Predictive factors for postoperative deformity in thoracolumbar burst fractures: a statistical approach.

OBJECTIVE: To assess, using statistical analysis, if and to what extent the final outcome of surgical treatment for burst fractures depends on operation type, fracture level and initial deformity severity. METHODS: A database of 287 patients with single-vertebral-level thoracic and lumbar spine fractures analysed using simple and multiple linear regression analyses models. The dependent variable was last follow-up (LFU) kyphotic angle and the predictor variables were operation type [anterior approach (AA), posterior short-segment fixation (PSSF) and posterior monosegmental fixation (PMF)], fr

Titanium screw entered into maxillary sinus: a rare incident during rigid fixation of the porous polyethylene implant in enophthalmos correction.

Screw fixation is used for accurate augmentation by porous polyethylene implant in traumatic enophthalmos correction to avoid complications such as migration and protrusion. We report an incident of titanium screw entered into the maxillary sinus during enophthalmos correction with porous polyethylene implant. Such incident could be avoided by standard manipulation. We here present the rare case and offer proposals for the screw fixation of porous polyethylene implant during traumatic enophthalmos correction.

Biomechanical stability according to different configurations of screws and rods.

STUDY DESIGN: Comparison of biomechanical strength according to 2 different configurations of screws and rods. OBJECTIVE: To compare the biomechanical strength of different configurations of screws and rods composed of the same material and of the same size. SUMMARY OF BACKGROUND DATA: Many complications related to instrumentation have been reported. The incidence of metallic failure would differ according to the materials and configurations of the assembly of the screws and rods used. However, to our knowledge, the biomechanical effects of implant assembly rods and screws with different con

The biomechanics of a multilevel lumbar spine hybrid using nucleus replacement in conjunction with fusion.

BACKGROUND CONTEXT: Degenerative disc disease is commonly a multilevel pathology with varying deterioration severity. The use of fusion on multiple levels can significantly affect functionality and has been linked to persistent adjacent disc degeneration. A hybrid approach of fusion and nucleus replacement (NR) has been suggested as a solution for mildly degenerated yet painful levels adjacent to fusion. PURPOSE: To compare the biomechanical metrics of different hybrid implant constructs, hypothesizing that an NR+fusion hybrid would be similar to a single-level fusion and perform more natural

Less invasive palliative surgery for spinal metastases.

BACKGROUND: There may be patients with spinal metastasis for whom neither vertebroplasty/kyphoplasty nor open surgery is appropriate. Percutaneous pedicle screw fixation (PPSF) may fill the gap between techniques. OBJECTIVES: To assess the outcome of the PPSF and radiotherapy/chemotherapy. METHODS: PPSF was performed for 16 patients (mean age, 57  ± 12 years) with pathologic fractures, Tomita score ≥ 5, 3-6 months of life expectancy, minimal epidural tumor extension and a Nurick grade better than 3. PPSF was performed under general anesthesia. Collapsed vertebra(e) was restored during assemb

How is the trachea at risk of injury from pedicle screw insertion in proximal thoracic curve of adolescent idiopathic scoliosis patients?

PURPOSE: The rotation or translation of vertebrae in adolescent idiopathic scoliosis (AIS) patients could cause the relative migrations of surrounding vital structures and lead to smaller safe zones for pedicle screw insertion. This study aimed to determine the changed relative position of trachea to spine in the proximal thoracic curve (T1-T4) and to analyze the potential risks of tracheal injuries from pedicle screw insertions in AIS patients. METHODS: Twenty-three patients with complete proximal thoracic curve (CPT group), 25 patients with fractional proximal thoracic curve (FPT group) and

The fate of syndesmotic screws.

BACKGROUND: A standard protocol for the management of syndesmosis injuries has yet to be established. Debate persists regarding number of screws, screw diameter, number of cortices purchased, and the need for and timing of screw removal. The purpose of this study was to identify factors related to screw fixation that may lead to the ultimate failure of syndesmosis fixation defined as a loss of reduction of the syndesmosis, screw breakage, screw loosening, or widening of the medial clear space. MATERIALS AND METHODS: A retrospective assessment of 137 consecutive patients who underwent open red

Iatrogenic radial nerve injury with cannulated fixation of medial epicondyle fractures in the pediatric humerus: a report of 2 cases.

