Sotatercept in Children with Pulmonary Hypertension-A Narrative Review.

BACKGROUND/OBJECTIVES: Pulmonary arterial hypertension (PAH) is a rare but life-threatening disease that presents particular therapeutic challenges in children. It is characterized by pulmonary vasoconstriction and vascular remodeling, leading to right ventricular strain and eventually right heart failure. Although advances in pharmacotherapy have improved outcomes, treatment options remain limited. This review aims to evaluate the potential role of sotatercept, a novel fusion protein recently approved for adult PAH, and to assess the translatability of adult data to the pediatric population.

Relationship Between Right Ventricular Function and Chronotropic Incompetence in Pulmonary Arterial Hypertension Associated With Connective Tissue Disease.

BACKGROUND: Pulmonary arterial hypertension associated with connective tissue disease (CTD-PAH) is a progressive disease characterized by remodeling of the pulmonary vasculature, leading to increased right ventricular (RV) afterload and impaired exercise capacity. RESEARCH QUESTION: The impact of RV dysfunction on chronotropic response remains poorly understood in patients with CTD-PAH. STUDY DESIGN AND METHODS: This observational cohort study was conducted at our institution between April 2022 and March 2025. We enrolled 89 consecutive patients with CTD-PAH; 22 patients were excluded becaus

Prognostic value of soluble ST2 in cardiac magnetic resonance defined myocardial infarction with non-obstructive coronary arteries.

BACKGROUND: Cardiac magnetic resonance (CMR) plays an essential role in identifying the underlying etiology of myocardial infarction with non-obstructive coronary arteries (MINOCA). However, risk stratification in patients with CMR-confirmed MINOCA remains insufficiently defined. Soluble suppression of tumorigenicity 2 (sST2), a biomarker related to myocardial fibrosis and adverse cardiac remodeling, may provide additional prognostic information in this population. This study aimed to evaluate the prognostic value of sST2 in patients with CMR-confirmed MINOCA. METHODS: This single-center retr

Predictors of Left Ventricular Fibrosis in Mitral Annular Disjunction: Cardiac Magnetic Resonance Imaging Study.

BACKGROUND: Mitral annular disjunction (MAD) is an anatomical anomaly increasingly recognized for its association with myocardial fibrosis and malignant arrhythmias. Identifying predictors of this fibrosis is crucial for risk stratification. This study aimed to define the functional and morphological predictors of left ventricular (LV) fibrosis in patients with MAD using cardiac magnetic resonance imaging (CMR). METHODS: In a retrospective single-center study, 60 patients with CMR-confirmed MAD were evaluated. Late gadolinium enhancement (LGE) was used to identify LV fibrosis, categorizing pa

Coronary slow flow and myocardial contractile reserve: a speckle‑tracking echocardiography study.

BACKGROUND: The Coronary Slow Flow Phenomenon (CSF) is increasingly recognized as a distinct phenotype of Ischemia with Non-Obstructive Coronary Arteries (INOCA). However, debate persists regarding whether CSF represents fixed structural microvascular remodeling or functional dysregulation. Conventional Left Ventricular Ejection Fraction (LVEF) is often insensitive to the subtle subendocardial mechanics associated with this condition. OBJECTIVES: This study aimed to interrogate the functional reserve of the left ventricle in patients with angiographically defined CSF using Two-Dimensional Spe

ID2 attenuates post-MI ventricular arrhythmias by targeting GATA4 suppression to preserve mitochondrial function and Na+/K+-ATPase activity.

OBJECTIVES: Post-myocardial infarction (MI) ventricular arrhythmia (VA) remains a leading cause of sudden cardiac death. Current antiarrhythmic drugs often fail to target the core metabolic-structural remodeling underlying arrhythmias. The transcriptional repressor ID2, which has been implicated in cardiovascular development, remains poorly characterized regarding its role and contribution to VA pathogenesis. This study investigated the role of the transcriptional repressor ID2 in post-myocardial infarction ventricular arrhythmia. METHODS: An MI rat model was established via left anterior des

End-stage valvular heart disease requiring heart transplantation: risk profile, surgical modifiers, and myocardial pathology.

