Murine Lyme borreliosis is characterized by arthritis and carditis that are most severe at 2 to 3 wk, then regress during the course of persistent infection. Borrelia burgdorferi-specific Abs and CD4+ T cells have been implicated in the resolution phase of arthritis. Therefore, MHC class II transactivator (CIITA)-deficient mice that do not express conventional class II molecules and lack the normal CD4 repertoire were used to investigate the role of MHC class II-mediated responses in Lyme disease. The development of arthritis and carditis, and the resolution of arthritis, were similar in CIITA

Shortly after the discovery of the hepatitis C virus, it was realized that this infectious agent caused more than just liver disease. A remarkable array of extrahepatic manifestations of hepatitis C has now been described. Many of these associated syndromes implicate the hepatitis C virus as a mediator of autoimmunity or of immune complex formation. These disorders include mixed essential cryoglobulinemia, autoimmune hepatitis, glomerulonephritis, thyroiditis, and possibly Sjogren's syndrome. The hepatitis C virus has also been strongly linked to two skin disorders: prophyria cutanea tarda and

A total of 35 patients with late stage Lyme borreliosis with involvement of the joints was followed up until 3 years after a 14 day course of 2 g ceftriaxone once daily i.v. Diagnosis was confirmed by indirect and direct microbiological methods as well as clinical signs and symptoms. Long term clinical results in 26 patients at 36 months were complete response or marked improvement in 19, relapse in six and new manifestations in four of the cases, respectively. Possible mechanisms for non-responding to therapy are discussed. Therapy was well tolerated; in no case discontinuation of treatment w

BACKGROUND: The aim of this study was to know the clinical characteristics of osteoarticular infection by Salmonella non typhi (SNT) in Spain.
PATIENTS AND METHODS: The medical records of 14 patients with osteoarticular infection by SNT from January 1970 to December 1992 were reviewed.
RESULTS: Over the above mentioned 23 years, 1334 patients had been diagnosed with one or more SNT isolations, with 14 (1%) having osteoarticular infection. Seven patients had septic arthritis, 3 osteomyelitis and 4 mixed infection (arthritis and osteomyelitis). Thirteen patients (93%) presented with underlying

OBJECTIVE: To describe clinical and serologic features of Lyme borreliosis, particularly the joint abnormalities, in Russian patients.
METHODS: Physicians were invited to refer patients with past or present erythema migrans to the Rheumatology Institute in Moscow; field studies were done of residents in an area endemic for Lyme borreliosis, and a serosurvey was conducted of patients with nonspecific neurologic, cardiac, or joint symptoms. Serologic testing was done by ELISA and Western blotting using a strain of Borrelia afzelii, a member of the B. burgdorferi sensu lato complex, recovered fr

The role of IFN-gamma in the regulation of host resistance of Staphylococcus aureus was studied using IFN-gamma receptor-deficient (IFN-gamma R-/-) mice in a model of S. aureus-induced septicemia and arthritis. IFN-gamma R-/- mice and wild-type controls were inoculated intravenously with a toxic shock syndrome toxin-1-producing S. aureus LS-1 strain. IFN-gamma R-/- mice displayed significantly more frequent and more severe arthritis compared with wild-type littermates (p < 0.01) throughout the course of infection. Notably, IFN-gamma R-/- mice developed severe sepsis with high mortality early a

The medical records of 192 horses with septic arthritis/tenosynovitis 1979-1989 were reviewed. Forty-three horses developed infection after an intra-articular injection, 46 following a penetrating wound, 25 following surgery, 66 were foals less than 6 months old, and 12 were adult horses without a known aetiology. Haematogenous infection of a joint occurs in adult horses and should be considered as a differential diagnosis in horses with an acute onset of severe lameness. The aetiology of the infection had a significant effect on the type of bacteria identified by culture. Staphylococcus was c

In the period from 1977 to 1989 105 arthrolyses were performed in 102 patients. There were mainly posttraumatic fibroarthroses: 51 consecutive to fractures, 39 after ligamentous injuries, 13 post-infectious of which 11 were also post-traumatic and only 2 patients with rheumatological diseases. Remarkable is the increased number of fibroarthrosis after polytrauma (n = 26). The preoperative range of motion was under 30 degrees in 26 patients and 48 patients had a severely limited range of motions between 30 and 60 degrees. The follow-up examination performed in an average of 2 years after the op