UNLABELLED: Surgical treatment for medial epicondylar fractures is controversial, and indications depend on variety of variables, including the degree of displacement and future goals of the patient. If surgery is to be performed, typically open reduction of the medial epicondylar fragment, with visualization of the ulnar nerve, and fixation is undertaken. Pins, screws, and cannulated screws have all been described for fixation. The use of cannulated screw systems, especially when coupled with fluoroscopy, has many advantages, and is generally considered a safe way to place a screw accurately

Anatomy of subaxial cervical foramens: the safety zone for lateral mass screwing.

INTRODUCTION: The purpose of this study was to measure the structures of the ventral of lateral masses using cadaver specimens and to quantitatively compare the safety zone for the two major techniques used on each vertebral level from C3 to C6. METHODS: This study is based on 52 cervical vertebrae of 13 cadavers. The anatomical measurements focused on the anterior surface of the lateral mass. We investigated the safety width, heights, and the height of nerve roots. RESULTS: The mean values of the safety width of the Magerl technique from C3 to C6 were 6.1, 7.3, 6.4 and 4.3 mm, respectively.

A technique of anterior screw removal through a posterior costotransversectomy approach for posterior-based osteotomies.

STUDY DESIGN: Case report. OBJECTIVE: To describe a novel technique to remove anterior instrumentation from a posterior approach while performing posterior-based osteotomies for spinal deformities. SUMMARY OF BACKGROUND DATA: Posterior-based osteotomies such as pedicle subtraction osteotomies (PSOs) and vertebral column resections are performed to restore sagittal alignment. The removal of previously placed anterior implants at the desired osteotomy level can often be challenging. We propose a technique for the removal of anterior instrumentation through a posterior approach to facilitate os

Screw fixation of radial head fractures: compression screw versus lag screw--a biomechanical comparison.

INTRODUCTION: Secondary loss of reduction and pseudarthrosis due to unstable fixation methods remain challenging problems of surgical stabilisation of radial head fractures. The purpose of our study was to determine whether the 3.0mm Headless Compression Screw (HCS) provides superior stability to the standard 2.0 mm cortical screw (COS). MATERIALS AND METHODS: Eight pairs of fresh frozen human cadaveric proximal radii were used for this paired comparison. A standardised Mason II-Fracture was created with a fragment size of 1/3 of the radial head's articular surface that was then stabilised ei

Bilateral slipped capital femoral epiphysis: a case report and review of literature.

Slipped capital femoral epiphysis (SCFE) is a common adolescent problem in the western populations but it is extremely rare in the Asian regions. We report a case management of a teen age boy (16 years old at the time of operation) with bilateral SCFE. After fall injury, the boy sustained SCFE on the left side and was treated. Again after six months with trivial injury he got SCFE on the right side. In both cases the boy was managed by closed reduction and percutaneous cannulated cancelleous screws fixation under c-arm fluoroscopy control. The cannulated screws were removed after one year of i

Surgical treatment for unstable low-grade isthmic spondylolisthesis in adults: a prospective controlled study of posterior instrumented fusion compared with combined anterior-posterior fusion.

BACKGROUND CONTEXT: The surgical treatment for low-grade isthmic spondylolisthesis in adults with intractable lumbar pain is usually spinal fusion. It has been postulated that anterior column reconstruction may be relatively advantageous in those patients with unstable slips. PURPOSE: To compare the early and medium term treatment efficacy of two common fusion techniques in isthmic spondylolisthesis. STUDY DESIGN/SETTING: Prospective controlled trial comparing single-level posterior-lateral instrumented fusion with combined anterior and posterior-lateral instrumented fusion in sequential mat

Esophageal injury from thoracic pedicle screw placement in a polytrauma patient: a case report and literature review.

The authors present a case report illustrating a visceral complication that may occur as result of thoracic pedicle screw placement. The case describes the previously unreported occurrence of esophageal impingement secondary to anterior vertebral body perforation by a pedicle screw at the third thoracic vertebra. This case highlights the challenge of thoracic pedicle screw placement and the importance of preoperatively measuring the maximum anterior-posterior dimension of the vertebral body.

Pars screw fixation of a hangman's fracture: technical case report.

OBJECTIVE AND IMPORTANCE: Traumatic spondylolisthesis of the axis may be treated by external immobilization or surgical fixation. CLINICAL PRESENTATION: We report the case of a 23-year-old man who sustained an Effendi Type II fracture of the axis, for which halo immobilization did not provide adequate stability. INTERVENTION: The unstable fracture was treated by placing lag screws in the pars interarticularis of C2, which reduced the fracture directly but sacrificed no normal spinal motion. The patient developed a solid fusion, and cervical alignment was normal at his 6-month follow-up exami
