth. No pathological findings were found in other tissues of the ectodermal. Herein, we report a case of a NSTA patient with severe dental anxiety and poor oral health. CASE PRESENTATION: A 5-year-old boy without systemic diseases presented as a patient with oligodontia, extensive caries, and periapical periodontitis. Molecular genetic analysis found a mutation in the Ectodysplasin A (EDA) gene, confirming the diagnosis of NSTA. CONCLUSION:

Barriers to cultural participation by people with disabilities in Europe: a study across 28 countries.

This article discusses the findings of a new qualitative study conducted in 28 European countries, examining barriers to cultural participation as perceived by representatives of organisations of people with disabilities. The study explores barriers operating in all art-forms as well as in cultural heritage, and it encompasses participation of people with a broad range of disability types both as audiences and as creators of culture. The article evidences that a range of interlinked barriers are commonly perceived by people with disabilities in five areas - lack of effective laws and policies;

Spontaneous Regression of Ossifying Fibroma in the Oral and Maxillofacial Region: A Rare Case With 4-Year Follow-Up.

The case presented in this study is a rare instance of an ossifying fibroma (OF), a type of benign fibro-osseous lesions, spontaneously regressed without surgical intervention. A 9-year-old boy with an intraosseous lesion in the left maxilla was diagnosed as OF. The surgeon suggested surgical excision, but due to personal reasons, the patient and his parents deferred the surgery and opted for regular follow-up. During a 4-year follow-up, the surgeon found that the lesion had significantly decreased in size and the facial deformity had remitted. At the latest follow-up, the deformity of his lef

[Clinical and socially significant consequences of falls in elderly and senile persons (analytical review).].

Falls refers to geriatric syndromes, which is accompanied by a significant number of adverse clinically and socially consequences. For the rational organization of medical, social, psychological and other types of assistance and rehabilitation measures, separate groups of consequences developing as a result of a fall are distinguished: physical, functional, psychological, social. Every year, every fourth elderly and older person faces a fall, while about half of the victims seek medical help. Among people of older age groups who have suffered a fall episode, 20-30% have complications in the fo

Current and upcoming radionuclide therapies in the direction of precision oncology: A narrative review.

As new molecular tracers are identified to target specific receptors, tissue, and tumor types, opportunities arise for the development of both diagnostic tracers and their therapeutic counterparts, termed "theranostics." While diagnostic tracers utilize positron emitters or gamma-emitting radionuclides, their theranostic counterparts are typically bound to beta and alpha emitters, which can deliver specific and localized radiation to targets with minimal collateral damage to uninvolved surrounding structures. This is an exciting time in molecular imaging and therapy and a step towards personal

Etiological Diagnosis and Personalized Therapy for Hypertension: A Hypothesis of the REASOH Classification.

With the epidemic of risk factors such as unhealthy lifestyle, obesity and mental stress, the prevalence of hypertension continues to rise across the world. Although standardized treatment protocols simplify the selection of antihypertensive drugs and ensure therapeutic efficacy, the pathophysiological state of some patients remains, which may also lead to the development of other cardiovascular diseases. Thus, there is an urgent need to consider the pathogenesis and selection of antihypertensive drug for different type of hypertensive patients in the era of precision medicine. We proposed the
