Spontaneous Regression of a Retinal Mass With Epstein-Barr Virus-Positive and Cytomegalovirus-Positive Panuveitis in Methotrexate-Associated Lymphoproliferative Disorder.

Methotrexate (MTX) has been widely used for patients with autoimmune diseases as a corticosteroid-sparing agent. We here report an 86-year-old Japanese woman treated with MTX for rheumatoid arthritis who presented with panuveitis along with a yellowish retinal mass. Both Epstein-Barr virus (EBV) and cytomegalovirus (CMV) were detected in aqueous humor and vitreous fluid by quantitative polymerase chain reaction. The retinal mass gradually regressed over eight months after MTX withdrawal. The present case indicates that intraocular MTX-lymphoproliferative disorder (MTX-LPD) should be considered…

Red eye: diagnostic criteria, signs of alert and therapeutic approach.

"Red eye" is one of the main reasons of urgent consultation in Primary Care and in Ophthalmologic Emergencies. In many cases, the underlying cause is a benign condition that can be managed by the Primary Care Physician. However, not every red eye is a conjunctivitis, so it is important to recognize the warning signs and symptoms associated with potentially serious conditions. Causes of red eye include diseases of the ocular surface (conjunctivitis), which are generally benign; internal eye diseases (uveitis, glaucoma, scleritis, trauma) and extraocular diseases (cavernous sinus thrombosis, orb…

Bilateral Intravitreal Onchocerciasis Associated with Panuveitis: A Case Report.

PURPOSE: To report a rare case of bilateral intravitreal onchocerciasis presenting with panuveitis and managed successfully through combined medical and surgical interventions. METHODS: A 42-year-old female with bilateral panuveitis and suspected intravitreal nematodes underwent comprehensive ophthalmologic evaluation, systemic investigations, and bilateral pars plana vitrectomy. Extracted nematodes were identified through macroscopic and molecular analysis. RESULTS: The patient presented with decreased vision and panuveitis in both eyes. Vitrectomy revealed fragmented nematode tissues, late…

Complement and local ocular inflammatory response: Chronic immune factors in myopia.

Myopia has emerged as a globally pervasive visual disorder, with its incidence rising steadily, particularly among children and adolescents, posing a significant threat to ocular health. Recent research underscores a strong link between chronic inflammatory processes and both the onset and progression of myopia. As a cornerstone of the innate immune system, the complement cascade plays a pivotal role in mediating ocular inflammatory diseases associated with high myopia, including macular degeneration, glaucoma, allergic conjunctivitis, uveitis, and scleritis. Accumulating evidence indicates th…

Rare Case of Tubercular Serpiginous-Like Choroiditis.

Serpiginous choroiditis is a rare cause of posterior uveitis, included in the spectrum of white dot syndromes. It occurs as a result of an autoimmune process but could be associated with infections such as tuberculosis (TB) (serpiginous-like choroiditis). Tubercular serpiginous-like choroiditis is more commonly reported in Southeast Asian countries than in Western countries. We report a case of an Indian male in his late 30s with bilateral grey-yellowish subretinal infiltrates at the level of choroid with active scalloped edges having a positive TB-QuantiFERON Gold test (Cellestis Limited, Car…

Behçet's Disease, Pathogenesis, Clinical Features, and Treatment Approaches: A Comprehensive Review.

Behçet's disease is a systemic inflammatory disorder of unknown etiology. The disease manifests with diverse clinical symptoms, most commonly recurrent oral and genital ulcers, skin lesions, and uveitis, though it can affect multiple organ systems. Diagnosis is primarily clinical due to the lack of a definitive diagnostic test, and management involves a multidisciplinary approach to control inflammation and manage symptoms. Current treatment strategies involve corticosteroids, immunosuppressive agents, and, increasingly, biological therapies. Behçet's disease exhibits a higher prevalence along…

Behçet's disease (BD) is a rare autoimmune disease with an unknown aetiology. It is mostly found in the ancient "silk route" (from the Mediterranean region to the Far East). BD is a vasculitis that can involve veins and arteries of all sizes. Clinical features are dominated by oral and genital aphthous ulcers and uveitis. Central nervous system manifestations include parenchymal (80%) and non-parenchymal involvement (20%). Non-parenchymal forms can include cerebral venous thrombosis. Treatment is based on anti-inflammatory, immunosuppressive and anticoagulant agents, but it remains controversi…

Endovascular treatment in Behçet's disease: an integrative review.

Behçet's Disease (BD) is a rare, chronic, relapsing, inflammatory, and multisystemic disease. There is no universally described test for confirming diagnosis, so it is made clinically, on the basis of its classic triad of signs: oral ulcers, genital ulcers, and uveitis (inflammation of the uveal tract of the eye). The aim of this study is to evaluate the importance of endovascular treatment in Behçet's disease. The literature review conducted to achieve this objective covered 30 articles published between 2002 and 2021. Behçet's Disease affects both the venous and arterial systems. Rupture of …

[OCULAR MANIFESTATIONS IN BEHÇET DISEASE].

Ocular involvement in Behçet disease occurs is most patients and can be the presenting organ. Intra-ocular inflammation (uveitis) related to Behçet disease is mainly a panuveitis including an occlusive retinal vasculitis. Recurrent inflammation can result in the development of ocular complications, including macular edema and retinal neovascularization, with up to a quarter of eyes developing vision loss. Diagnosis is based on clinical ophthalmic findings and ocular imaging including retinal fluorescein angiography and optical coherence tomography. Early diagnosis and treatment may prevent the…

Vitreoretinal Lymphoma.

Vitreoretinal lymphoma (VRL) is a rare variant of primary central nervous system lymphoma (PCNSL), mostly of diffuse large B cell lymphoma, which affects the retina and/or the vitreous with or without optic nerve involvement. The disease course is aggressive. Up to 90% of the patients develop central nervous system lymphoma within one year. The diagnosis of VRL is challenging due to nonspecific chronic and relapsing uveitis and is made by anterior chamber tab or vitreous aspirate biopsy. There is no established treatment protocol for VRL patients with bilateral involvement without CNS involvem…
