OBJECTIVE: To evaluate the fetal middle cerebral artery Doppler waveform for the prediciton of anemia in the RhD-alloimmunized fetus.
STUDY DESIGN: Doppler velocimetry of the fetal middle cerebral artery peak systolic velocity was measured in 52 non-hydropic, RhD-alloimmunized fetuses who underwent 103 cordocenteses. We report a case of an intra-abdominal abscess caused by Listeria monocytogenes in a postoperative patient with Evans syndrome (acquired hemolytic anemia and thrombocytopenia). Focal infections with L. monocytogenes are uncommon but have been reported in immunocompromised patients. A few cases of liver abscess in diabetic patients have also been reported. The present case is significant because of the paucity of previously described focal intra-abdominal infections caused by L. monocytogenes, particularly as a postsplenectomy pathogen.

A 7-month-old infant with cutaneous anthrax developed severe systemic illness despite early treatment with antibiotics. The infant displayed severe microangiopathic hemolytic anemia with renal involvement, coagulopathy, and hyponatremia. These findings are unusual with cutaneous anthrax, but have been described in illness resulting from spider toxin and may delay correct diagnosis. The systemic manifestations of the disease persisted for nearly a month despite corticosteroid therapy, but resolved.

Toxic epidermal necrolysis (TEN) and severe aplastic anemia (SAA) are rare nonpredictable side effects of several drugs. To our knowledge there are no reports on the coexistence of these two disorders. This study presents a 23 year-old man with TEN diagnosed 6 days after treatment with aminophenazone, paracetamol and phenoxymethylpenicillin. The resolution of the skin disorder was observed after few days of treatment with G-CSF and cyclosporin A. In contrast, SAA only partially responded to treatment with these agents.

A 23-year-old female with polymyositis received high dose intravenous immunoglobulin (IVIg) therapy. The patient suffered severe hemolytic anemia after receiving first course of IVIg infusion. This adverse reaction was likely due to allohemaglutinin A and B and from or high molecular weight IgG complexes contained in the preparation. Though this effect was observed, the treatment was repeated six times. A mild hemolysis occurred following each IVIg, with no clinical consequence. Involvement of the saturation of macrophagic receptors might explain this partial destruction of erythrocytes.

Aplastic anaemia is a potentially fatal haematopoietic disorder whose aetiology is not yet clarified. In our preliminary study we have introduced cyclosporin in the aplastic anaemia treatment to evaluate its effect on the disease evolution. Ten aplastic anaemia patients, mean age 33.33 +/- 20.01 years, were treated with cyclosporine (9 +/- 2.35 mg/kg/d), prednisolone (0.5 mg/kg/d) and androgens (1 mg/kg/d).