BACKGROUND: End-stage valvular heart disease (VHD) constitutes a small but high-risk subset of heart transplant (HTx) recipients, and data describing their natural history and determinants of progression to HTx are limited. We aimed to characterize the clinical, genetic, surgical, and pathological substrates of patients undergoing HTx for end-stage VHD and to assess whether timely cardiac valvular surgery (CVS) mitigates progression to end-stage heart failure. METHODS: We retrospectively screened 961 consecutive recipients who underwent HTx for end-stage heart failure between January 2012 and

Electromechanical Profiling in Genotyped Dilated Cardiomyopathy with Left Bundle Branch Block.

AIMS: Genetic testing is routinely recommended in dilated cardiomyopathy (DCM), yet the prevalence and implications of pathogenic/likely pathogenic (P/LP) variants in patients with DCM and left bundle branch block (LBBB) remain unclear. We therefore investigated the electromechanical profile of genotyped patients with DCM and LBBB, and its relationship with cardiac resynchronization therapy (CRT) response and clinical outcomes. METHODS: Patients with LBBB were selected from a multicenter cohort of 1206 consecutive DCM patients undergoing genetic testing. All underwent sequencing of 20 clinica

Imaging signatures of high-risk arrhythmogenic genotypes in dilated cardiomyopathy.

BACKGROUND: Dilated cardiomyopathy (DCM) exhibits substantial genotypic heterogeneity, paralleled by diverse cardiac magnetic resonance (CMR … tinct CMR tissue phenotype marked by more extensive myocardial injury and fibrosis, despite similar ventricular remodeling and systolic impairment. These findings suggest that CMR tissue characterization may help identify genotype-associated phenotypic differences in DCM.

AI-Derived Pericardial Effusion Volume and Long-Term Mortality After Transcatheter Aortic Valve Implantation.

Background: Pericardial effusion is a common finding on pre-procedural computed tomography (CT) in patients undergoing transcatheter aortic valve implantation (TAVI). However, its clinical significance remains uncertain. Using artificial intelligence (AI)-based image analysis, we sought to determine whether pericardial effusion represents an incidental imaging finding, a marker of hemodynamic burden, or a prognostically relevant phenotype. Methods: This retrospective single-center study included 470 consecutive patients undergoing TAVI for severe aortic stenosis. Pericardial effusion volume wa

Oral Relaxin Receptor Agonist AZD5462 in Participants With Chronic Heart Failure: Primary Results From the LUMINARA Trial.

BACKGROUND: Stimulation of the relaxin family peptide receptor 1 experimentally reduces systemic vascular resistance and afterload, improves organ perfusion, and fosters reverse cardiac remodeling. We hypothesized that AZD5462, an oral once-daily relaxin family peptide receptor 1 agonist, may benefit individuals with chronic heart failure (HF). METHODS: In this international, multicenter, double-blind, placebo-controlled, dose-ranging trial, adults with chronic HF and left ventricular ejection fraction ≤35% (cohort A) or 41% to 55% (cohort B) were randomized 1:1:1:1 to 3 (20, 80, or 360 mg) o

A Challenging Case of Wild-Type Transthyretin Amyloidosis (ATTR) Amyloidosis Treated With Cardiac Resynchronization Therapy.

Wild-type transthyretin amyloidosis (ATTRwt) is an age-associated systemic disorder characterized by extracellular deposition of misfolded transthyretin amyloid fibrils, leading to progressive organ dysfunction. Cardiac involvement is common and may result in restrictive cardiomyopathy, arrhythmias, and conduction disturbances, including left bundle branch block (LBBB). Although pharmacological therapy with transthyretin stabilizers, such as tafamidis, has been shown to reduce mortality in patients with ATTRwt cardiac amyloidosis (ATTRwt-CA), the role of device-based therapies, such as cardiac

Impact of Finerenone in Patients with Heart Failure and Reduced/Mildly Reduced Ejection Fraction, Diabetes Mellitus, and Chronic Kidney Disease.

Background: Heart failure (HF) remains a major global health challenge with substantial morbidity, mortality, and healthcare burden. Finerenone, a novel non-steroidal mineralocorticoid receptor antagonist (MRA), has demonstrated cardiovascular and renal benefits in patients with diabetes mellitus (DM) and chronic kidney disease (CKD), as well as reduced HF events, in patients with HF with preserved or mildly reduced ejection fraction (HFpEF/HFmrEF). However, its efficacy and safety in patients with HF with reduced EF (HFrEF) or HFmrEF, DM, and CKD, remain unclear. Notably, finerenone is curren