Twenty-three patients who required resection arthroplasty because of pyogenic infection of a hip joint were reviewed. Nine of the patients were ambulatory, and the others were either paraplegic or bed-ridden because of chronic neurologic disease. The average duration of symptoms was more than two months. The only consistently abnormal laboratory test was the erythrocyte sedimentation rate (ESR). Joint-space narrowing and bone erosion due to osteomyelitis were the most common roentgenographic findings. Different microorganisms were isolated from the different cases, but Staphylococcus aureus wa

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1988eng
Nine cases of septic arthritis following arthroscopy are reviewed retrospectively. All cases of septic arthritis followed arthroscopic surgery. The average age of the patients was 49 years. The diagnosis was confirmed by bacterial cultures of the joint aspirates. All cases were treated by appropriate antibiotics supplemented by repeat arthroscopy and placement of a suction irrigation system. The average stay in hospital was 21 days. The authors suggest that it is not possible to predict such a major complication based on risk factors and therefore present a cost/benefit analysis of antibiotic

A total number of 204 patients presenting with an acutely or subacutely painful hip joint were investigated in two prospective studies. Intraarticular fluid collections were detected in 104 patients. The joint effusions showed different echopatterns depending on etiology and duration of disease. Additional diagnostic criteria such as synovial hypertrophy and thickening of the articular capsule allowed the distinction between transient synovitis and septic arthritis. Bony changes were encountered in 40 patients with osteomyelitis, slipping femoral epiphysis or Perthes-Calvé disease. Ultrasonogr

The incidence of antibodies to Epstein-Barr nuclear antigen type 2 (EBNA-2) was determined in sera from rheumatoid arthritis (RA) patients and control subjects, by protein immunoblotting. Sixty-eight percent of the RA patients and 48% of the controls possessed anti-EBNA-2 antibodies. The titer of anti-rheumatoid arthritis nuclear antigen (RANA) in RA patient sera showed a stronger correlation with serum reactions to EBNA-2 than with reactions to EBNA-1. Our results indicate that the presence of EBNA-2 may make a major contribution to the RANA reaction.

Twenty-six patients with suspected or known septic joints who had been studied by computed tomography were retrospectively reviewed. Eighteen patients (20 joints) proved to have septic arthritis. Computed tomography was advantageous in patients with acute septic arthritis of deep-seated axial joints, such as the hip and sacroiliac, where it demonstrated particular soft tissue swelling as early as 36 hours after the onset of symptoms. In three patients, this finding on computed tomography was helpful in accurately localizing the disease process and facilitating a diagnostic closed needle aspira

The multiple nature of heterophile, Paul-Bunnel (P-B) antigen has been suggested by our previous studies as well as those of others. In the present study, two distinct antigens of P-B specificity were defined by means of hemagglutination and immunodiffusion tests in agarose gel; one antigen (BS) was shared by bovine (BRBC) and sheep red blood cells (SRBC) and another antigen (B) was limited to BRBC. Both anti-BS and anti-B antibodies were shown to be present in sera of 106 patients with infectious mono-nucleosis (IM) whereas they were virtually absent from the sera of the vast majority of pati

BACKGROUND: Biologic and targeted immune-modulating agents are increasingly used to manage autoimmune diseases like rheumatoid arthritis, psoriasis, and inflammatory bowel disease. As more patients receiving these therapies undergo elective breast augmentation and implant-based reconstruction, the effect of biologic exposure on postoperative outcomes remains unclear.
OBJECTIVES: This study evaluated whether preoperative use of biologic or immune-modulating agents is associated with increased complications following reconstructive and aesthetic breast surgery procedures.
METHODS: A retrospect

PURPOSE: Low body mass index (BMI) is an independent risk factor for morbidity and mortality, yet patients who have a BMI < 20 undergoing total hip arthroplasty (THA) have attracted little attention. This paper investigates the incidence of (1) mechanical complications (periprosthetic fracture (PPFX), aseptic loosening, and aseptic revision); (2) infectious complications (prosthetic joint infection (PJI) and surgical site infection (SSI)); and (3) venous thromboembolism (VTE) in this population up to two years postoperatively.
METHODS: A national all-payer database identified THA patients fro
